Abstract
Description
The invention relates to medicine, in particular to dentistry, can be used for splinting of movable teeth for periodontal diseases.
Relevance is due to the high frequency of treatment of patients with periodontal diseases and the use of splinting of adhesive structures.
Currently, traumatic occlusion as a syndrome resulting from an imbalance between the adaptive capabilities of periodontal supporting tissues and the forces applied to the tooth, is possible as a complication during periodontal disease, and due to other reasons. In addition, traumatic occlusion may be one of the reasons or cofactor of periodontal destruction in periodontitis. In this regard, the treatment of one disease against the background of others that are not currently in remission may not be effective enough.
Among the methods for eliminating traumatic occlusion, splinting of moving teeth is considered as one of the methods of therapeutic effect on weakened periodontium.
Indications for splinting are currently formulated as follows:
- для стабилизации зубов с повышенной подвижностью, которая не изменяется после окклюзионного регулирования и лечения заболеваний пародонта;- to stabilize teeth with increased mobility, which does not change after occlusal regulation and treatment of periodontal diseases;
- для предупреждения перемещения зубов;- to prevent the movement of teeth;
- для стабилизации зубов после ортодонтического перемещения;- to stabilize teeth after orthodontic movement;
- для стабилизации зубов после острой травмы.- to stabilize teeth after an acute injury.
According to the necessary time for their use, the tires are divided into temporary, long-term and permanent.
- 336 с. (с. 269-277)).Most often used for splinting are extra-funnel temporary tires. Being the first in historical terms, they have now evolved into a composite tire reinforced for strength with an orthodontic ligature, clasp wire, metal mesh, synthetic fiber or tape. Currently, commercial reinforcement products are represented by two groups of materials: fiberglass-based (Fiber Splint, Switzerland; Glas Span, USA) and polyethylene-based (Connekt, USA; Ribbond, USA). Such a tire is easily mounted on the teeth, not very expensive and aesthetically acceptable, since it is often located only on the lingual (palatal) surface, fixing it with composite filling materials of low viscosity using adhesive systems (1. Grudyanov AI, Periodontal disease. - M .: Publishing house "Medical Information Agency", 2009. - 336 p. (P. 269-277)).
When splinting mobile teeth in patients with periodontal diseases using adhesive technologies, the exclusion of aggressive agents (acid for etching hard tooth tissues, adhesive) on the tissue structures of the periodontal pocket, and especially on the restored epithelium of gingival attachment with achieved remission of the disease, is of particular importance. To do this, it is necessary to introduce a retraction thread into the gingival sulcus or periodontal pockets of all teeth prepared for splinting before etching hard tissues and applying adhesive, which takes considerable time and does not exclude additional mechanical injury to the protected structures that can affect the favorable course of the disease.
When splinting teeth using fiber scaffolds, there are drawbacks to this technique, namely the protrusion of the tire fibers from a composite material that irritates soft tissues, contributes to the accumulation of plaque and the formation of caries. In the event of damage to the fibers, the structural strength decreases sharply, therefore, the composite material must completely overlap the fiber bus, including interproximal surfaces.
О.О. - 160 с. (С. 67-70)).It is important that the tire does not interfere with good oral hygiene. After installing the tire, the interdental spaces should be free so that there is access for oral hygiene. To protect the interdental spaces in the process of splinting, wedges are used, but if the wedging is carried out actively, a significant excursion of the teeth can be caused, which will lead to the appearance of zones of increased stress on the structure (2. Periodontal disease. A modern look at the clinical diagnostic and therapeutic aspects: study site / Under the editorship of OO O. Yanushevich. - M.: GEOTAR-Media, 2010 .-- 160 p. (P. 67-70)).
(3).When using wedges, there is a possibility that teeth with a high degree of mobility can be chipped in the wrong position (3).
- 2012. - №4. - С. 28-32).A known method in which the excess of composite material is prevented from entering the interproximal gingival portions by passively filling the interdental spaces with an elastomeric impression material. The technique consists in isolating the teeth with a cofferdam, preparing the teeth and fiberglass tape for applying the tire, introducing a medium-viscosity polysiloxane impression material of fast curing using a special dispenser with a nozzle in the interdental spaces, while the impression material should be applied after the teeth have been etched, washed and dried to prevent trapping of moisture, splinting, followed by removal of the impression mat from the interproximal gingival areas series (3. Strassler GE Periodontal splinting using Ribbond THM // Modern dentistry. - 2012. - No. 4. - P. 28-32).
The disadvantage of this method are:
1. The application of a cofferdam to isolate the teeth can displace the movable teeth, which subsequently can lead to the appearance of zones of increased tension on the design of the tire and periodontal tissue.
2. Filling of the interdental portions with the impression material to the fiberglass tape makes it impossible to cover it with composite material, which leads to razlokivanie fiberglass splinting design.
Silicone material is applied after the start of adhesive preparation (after etching the enamel with acid).
4. The lack of protection of the cervical part of the tooth can lead to the ingress of excess material onto it, which complicates and increases the processing time of the superimposed tire.
The technical result of the claimed invention is to ensure the safety of adhesive preparation, eliminating the displacement of movable teeth and reducing the time it takes to process a splint construction.
The technical result allows to provide:
1) защиту зубодесневой борозды от попадания в нее протравливающей кислоты и адгезива;1) protection of the gingival sulcus from the penetration of etching acid and adhesive;
2) исключение смещения подвижных зубов;2) the exclusion of displacement of movable teeth;
3) исключение попадания излишков материала в межзубные промежутки и на пришеечную часть зубов;3) the exclusion of excess material in the interdental spaces and on the cervical part of the teeth;
4) создание места для закрытия шины композитным материалом в межзубных участках;4) creating a place for closing the tire with composite material in the interdental areas;
5) reduction of time for processing and polishing of the splinting structure.
A method of manufacturing a gingival interdental silicone protective template when splinting teeth with adhesive structures involves applying the silicone material from the oral side to the marginal gum.
The difference is that before applying the material, the border of the future splint design is determined by creating a groove in the enamel of the splint teeth and then silicone material is applied that overlaps the marginal gum, the neck of the teeth with the closure of the interdental spaces, while not reaching the borders of the splint design by 0.5- 1 mm.
The gingival part of the protective template protects the gingival sulcus of the splinted teeth from getting etching acid and adhesive; the cervical part of the teeth protects against excess composite material. The interdental part of the protective template protects against excess material entering the interdental spaces, prevents tooth displacement, which can be observed when using wedges. The application of the protective template, not reaching the borders of the tire by 0.5-1 mm, allows you to block the tire with composite material in the interdental areas, which ensures the safety of adhesive preparation.
The claimed technical solution is illustrated by drawings, in which:
на фиг. 1 показан вид модели с подготовленными для шинирования зубами;in FIG. 1 shows a view of a model with teeth prepared for splinting;
на фиг. 2 показан вид модели с изготовленным придесневым межзубным силиконовым защитным шаблоном;in FIG. 2 shows a view of a model with a manufactured gingival interdental silicone protective template;
на фиг. 3 - вид готовой шинирующей конструкции после удаления придесневого межзубного силиконового защитного шаблона;in FIG. 3 is a view of the finished splinting structure after removal of the gingival interdental silicone protective template;
4 - придесневой межзубной силиконовый защитный шаблон со стороны зубодесневой поверхности.in FIG. 4 - gingival interdental silicone protective template from the side of the gingival surface.
A causal relationship between the features of the claims and the technical result is as follows.
The definition of the boundaries of the protective template and the establishment of it before the start of adhesive preparation ensures the safety of adhesive preparation and subsequent splinting for periodontal tissues:
1) защищается десневая борозда от попадания в нее протравливающей кислоты и адгезива;1) the gingival sulcus is protected from contact with etching acid and adhesive;
2) исключается попадание излишков композитного материала в межзубные промежутки и пришеечную часть зубов;2) excludes the ingress of excess composite material into the interdental spaces and the cervical part of the teeth;
3) the displacement of movable teeth is excluded.
The specified technical result is achieved by the fact that when splinting movable teeth before etching, applying adhesive and applying a tire, a gingival interdental silicone protective template is pre-made.
Gingival interdental silicone protective template is made by applying a corrective A-silicone material (polymerized by the type of polyaddition) from the oral surface of the teeth and gingival margin, starting from the gingival part, closing the gingival sulcus; interdental spaces are filled perpendicular to the marginal gingival margin; the material is applied to the cervical part of the splinted teeth, not reaching the splint design by 0.5-1 mm.
The silicone template is applied from the cartridge using a dispensing gun with an intraoral cannula attached to the mixing cannula or manually kneading with a syringe to apply fluid impression material.
А. - М., 2006. - 227 с. (С. 44-48)).The use of A-silicone material as a protective template is justified by its medical and technical characteristics: A-silicones are characterized by optimal compatibility with the skin and oral mucosa, which makes them well tolerated by patients; possess thixotropy, hydrophilicity, moderately short working time and setting time; do not absorb water; they have no taste or smell (4. Ryakhovsky AN, Muradov MA Exact print. - M., 2006. - 227 p. (S. 44-48)).
Subsequent splinting of teeth is carried out according to standard methods. After splinting of teeth, the gingival interdental silicone protective template is easily removed from the oral cavity in a single block, the splinting structure is processed and polished according to the generally accepted method.
The inventive method of manufacturing a gingival interdental silicone protective template when splinting teeth with adhesive structures is as follows:
