Abstract

FIELD: medicine.
SUBSTANCE: invention relates to medicine, specifically to immunology, and can be used for estimating human health condition. In the peripheral blood of the subject, using the flow cytofluorometry method, the percentage of Th17 lymphocytes is determined, namely the percentage of lymphocytes with the phenotype CD3+CD4+CD161+CD45R0+ from the gate of CD3+CD4+ lymphocytes. Estimated percentage of Th17 lymphocytes in peripheral blood is calculated according to the following formula: y = -0.0059x2 + 0.6819x + 1.1671, where x is the age of the subject in years, y is the calculated percentage of Th17 for a given age; the calculated and real values of the percentage of Th17 lymphocytes in the peripheral blood are compared. If they coincide or if the difference between them does not exceed 2σ, the subject is considered healthy. If the actual Th17-lymphocyte count exceeds the calculated value by more than 2σ the patient is considered ill, where σ – standard deviation.
EFFECT: use of this method allows to identify a group of healthy individuals and a group of sick persons on the basis of the criterion of the correspondence of the age-specific norm of Th17-lymphocytes in peripheral blood to those with unknown anamnesis and unclear clinical manifestations.

Description

The invention relates to the field of medical immunology, namely to immunological methods for assessing human health.

The invention can be used in medicine to confirm the state of human health. The preamble to the Charter of the World Health Organization says that health is a state of complete physical, mental and social well-being, and not just the absence of diseases and physical defects. However, in health statistics, individual health refers to the absence of identified disorders and diseases. To date, there is no standard method to clearly distinguish between health and disease. To assess the state of health, an anamnesis, a clinical examination of a doctor, thermometry, measurement of heart rate, blood pressure, results of a clinical blood test and general urine test are usually used. Other laboratory and instrumental examination methods are also used. Thus, confirmation of the state of health is a rather laborious and often lengthy process. Nevertheless, it is far from always possible to be sure that all the necessary examinations of a person have been carried out and some disease has not been missed.

It is known that the immune system is involved in the pathogenesis of almost any disease. In the process of an immune response to antigens, several subpopulations are formed from an initially homogeneous subpopulation of T-helpers (CD4 + T-lymphocytes). To date, the following subpopulations of T-helpers (Th) have been identified: Th1, Th2, Th9, Th17, Th22, follicular T-helpers and regulatory T cells. A subpopulation of Th17 lymphocytes (Th17) regulates the antibacterial and antifungal immune response and activates neutrophils, but can support autoimmune diseases [1]. The main transcription factor for the subpopulation of human Th17 is RORC2, and the main cytokines are pro-inflammatory interleukins (IL): IL-17A, IL-21 and IL-22 [2]. One of the important surface molecules characteristic of Th17 is CD161, as well as the chemokine receptor CCR6 [3]. In the process of postnatal development, long processes of formation of many parts of the immune system, including subpopulations of Th17, take place. We were unable to find studies that track changes in Th17 levels from birth to very old age. However, if we analyze the level of Th17 in the control groups in numerous studies of the subpopulation of Th17 for various diseases in people of different ages [4, 5, 6, 7], it can be noted that in healthy infants and young children Th17 are determined at the level of 1-3% [4, 5], and their number progressively increases with age [7]. Additional changes are made by various diseases.

In pathology of various origins, an increase in the level of Th17 is noted [1, 4, 5, 6]. The level of Th17 in peripheral blood rises with various autoimmune diseases [8]; infections [9]; allergic [10] and oncological diseases [11]. The dynamics of the level of Th17 in response to the introduction of viral antigens was tracked by the example of vaccination with a live virus vaccine against measles, rubella and mumps. It turned out that after the introduction of the vaccine, the amount of activated Th and Th17 in the peripheral blood rises rapidly. Upon completion of the response to the vaccine, the level of Th17 decreases, but remains slightly higher than the initial one. This excess was not significant, but such a trend was observed [12]. Specific Th17 lymphocytes appear to be formed during each immune response, and after its completion some small portion of Th17 formed in this immune response turns into Th17 memory and remains in the body, which explains the gradual increase in the Th17 subpopulation with age. Thus, we can conclude that with diseases of various origins, the formation of Th17 occurs, the level of these cells in peripheral blood rises. This phenomenon can be used to separate health status and disease.

Th17.We were unable to find direct analogues of the claimed invention. The closest technical solution is described in the article N.E. Galich [13], however, predicting the state of health or illness is based not on the analysis of any subpopulations of lymphocytes using flow cytometry, but on the analysis of various parameters of the cytofluorometric analysis files (analysis of the number of outbreaks, distribution of fluorescence intensities over the channels, etc.). These parameters depend on the settings that the operator uses for a specific measurement, therefore, the proposed method for assessing health is very approximate and can give significant errors in diagnosis. Ways have also been proposed to assess the state of health using a comprehensive assessment, which included some immunological parameters [14, 15]. The first patent took into account a parameter such as malonic aldehyde and the ratio of Th to T-suppressors, and the second - the level of total serum IgA, IgG, IgE, but did not use such a parameter as Th17 in either patent, which is not surprising since in 2001, when both patents were filed, they still did not know about the existence of Th17.

The objective of the invention is to develop a method for assessing human health using immunological blood tests, which allows to confirm the state of health on the basis of compliance with the individual level of Th17 lymphocytes age standards.

The technical result consists in the development of a method for assessing a person’s state of health, which makes it possible to clearly distinguish between a state of health and an illness based on a criterion for matching the age norm of an individual level of Th17 lymphocytes in peripheral blood. Such knowledge makes it possible to identify among individuals with an unknown history and unclear clinical manifestations a group of healthy individuals and a group of patients. The developed method for assessing the state of health helps to monitor the state of health of groups of citizens and to carry out the initial selection of a group of patients requiring further investigation of the causes of their disease, and a group of healthy patients that do not require further research.

The task is achieved by developing a method for assessing the state of health based on the correspondence of the individual level of Th17 lymphocytes in the peripheral blood of a particular person, determined by flow cytometry, to its age norm, calculated by the proposed formula.

The invention consists in the following.

To establish the dynamics of age norms of the level of Th17 lymphocytes in the peripheral blood, 352 healthy people (168 females and 184 males) were examined, from the age of 1 month to 85 years, including 79 under the age of 1 year, and aged 1- 2 years 11 months - 34, 3-4 years 11 months. - 24, 5-6 years 11 months. - 28, 7-8 years 11 months. - 25, 9-11 years 11 months. - 36, 12-14 years 11 months. - 39, 15-18 years old - 26, 20-35 years old - 25, 36-49 years old - 11, 50-70 years old - 16 years old and over 70 years old - 9 people. The state of health was assessed during preventive examinations and annual medical examination, the results of which confirmed the absence of acute and chronic diseases in all examined. Obtained as a result of a laboratory examination, indicators of a clinical blood test were within the age limits.

Immunophenotyping of blood lymphocytes was carried out by flow cytometry with four-color staining in a direct immunofluorescence reaction using the technology of staining lymphocytes in whole blood followed by lysing of red blood cells (BD Biosciences technology and reagents (FACSCalibur flow cytometer, collection and processing program) CellQu information. The lymphoid region was isolated by direct and lateral light scattering taking into account the expression of CD45. Th17-determined level of lymphocytes with a phenotype CD3 + CD4 + CD161 + CD45R0 + , for this population was estimated in percentage composition of Th-cells (Gate CD3 + CD4 +). The age-related dynamics of the percentage of Th17 lymphocytes in the peripheral blood of healthy people in the CD3 + CD4 + lymphocyte (M ± SE) gate is shown in FIG. 1. The dashed line indicates the approximation graph.

Analysis of the level of Th17 lymphocytes revealed a strong positive correlation between the age and the level of lymphocytes of this subpopulation in the peripheral blood of healthy individuals, the Pearson correlation coefficient was 0.82. As a result of mathematical analysis, it was found that the identified relationship is approximated by a quadratic equation: y = 2 + -0,0059h 0,6819h + 1.1671, where x - the age of the subject in years, and we - the estimated percentage of Th17-for-age.

The reliability value of the approximation of R 2 was 0.98, which indicates a very high degree of model compliance.

To assess the state of health of the subject should be calculated according to the formula obtained by approximating our data (y = -0.0059x 2 + 0.6819x + 1.1671, where x is the age of the subject in years), its age norm of the percentage of Th17 lymphocytes and then, using flow cytometry, determine the percentage of Th17 (lymphocytes with the phenotype CD3 + CD4 + CD161 + CD45R0 + , the percentage of the gate CD3 + CD4 + lymphocytes) in a given person in peripheral blood. If the calculated and real indicators coincide or are close, then a person can be considered healthy if the real level of Th17 lymphocytes exceeds the calculated level by more than 5% (2σ) for children under 3 years of age, 7% (2σ) for children from 3 to 7 years and 10% (2σ) for people over 7 years old, then the person is probably sick, and you need to look for the cause of his illness.

ПримерExample

As an example, Table 1 shows the percentage of Th17 lymphocytes in the peripheral blood of people with an unknown history.

Figure 00000001
Figure 00000001

2σ.Further, according to the formula obtained by approximating our data (y = -0,0059h 0,6819h + 2 + 1.1671, where x - age in years of the subject), for each of the surveyed individuals with a history of unknown age level standards were calculated Th17 (%) - column 4 in Table 1, and compared, using criterion 2σ (column 5 in Table 1), with the actual level of Th17 (%) - column 3 in Table 1. As a result of comparisons, it was found out that those examined No. 1, 6, 7, 8, 9 and 10 their real level of Th17 (%) corresponds to the age norm, therefore, these people should be regarded as healthy, which corresponded to reality. On the other hand, in No. 2, 3, 4, 5, the real level of Th17 (%) exceeds the calculated level by more than 2σ, therefore, these people should be regarded as sick, which corresponded to reality. It is interesting to examine in more detail the examined people No. 8 and 10. Thus, the examined No. 8, according to the threshold criterion and according to the criterion of compliance with the age norm, should be assigned to the group of healthy people. However, from the anamnesis it became known that this person is diagnosed with juvenile rheumatoid arthritis, but this patient is undergoing biological therapy (mabthera) and, in all respects, demonstrates persistent clinical and laboratory remission. Its real level Th17 (%) is slightly higher than the estimated age level, but does not exceed 2σ. Patient # 10 is diagnosed with glomerulonephritis and also receives biological therapy. The real level of Th17 (%) exceeds the level of the age norm, but does not go beyond 2σ.

Thus, using the criterion for the correspondence of the real level of Th17 (%) to the estimated age norm, it was possible to correctly divide healthy individuals (No. 1, 6, 7, 9) and patients (No. 2, 3, 4, 5), as well as identify patients in remission on biological therapy (No. 8 and 10), for this, however, history data were needed.
