Пищевая аллергия (ПА) является одной из актуальнейших проблем современной педиатрии и аллергологии. Многообразие клинических проявлений, отсутствие общепринятой классификации, возможности «безграничной интерпретации неизвестных» механизмов реакций на пищевые продукты сегодня зачастую приводят к сомнительному диагнозу и неоправданным ограничениям рациона питания. Сложность проблемы заключается и в том, что непереносимость пищевых продуктов может быть обусловлена различными механизмами. Так, под термином пищевая аллергия в настоящее время объединяется группа Ig E-зависимых и Ig E-независимых заболеваний. Наибольшую трудность представляет диагностика Ig E-независимой ПА в связи с отсутствием стандартизированных реагентов и методов [2,3,9]. Критерии диагностики патологии, сопровождающейся явлениями Ig E-независимой гастроинтестинальной пищевой аллергии, основываются на анализе данных анамнеза, комплексного клинико-лабораторного и инструментального исследования [3, 8]. Установление точного диагноза особенно важно у детей первых лет жизни, поскольку ошибочный диагноз может привести к развитию жизнеугрожающих реакций или назначению необоснованных ограничительных диет.Food allergy (PA) is one of the most pressing problems of modern pediatrics and allergology. The variety of clinical manifestations, the lack of a generally accepted classification, the possibility of a “limitless interpretation of unknown” mechanisms of reactions to food today often lead to a dubious diagnosis and unjustified dietary restrictions. The complexity of the problem lies in the fact that food intolerance can be due to various mechanisms. So, under the term food allergy, the group of Ig E-dependent and Ig E-independent diseases is currently combined. The greatest difficulty is the diagnosis of Ig E-independent PA due to the lack of standardized reagents and methods [2,3,9]. The criteria for the diagnosis of pathology, accompanied by the phenomena of Ig E-independent gastrointestinal food allergy, are based on the analysis of medical history, a comprehensive clinical, laboratory and instrumental study [3, 8]. Establishing an accurate diagnosis is especially important in children of the first years of life, since an erroneous diagnosis can lead to the development of life-threatening reactions or the appointment of unreasonable restrictive diets.
Общепринятым способом диагностики Ig Е-независимой ПА на настоящий момент является провокационная диетодиагностика [3]. Данный способ носит косвенный характер, недостаточно информативен и специфичен, диагностический процесс растянут во времени, кроме того, повторная провокация в диагностических целях нежелательна. Недостатками данного способа является необходимость проведения исследования в период ремиссии заболевания, необходимость длительного соблюдения строгих необоснованных элиминационных мероприятий, нередкое развитие дефицитных состояний, особенно у детей раннего возраста, обострение процесса в случае положительных результатов данного метода диагностики.The generally accepted method for the diagnosis of Ig E-independent PA is currently provocative diet diagnostics [3]. В качестве диагностического критерия используют концентрацию метаболитов N0 в копрофильтрате. В последние годы значительно расширились представления о роли NO и его метаболитов в различных физиологических и патологических процессах, в патогенезе различных заболеваний, в том числе и желудочно-кишечного тракта [1, 7, 8]. Среди физиологических функций NO в отношении пищеварительной системы наиболее важной является обеспечение моторной функции желудочно-кишечного тракта, а также регуляции поступления желчи в кишечник [1]. Кроме того, NO следует отнести к числу наиболее важных факторов защиты слизистой ЖКТ [8]. Его влияние осуществляется путем воздействия на кровоснабжение слизистой. NO при этом может выполнять регуляторные и эффекторные функции, оказывая протективный или тканеповреждающий эффект на разных стадиях иммунного ответа [9]. Очевидно при этом, что как недостаток, так и избыток NO играет значимую роль в повреждении тканей, развитии целого каскада иммунопатологических реакций и других нарушений. Авторами настоящей заявки впервые обнаружены разнонаправленные изменения продукции NO при Ig E-независимой ПА и других заболеваниях ЖКТ, включая Ig E-зависимые формы ПА. Блокада синтеза NO, обнаруженная нами при развитии Ig E-независимой ПА, значительно уменьшает кровоток в сосудах слизистой, косвенно это сказывается на секреторной функции ЖКТ, на способности его слизистой противостоять воздействию на нее факторов агрессии, на развитии клеточных реакций, возникновении повреждения слизистой оболочки и нарушении ее репарации [8]. Кроме того, в ранней фазе иммунного ответа NO выполняет как эффекторные, так и важные регуляторные функции, снижение эндогенного NO увеличивает апоптоз NK клеток, поддерживает их цитолитическую способность, а также препятствует продукции ими ИФНу [7]. Гиперпродукция NO, выявленная при заболеваниях ЖКТ другого генеза, в том числе на фоне Ig E-зависимой ПА, вероятно, является защитной реакцией, направленной на адаптацию к изменившимся условиям существования клетки, активации цитокинового каскада, развития гипериммуноглобулинемии Е и с переходом клеток кишечника на более экономный (нитритный) путь дыхания. The authors of this application for the first time discovered multidirectional changes in the production of NO in Ig E-independent PA and other gastrointestinal diseases, including Ig E-dependent forms of PA. The blockade of NO synthesis, which we discovered during the development of Ig E-independent PA, significantly reduces blood flow in the vessels of the mucosa, this indirectly affects the secretory function of the gastrointestinal tract, the ability of its mucosa to resist the effects of aggression factors on it, the development of cellular reactions, the occurrence of damage to the mucous membrane and violation of its repair [8]. In addition, in the early phase of the immune response, NO performs both effector and important regulatory functions, decreasing endogenous NO increases apoptosis of NK cells, supports their cytolytic ability, and also inhibits their production of IFNu [7]. Hyperproduction of NO, detected in diseases of the gastrointestinal tract of a different genesis, including against the background of Ig E-dependent PA, is probably a protective reaction aimed at adapting to the changed conditions of the cell’s existence, activation of the cytokine cascade, development of hyperimmunoglobulinemia E and with the passage of intestinal cells to more economical (nitrite) way of breathing. On the other hand, the formed biologically active molecules - NO, peroxynitrite, reactive oxygen species have a toxic effect on intestinal epithelial cells, their enzymatic systems, disrupts the intracellular stage of digestion, and aggravate inflammatory changes in the mucous membrane during a long process [9].

Группу сравнения составили 30 здоровых детей, сопоставимых по полу и возрасту, с неотягощенным аллергологическим анамнезом, пациенты с IgE зависимой ПА (n=18), имеющие повышенный уровень IgE (Ме=88.25; 95% CI: 52.18-184.98) и специфические IgE к БКМ; 1Б - пациенты с IgE - независимой ПА (n=21), у которых уровень общего и специфических IgE был сопоставим с контрольной группой и достоверно ниже, чем в подгруппе №1А (Ме=15.0; 95% CI: 7.91-23.0).
