Abstract

FIELD: medicine.

SUBSTANCE: surgical threads are introduced and fixed in neck soft tissues. Threads are introduced fan-shaped and evenly through the places of splicing of the posterior edge of platism and nodding muscle in the subcutaneous tissue.The ends of the threads are fixed on the front side of the neck along the line from the chin to the jugular notch.

EFFECT: method allows to reduce the traumatism of intervention, increase the duration of neck tissues fixation in order to rejuvenate it.

Description

The invention relates to the field of plastic surgery and cosmetology and can be used for thread tightening of the neck in order to rejuvenate it.

A known method of eliminating skin folds on the face and neck [RU 2186531, C2, A61B 17/00, A61B 17/322, 02.20.2001], including subdermal injection under the folds of biocompatible material, while first removing the subcutaneous fat in the chin area fiber by liposuction, and then the resulting adipose tissue of the patient is injected under the folds of the skin to the level of stationary subcutaneous fat, in portions, in the direction from top to bottom along the fold.

A feature of the method is that liposuction in the chin is carried out under the marker zone in the form of two concentric ellipses, reducing the intensity of suction of fatty tissue in the direction from the inner ellipse to the peripheral.

The disadvantage of this method is the relatively high complexity and relatively low security.

There is also known a method of eliminating skin folds on the face and neck [RU 2186531, C2, A61B 17/00, 02/20/2001], including subdermal injection under the folds of biocompatible material, and first, in the submental area, subcutaneous fat is removed by liposuction, and then the resulting adipose tissue of the patient is introduced under the folds of the skin to the level of stationary subcutaneous fat, in portions, in the direction from top to bottom along the fold.

The disadvantage of this method is the relatively low safety due to the transfer of fatty tissue.

Closest to the proposed one is the method [RU 2465844, C2, АВВ 17/04, 10.11.2012], which is implemented using a device comprising threads of non-absorbable material and means for fixing the threads in the soft tissues of the neck, and means for fixing the threads in soft the tissues of the neck are fragments of a synthetic mesh of non-absorbable material of various shapes and sizes, to which the ends of the threads of non-absorbable material are attached.

The essence of the method implemented using this device is that after placing the device in the subcutaneous tissue by the second stage, after 2 months, the threads are pulled and the tissue sagging is eliminated, and the degree of tissue tension can be changed at any time and their repeated sagging can be corrected without replacing the device repeatedly.

The disadvantage of the closest technical solution is the relatively low reliability and relatively short duration of tissue fixation in order to rejuvenate the neck, which explains the requirement of the need for multiple repetition of the procedure after relatively short periods of time.

The problem that is solved in the invention is aimed at creating a safety minimally invasive method of fixing surface tissues of the neck in order to ensure reliable and long-term fixation of tissues with the aim of rejuvenation.

The required technical result is to increase the safety, as well as increase the reliability and increase the duration of fixation of the tissues of the neck in order to rejuvenate it.

The problem is solved, and the desired technical result is achieved by the fact that according to the method, which consists in the fact that surgical threads are inserted and fixed into the soft tissues of the neck, according to the invention, surgical absorbable threads are used with fixing means in the form of counter-directed notches that are inserted through skin punctures five strands from the parotid and behind the ear region of each ear at the sites of fusion of the posterior edge of platism and sternocleidomastoid muscle and they are carried out fan-shaped and uniformly with subcutaneous tissue atsiey their ends on the front side of the neck along the line from the chin to the jugular notch.

To implement the proposed method, a surgical thread (Fig. 2) 2/0 USP with notches made from a copolymer of polycaprolactone and L-lactic acid can be used. The length of the thread is from 18 to 22 cm. The notches are made by the method of incision, directed towards each other. The step of drawing makes 5 mm.

Implemented the proposed method of thread tightening of the surface tissues of the neck as follows.

With age, changes occur in the skin layer of the surface tissues of the neck. Thinning and atrophy of the skin and subcutaneous muscle of the neck, and adipose tissue may increase or decrease in volume, which depends on the individual characteristics of each person. Common is the stratification and thinning of the fibers of the subcutaneous muscle of the neck and ligaments that hold it.

In the parotid and behind-the-ear region on the human head are ligaments between the auricle and platism, which are tightly spliced with fibrous structures between the posterior edge of platysma and the sternocleidomastoid muscle. The spurs of these fibrous fibers continue to the mastoid process and are tightly spliced with the periosteum in this zone. This makes it possible to create a thread supporting structure for the subcutaneous muscle of the neck and fix it in the area of the platysmal ligament and mastoid process. This fact is used in the proposed method, which can be used for patients with signs of withering of the surface tissues of the neck (the presence of "folding" and bifurcation of the subcutaneous muscle of the neck).

During the operation, anesthesia is performed - local infiltration (0.5% novaacaine, 2% lidocaine, ultracaine in dilution with saline, etc.). All anesthetics in dilution with saline 1: 2 or 1: 3 can be used. Infiltration skin detachment is performed to reduce subcutaneous hematomas. For subcutaneous exfoliation, saline can also be used.

Before starting the procedure, the threads are marked on both sides of the neck (Fig. 1). The installation of the threads is carried out from the area of the projection of the ligament along the edge of the subcutaneous muscle of the neck and sternocleidomium, which, according to the research results, provides the best fixation of the threads for the purpose of lifting soft tissues.

After performing an infiltration anesthesia with a 18G needle, an initial puncture of the skin is performed at the point of insertion of the threads. Next, the cannula with the pre-installed thread is injected subcutaneously through the prepared puncture and is carried out in the subcutaneous tissue. Given that the fiber on the neck may not be expressed, it is possible to conduct the thread through the fibers of the subcutaneous muscle of the neck. In this case, the muscle fibers of the subcutaneous muscle of the neck are contracted and contracted in response to trauma and the installation of material that causes neocollagenesis. The thread fixes the tissues in an elevated state. In the area of the mastoid process, all 5 threads should be additionally fixed with a surgical unit and “drown” it subcutaneously.

Thus, 5 threads are set on each side of the neck. The use of a blunt cannula allows minimizing tissue trauma, resulting in virtually no bruising after the procedure, and swelling is minimal.

After installing the threads there is no need for active tissue tightening along the thread. The effect will be expressed immediately after the procedure and without tension, because the tissues of the neck are thin and the process of fibrosis on a foreign body will strengthen and provide tension to the skin. If necessary, you can pull the side tissues of the neck quite moderately along the thread to the ear, which will provide tension not only to the upper sections, but also between the threads.

After about 1 month, you can visually see that the tissues are aligned and compacted due to increased neocollagenesis in the subcutaneous tissues. The process of improving the appearance of the surface tissues of the neck lasts for about a year.

The proposed method is characterized by the speed of the operation, using only local anesthesia, the absence of postoperative scars, and a short rehabilitation period.

A pronounced effect is achieved by the shape and direction of the incisions, the possibility of installing atraumatic 20G cannulas, installing threads through one puncture, the possibility of additional fixation, as well as by stimulating neocollagenesis in response to resorption of the material. This achieves the required technical result, which consists in increasing the safety, as well as increasing the reliability and increase the duration of fixation of neck tissues in order to rejuvenate it.
