Известны различные способы диагностики пищевой аллергии [2-7]. Существует «способ диагностики гастроинтестинальной формы пищевой аллергии» у детей, заключающийся в заборе слизистой оболочки тощей кишки. Полученный биоптат окрашивают и подсчитывают количество дегранулированных и недегранулированных тучных клеток. Определяют отношение количества дегранулированных к недегранулированным и при увеличении этого отношения устанавливают гастроинтестинальную форму пищевой аллергии [3].There are various methods of diagnosing food allergies [2-7]. Известен «способ дифференциальной диагностики гастроинтестинальной пищевой аллергии». Сущность способа заключается в том, что в сыворотке крови определяют содержание пре-β-липопротеидов и β-липопротеидов. При соотношении уровней пре-β-липопротеидов к β-липопротеидам выше 0,31 дифференцируют гастроинтестинальную форму пищевой аллергии от иной патологии желудочно-кишечного тракта. Самой распространенной формой пищевой аллергии у детей грудного возраста вне зависимости от вида вскармливания, является аллергия к белкам коровьего молока (АБКМ) [1, 6-11]. Своевременная постановка диагноза вызывает определенные трудности у специалистов первичного звена ввиду отсутствия патогномоничных симптомов. Наряду с поражением кожи, желудочно-кишечного тракта, на практике встречаются изолированные гастроинтестинальные формы, сочетанные формы с поражением кожи, пищеварительной, дыхательной системы, которые зачастую приводят к ошибкам диагностики, необоснованным ограничениям питания, полипрагмазии. Согласно изобретению у детей грудного возраста оцениваются клинико-анамнестические показатели, а именно наличие аллергических заболеваний у матери, ближайших родственников в анамнезе, наличие обострения хронических заболеваний во время беременности матери, угроза прерывания беременности, обострение аллергических заболеваний во время беременности у матери, патологическое течение родов, родоразрешение путем операции кесарево сечение, нерациональное вскармливание, нерациональный режим питания ребенка, аллергический дерматит в первые месяцы жизни. Также проводится оценка возможных ранних клинических проявлений пищевой аллергии у ребенка: это наличие кишечных колик, вздутие живота, нарушение консистенции и кратности стула, наличие кожного синдрома, респираторные проявления, не связанные с острой респираторной инфекцией. Наличие клинического этапа диагностики (с использованием балльной оценки) позволит оптимизировать и унифицировать процесс отбора пациентов для дальнейшего лабораторного тестирования. Пациенту проводят оценку клинико-анамнестических показателей, при наборе более 8 баллов выделяют группу высокого риска по развитию пищевой аллергии. Согласно изобретению выставлен диагноз: аллергия к белкам коровьего молока. Результаты проведенных провокационных тестов подтвердили наличие пищевой аллергии, аллергии к белкам коровьего молока. В 3 месяца на профилактическом осмотре матери ребенка предложено заполнить анкету по определению группы риска развития пищевой аллергии у ее ребенка. По итогам анкетирования сумма баллов составила 8. Для диагностики пищевой аллергии рекомендовано пройти обследование на содержание эозинофильного катионного протеина в копрофильтратах. Эти показатели подтверждают наличие аллергического воспаления, выявленного при использовании диагностической шкалы. Согласно изобретению выставлен диагноз: аллергия к белкам коровьего молока. Результаты проведенных провокационных тестов подтвердили наличие пищевой аллергии к белкам коровьего молока. При анкетировании по определению группы риска развития аллергии получен результат в 6 баллов. С дифференциально-диагностической целью в копрофильтрате определены эозинофильный катионный протеин, который составил 417 нг/мл. При комплексном клинико-лабораторном обследовании выставлен диагноз: Функциональное заболевание ЖКТ. Дисфункция билиарного тракта. Согласно изобретению диагноз пищевой аллергии к белкам коровьего молока не подтвержден. Предлагаемый способ диагностики был применен у 30 детей грудного возраста.
