Abstract
Description
The invention relates to medicine, in particular to therapeutic dentistry, and can be used as a stopper to block the spread and exposure of short-wave ultraviolet radiation to human biological tissues, namely to the organs of the oral cavity from the zone of interest (root canal of the tooth) during the final (final ) pre-obstructive sanitation (sterilization) of the root canal system of the tooth in the treatment of complicated forms of caries.
В., 2011).Due to the complex histomorphology of the root canal, no modification of the rotary endodontic instrument can directly mechanically clean the root canal of microorganisms and their decay products by direct contact (Abakarova D.S., 2011). In addition to the generally accepted mechanical and chemical means, in recent decades, in most countries of the world, in-channel use of physical factors has been intensively introduced into practical medicine (Olekhina O.V., 2011).
A well-known endodontic instrument for sterilizing tooth canals (Utility Model Patent No. 131609, A61C dated 19/10/06 dated 09/10/2012, author: A. Larinskaya), where the author proposes to sterilize root canals of a tooth root using UV radiation with a wavelength 254-257 nm.
A known method of atraumatic pre-obstructive sanitation of the root canal system of the tooth (Patent for invention No. 2567605, А61С 5/00, published: November 10, 2015 Bull. No. 31), where short-wave ultraviolet radiation is used in the oral cavity for sanitation of the root canal system of the tooth with endodoptic treatment of complicated forms of caries, while nothing is said about protecting the surrounding tissues of the oral cavity while working in the root canal system.
Despite the relevance of short-wave ultraviolet radiation in endodontic dentistry in order to sterilize the root canal system of the tooth, it is necessary to remember the harmful effects of this type of rays. Exposure to the skin of large doses of ultraviolet radiation (UVI) leads to skin diseases (dermatitis). Increased doses of UVI also affect the central nervous system, deviations from the norm are manifested in the form of nausea, headache, fatigue, fever, etc. (Lazarev D.N. Ultraviolet radiation and its use // L., 1950. - 18 from.). It is known that ultraviolet radiation with a wavelength of less than 0.32 microns adversely affects the retina, causing painful inflammatory processes. Already at an early stage of this disease, a person feels pain and a feeling of sand in his eyes. The disease is accompanied by lacrimation, damage to the cornea of the eye and the development of photophobia - "snow disease" is possible.
Some experts are of the opinion that when disinfecting canals with UVI, necrosis of the tissues of the oral cavity can occur.
Thus, summarizing all of the above, we can conclude that there is a need for the creation and implementation in practical dentistry of ergonomic means and objects of protecting biological tissues of the human oral cavity from the pathological effect of short-wave ultraviolet radiation (UVC) used to sterilize the root canals of the tooth.
Many means and objects of protection against ultraviolet radiation are known, but none of the known ones is positioned as a “protective agent” against the spread and penetration of short-wave ultraviolet radiation onto the organs of the oral cavity when working with UVC in the zone of interest (tooth root canal system). To protect the eyes from the adverse effects of ultraviolet rays, shields or helmets with special dark glasses, goggles are used in factories, and to protect the rest of the body and surrounding persons - with insulating screens, portable screens, work clothes.
Для защиты органов полости рта, слизистой оболочки в стоматологической практике используют «изолирующие системы», основным представителем является коффердам, представленный на фото 2 (синонимом является раббердам), он представлен тонким эластичным латексным полотном (Система Коффердам - Полонский Никита Анатольевич http.//www.polonsky.com.ua/vektor-sistema/cto-takoe-restavracia-zubov/sistema-kofferdam).It is known that in general medicine in the treatment of UVI, human skin and eyes can become a "target" for ultraviolet radiation. In dentistry, the "target" is the oral cavity,
Advantages of using insulating systems, such as cofferdam: protection of the mucous membrane, respiratory tract, gastrointestinal tract from unpleasant and aggressive dental preparations (phosphoric acid, adhesive systems, antiseptics, etc.); the exclusion of aspiration and ingestion of instruments; abduction of soft tissues, gum retraction; aseptic conditions during treatment; protection of medical staff from cross infection; work on a dry surgical field, without cotton rolls, with dry tools; excellent overview and access to the treatment facility; the exclusion of the negative effects of wet mouth breathing on the adhesion of composite materials; prevention of excessive dry mouth that occurs when using saliva ejector; the latex elasticity allows the patient to keep his mouth open for a long time. Indications for working with cofferdam are: endodontic treatment, sealing fissures, filling cavities of any classes, conducting extensive direct restorations, conducting direct splinting in the oral cavity, and teeth whitening.
The device, using the example of a cofferdam (photo 2), is the closest analogue to the achieved result. And it could isolate the organs of the oral cavity during work with short-wave ultraviolet radiation, if not for a number of contraindications to the application of this type of isolation, namely: an allergic reaction to latex, obstructed nasal breathing, diseases of the upper respiratory tract, the patient’s emotional state and the presence of non-removable orthodontic appliances (bracket systems) (http://belmapo.by/downloads/stomObschav/2009/sist_izoliacii.pdf; http://medical-diss.com/medicina/kliniko-laboratornoe-obosnovanie-primeneniya-kofferdama-v- terapevticheskoy-stomatologii). When working with the endodontic device, a short-wave ultraviolet ray (255 ± 5 nm) is emitted into the root canal system of the tooth using a fiber optic microcable, which has direct contact only with the wall of the root canal, but at the same time, to prevent the UV radiation from leaking onto the surrounding tissues of the oral cavity , provided that the patient is not included in the category of “contraindications”.
The objective of the invention is to develop a stopper to protect the organs of the oral cavity and mucous membrane from exposure to short-wave ultraviolet radiation from the zone of interest (root canal of the tooth) during the final pre-obstruction sanitation (sterilization) of the root canal system of the tooth, in the treatment of complicated forms of caries.
Технический результат - защита органов полости рта от воздействия коротковолнового ультрафиолетового излучения из зоны интереса (корневого канала зуба).The technical result is the protection of the oral cavity from exposure to short-wave ultraviolet radiation from the zone of interest (root canal of the tooth).
The above technical result is achieved due to the fact that the stopper for protecting the organs of the oral cavity from the propagation of short-wave ultraviolet radiation is made in the form of a conical base made of a light-tight biocompatible silicone material with a chrome-plated inner surface and a central hole. The stopper is tightly connected to a disposable fiber-optic tip that passes through the center hole of the stopper, being the source emitted by short-wave ultraviolet radiation (UVC) for pre-obstructive sanitation (sterilization) of the tooth root canal system in the treatment of complicated caries.
The applicant has not found sources of information containing the same set of essential features specified in the claims, which allows us to conclude that the proposed technical solution meets the criterion of "novelty."
3-4.The design of the stopper to protect the organs of the oral cavity from UVI is illustrated in the drawing (Fig. 1-3) and the attached photos 3-4.
