В настоящее время в оториноларингологии известны и широко применяются различные способы лечения больных, связанные с заболеваниями голосового аппарата паралитического генеза. Технической задачей, решаемой нами, является разработка хирургического способа лечения срединных стенозов гортани, осуществляемого в зависимости от клинической картины стеноза, в частности его степени.The technical problem that we solve is the development of a surgical method for the treatment of median stenosis of the larynx, carried out depending on the clinical picture of stenosis, in particular its degree.

Для понимания сущности предлагаемого нами способа необходимо учесть следующее. Несмотря на то, что диагностика анатомических и функциональных изменений при стенозе гортани и трахеи поставлена достаточно четко, однако классификация паралитических стенозов гортани имеет свои особенности, связанные с трудностью интерпретации клинической картины, оцениваемой по внешним проявлениям стеноза (хриплость голоса и т.д.). Поэтому нами в качестве критерия стеноза использовалась величина раскрытия голосовой щели, что согласуется с известными подходами, применяемыми для оценки стеноза гортани [7].To understand the essence of our proposed method, it is necessary to consider the following. Для исследования гортани проводится анестезия глотки 10% раствором лидокаина, устанавливается ларингоскоп и производится осмотр голосовой щели. Если размер голосовой щели 6-7 мм или 4-5 мм, то присваивают I или II степень стеноза гортани соответственно. Под нашим наблюдением с 2007 по 2011 находилось 33 пациента с диагнозом: двусторонний периферический паралич гортани, перенесших струмэктомию по поводу различных заболеваний щитовидной железы. Сроки после струмэктомии составляли от 6 месяцев до 3-х лет. Основные жалобы пациентов были на выраженное затруднение дыхания и изменение голоса. Части больных в связи с декомпенсацией дыхания ранее была произведена трахеотомия. В зависимости от результатов осмотра больные разделялись на две группы - со стенозом I и II степени и стенозом III степени. Исходя из этих параметров, больным производилась двусторонняя (пациентам со стенозом III степени) либо односторонняя (пациентам о стенозом I-II степени) задняя хордэктомия. В результате проведения операции достигнуты хорошие результаты. Послеоперационный период проходил без осложнений. При контрольном осмотре: состояние больных удовлетворительное, дыхание через естественные дыхательные пути свободное. Фонация удовлетворительная. При последующем осмотре через 1-1,5 года состояние больных удовлетворительное, дыхание через естественные дыхательные пути свободное. Больная Л., 51 г, поступила в НКЦО 20.06.12 с жалобами на одышку, чувство нехватки воздуха при минимальной физической нагрузке. Из анамнеза: в 2007 г. по поводу рака щитовидной железы выполнена струмэктомия. В настоящее время госпитализирована для хирургического лечения. При эндоскопическом осмотре определяется двусторонний абдукционный паралич гортани, просвет сужен до 4 мм, складки при фонации неподвижны. Степень стеноза II. 22.06.12 больной выполнена операция: Задняя хордэктомия левой голосовой складки СO2-лазером. Под общей анестезией установлен подвесной ларингоскоп, далее СО2-лазером мощностью 1 Вт в непрерывном режиме произвели иссечение задней трети левой голосовой складки с резекцией голосового отростка черпаловидного хряща слева. Послеоперационный период прошел без осложнений. При контрольном осмотре: состояние больной удовлетворительное, дыхание через естественные пути свободное. При эндоскопическом осмотре: двусторонний абдукционный паралич гортани, просвет сужен до 2-3 мм, голосовые складки при фонации неподвижны. 16.11.11 выполнена операция: Двусторонняя задняя хордэктомия с применением СО2-лазера. Под общей анестезией установлен подвесной ларингоскоп, далее СО2-лазером мощностью 1,5 Вт, в непрерывном режиме произвели иссечение задних третей обоих голосовых складок с резекцией голосовых отростков черпаловидных хрящей. Последующий операционный период протекал без осложнений.
