Для обезболивания в активную фазу первого периода родов применяют различные комбинации анальгетиков (промедол, трамал, фентанил) и седативных средств (димедрол, седуксен, дроперидол). Однако при преэклампсии нежелательно глубокое седативное воздействие на мозг (опасно и неэффективно). Принципиальным правилом обезболивания родов является адекватное, раннее, поэтапное обезболивание, углубляемое по мере развития родовой деятельности, что возможно при эпидуральной анестезии. При таком обезболивании нормализуется общая гемодинамика роженицы; отсутствует угнетающее действие на плод (новорожденного); снижается среднее артериальное давление; увеличивается приток крови в межворсинчатый отдел плаценты. With epidural anesthesia, this test of loss of resistance is most useful for determining the location of the needle lumen. A solution of local anesthetic of 22-26 ml is introduced into the puncture zone of the epidural space. To ensure continuous infusion of the anesthetic, catheterization of the epidural space with the introduction of a catheter through the lumen of the epidural needle to a depth of 3 cm is possible.

Известен способ медикаментозной подготовки к родам беременных группы риска по развитию аномалий родовой деятельности, при котором пациентам назначают лекарственное вещество мифепристон (мифегин), однократно, в дозе 200 мг. Через 12 часов после приема мифепристона в цервикальный канал вводят препидил-гель. Такой режим введения препаратов обеспечивает полноценную готовность всех отделов репродуктивной системы к родам в течение суток, что позволяет снизить уровень осложнений в родах и в послеродовом периоде у женщин, уменьшить количество экстренных оперативных вмешательств в конце первого и во втором периодах родов. Для осуществления такого способа беременные на сроке 38-39 недель должны быть госпитализированы в акушерский стационар для тщательно обследования. При обследовании беременных уточняют форму и - что очень важно - размеры таза, степень его сужения. Определяются положение, предлежание, предполагаемая масса плода, размеры головки по результатам УЗИ, оцениваются состояние родовых путей и соответствие головки плода тазу матери. Выявляют сопутствующую акушерскую и экстрагенитальную патологию. Оценка «зрелости» шейки матки проводилась по шкале Бишопа. Для подготовки шейки матки к родам использовался мифепристон - синтетический антигестаген, который блокирует действие прогестерона на уровне рецепторов, вызывает активную миграцию лимфоцитов и макрофагов в ткань миометрия, увеличивает секрецию иммунокомпетентными клетками, вызывает повышение активности лизосомального аппарата миоцитов, увеличивает активность металлопротеиназ, в результате чего происходит перестройка межклеточного матрикса и созревание шейки матки. Мифепристон назначался в дозе 200 мг беременным, у которых оценка по Бишопу была ниже четырех баллов.
