Abstract
Description
The invention relates to medicine, and more specifically to medical laboratory diagnostics, and can be used in obstetrics and gynecology.
Early diagnosis of anomalies in the secretory function of gonads, expressed in violations of ovulation processes, can significantly reduce the number of infertile marriages and improve the demographic situation in the population. Determination of the initial signs of hormonal status and fertility disorders is an urgent problem of modern medicine, since the natural population decline in Russia alone today amounts to about 1 million people in three years.
Today, there is no doubt that the fact that one of the main factors leading to a decrease in the birth rate is the deterioration of the reproductive health of women. According to some authors, over the past 10 years, the number of women who are absolutely healthy from a gynecological point of view has decreased to 6.3% of the total number of women of childbearing age (Kulakov V.I., Korneeva I.E. Diagnosis and treatment of female infertility // Practical Gynecology. / Under the editorship of V.I. Kulakov and V.N. Prilepskaya. - M.: MEDpress-inform, 2001. - S. 298-317). This leads to the fact that about 20% of couples planning a pregnancy encounter certain problems that impede conception.
Fertility impairment (infertility) is diagnosed when a woman is unable to conceive during one year of active sex with a healthy man without contraception. The etiology of female infertility is diverse and, according to most authors, endocrine disorders come first. In some cases, these disorders are reversible and can be corrected; in others, they require the use of more radical methods, such as IVF or surrogacy.
To predict infertility, various biochemical and instrumental diagnostic methods are currently proposed.
Существует способ прогнозирования состояния фертильности у женщин репродуктивного возраста с миомой матки путем определения концентрации 2-гидроксиэстрона и 16α-гидроксиэстрона в моче, концентрацию прогестерона в сыворотке крови (Патент RU №2433411, G01N 33/74, 13.07.2009).There is a method for predicting fertility in women of reproductive age with uterine myoma by determining the concentration of 2-hydroxyestrone and 16α-hydroxyestrone in the urine, the concentration of progesterone in the blood serum (Patent RU No. 2433411, G01N 33/74, 07/13/2009).
The essence of the method: in the luteal phase of the menstrual cycle, determine the concentration of 2-hydroxyestrone and 16α-hydroxyestrone in the urine, the concentration of progesterone in the blood serum. Then the canonical value is calculated by the formula K = 4.23-0.04 × A + 0.0016 × B-0.18 × C, where A is the concentration of progesterone in the blood serum for 20-21 days of the menstrual cycle (luteal phase) ( nmol / L), B is the concentration of 2-hydroxyestrone in the urine (nmol / L), C is the concentration of 16α-hydroxyestrone in the urine (nmol / L). With K greater than -0.175 infertility is predicted, with K less than -0.175 - preserved fertility. Using the method improves the accuracy of predicting infertility and intact fertility in women with uterine fibroids.
Метод привязан к лютеиновой фазе менструального цикла, что делает возможность его применения в очень ограниченный интервал времени (21-22 дни менструального цикла).The disadvantages of this method are the possibility of its use in women with uterine myoma, the calculation of indicators requires special mathematical training and is difficult in practical application. The method is tied to the luteal phase of the menstrual cycle, which makes it possible to use it in a very limited time interval (21-22 days of the menstrual cycle).
Another method for diagnosing the endocrine form of infertility is dopplerographic and ultrasound (Patent RU No. 2350272, A61B 8/00, 03/27/2009), which allows you to directly monitor ovulation processes.
The method consists in conducting dopplerographic and ultrasound examinations before ovulation every day, starting from the seventh day of the cycle, with the determination of the growth of the endometrium, the dominant follicle and changes in blood flow in the aromatic arteries of the uterus and perifollicular blood flow. They assess the achievement of the dimensions of the thickness of the endometrium and the dominant follicle, as well as changes in blood flow indices on the 9th, 11th day of the cycle and at the time of ovulation with an individual selection of hormonal preparations. A control study is carried out 7 and 14 days after ovulation. Evaluate the thickness of the endometrium, the size of the corpus luteum, blood flow in the arcuate arteries of the uterus and the corpus luteum. Prescribe individual hormone therapy. Further tactics of hormone therapy are chosen in the presence or absence of pregnancy. The method allows to increase the accuracy of diagnosis of disorders of the ovarian-menstrual cycle in women suffering from the endocrine form of infertility, and to individualize the selection of the dose of hormonal drugs by increasing the controlled parameters.
The disadvantage of this method is the impossibility of an outpatient examination, the duration of the study (since even in healthy women, according to statistical data, up to 10% of the cycles can be anovulatory), a significant cost and subjectivity in the interpretation of ultrasound data.
There is also known a method for the diagnosis of endocrine infertility by measuring the rheographic parameters of the tissue during the menstrual cycle (AS No. 1398821, A61B 5/02, 05/30/1988). The essence of the method lies in the fact that in the cervical canal the electrical resistance is measured by a bipolar sensor (not described by the authors). At the same time, in women with a normal menstrual cycle, when applying the measurement results to the cyclogram, a clear biphasicity of the obtained curve was revealed, and in women with the anavulatory cycle, the graphic image was distinguished not only by its size, but also by the absence of dynamic changes.
The disadvantage of this method is the methodological complexity of the study and its invasiveness for the examined woman, the impossibility of dynamic observation, since trauma to the cervical canal epithelium reduces the diagnostic value of the indicators.
- 656 с.).There are also well-known general clinical methods for the diagnosis of endocrine infertility. The main way to diagnose endodrine disorders is to determine the level of sex hormones in the blood serum (Karnischenko A.I. Medical laboratory technologies and diagnostics: a reference book. Medical laboratory technologies. - St. Petersburg: Intermedika, 1999. - 656 p.).
Показателями, по которым можно судить о гормональном статусе женщины, являются лютеинизирующий гормон (ЛГ), фолликулостимулирующий гормон (ФСГ), пролактин, тестостерон, эстрадиол и дегидроэпиандростерон-сульфат (ДГЭА-сульфат).Luteinizing hormone (LH), follicle-stimulating hormone (FSH), prolactin, testosterone, estradiol and dehydroepiandrosterone sulfate (DHEA sulfate) are indicators that can be used to judge a woman’s hormonal status.
To determine the hormonal regulation of the menstrual cycle, hormones in the blood are determined strictly according to the days of the cycle.
3-5 day cycle: LH FSH Prolactin
7-10 day cycle: Testosterone Estradiol DEA-sulfate
20-21 day cycle: Estradiol Progesterone
п.All hormonal studies are technologically complex and are characterized by great variability of results due to the extreme lability of the recorded parameters and the duration of the menstrual cycle, which can be influenced by various external and internal factors - stress, somatic acute and chronic diseases, etc.
To assess fertility, there are also various functional tests: colpositological, morphological, biophysical. They allow you to indirectly judge the nature of hormonal secretion of the ovaries (K.N. Zhmakin et al. Gynecological endocrinology, M., p. 186., 1980) and ovulation processes. Among them, the most common are measurements of basal temperature, histological studies of vaginal smears and endometrium, and measurement of viscosity of cervical mucus.
All these tests are indicative only and are used to form groups of women for further in-depth research. The performance of these studies is characterized by invasiveness, subjective assessment of the results and, therefore, relatively low accuracy.
Analysis of existing methods for the diagnosis of endocrine infertility showed that none of them can be a prototype for the method proposed by the authors.
Recent Ovulation Processes (Gynecological Endocrinology 2011; 27 (1): 27-32 (doi: 10.3109 / 09513590.2010.493961) Elissavet Hatzi, Ioanna Bouba, Anna Galidi, Leandros Lazaros, Nectaria Xita, Prodromos Sakaloglou, Georgios Kolios Bairaktari, Apostolos Kaponis et al.), Are regulated in the body of a woman by kisspeptin.
(Ann Endocrinol (Paris). 2012 Nov; 73(5):448-58. doi: 10.1016 / j.ando. 2012.07.680. Epub 2012 Sep 20. «Overview of the impact of kisspeptin on reproductive function.)) Sonigo CI, Binart N.)The hormone kisspeptin (the “protein of kisses”), encoded by the kiSS-1 gene, was discovered in 1999. It was found that this substance, which is naturally produced in the body, initiates puberty in adolescence and is further responsible for the regulation of the reproductive function of the body. The interaction of kisspeptin and its cellular receptor GPR54 in the hypothalamus is a determining factor in the activation of neurons releasing gonadoliberin, which, in turn, controls the level of LH and FSH secretion, which determines the occurrence of ovulation and the possibility of conception. The Kisspeptin / GPR54 system activates the hypothalamic-pituitary-ovarian system, but the mechanisms of this influence are not completely understood (Ann Endocrinol (Paris). 2012 Nov; 73 (5): 448-58. The objective of the invention is to determine the primary link of the defeat of the female reproductive system, as well as the efficiency, high accuracy of the study and simplification of the diagnosis of female endocrine infertility compared to existing ones.
The task is achieved in that women of reproductive age on an empty stomach from the cubital vein, regardless of the phase of the menstrual cycle, take 5-10 ml of venous blood and determine the level of kispeptin in the resulting serum, while the level of 2.5 ÷ 12.0 ng is considered normal / ml, and endocrine infertility is reliably diagnosed with its value below 1.7 ng / ml.
The method proposed by the authors is pioneering, because determination of the level of the hormone kispeptin as a predictor of endocrine infertility has not yet been carried out.
In practice, the method is as follows. They take blood from a woman on an empty stomach from a cubital vein. The amount of test material 5-10 ml. Blood sampling from patients from the study group is carried out by the vacuum method. On a test tube indicate the name, date of blood sampling. Then the tubes are placed on a flat surface and left for 20-30 minutes until a clot forms. The laboratory centrifuges the test material at a speed of 2000 rpm for 10 minutes. The resulting serum is placed in an empty sterile
If necessary, whey can be frozen at -20 ° C and stored for up to 4 months.
Enzyme-linked immunosorbent assay (ELISA, enzyme-linked immunosorbent assay, ELISA) is a well-known and widely used laboratory immunological method for the qualitative and quantitative determination of various compounds, macromolecules, viruses, etc., based on a specific antigen-reaction. antibody. Identification of the resulting complex is carried out using the enzyme as a label for signal registration. To study blood serum at the level of kispeptin, a set of reagents Kisspeptin-54, metastin, 96 is used (Production: BCM (Boots Contract Manufacturing, Great Britain); Method: ELISA; Volume: 96; Cat.no .: S-1308) peptide test system. This reagent kit is designed for 96 determinations.
The result of the study - the level of kisspeptin in a patient from the study group is known on the same day.
Т.к. Because Since kispeptin is genetically programmed, its indicator is not affected by the day of the menstrual cycle, which allows the study to be performed once in any phase. The result obtained is considered reliable.
For the examination, groups of women were selected for whom, according to the WHO classification, infertility could be suspected - the absence of pregnancy after one year of regular sexual activity with a healthy partner, with the exclusion of other causes of infertility.
The following groups of women were examined to obtain reference diagnostic concentrations of kispeptin in normal and pathological conditions:
1. Женщины в возрасте 18-35 лет, имеющие детей, которые не имеют проблем с наступлением беременности, = 212 человек;1. Women aged 18-35 years, having children who have no problems with pregnancy, = 212 people;
2. Женщины в возрасте 18-35 лет, имеющие детей, которые не могли забеременить более одного года при регулярной половой жизни со здоровым партнером и у которых были после комплексного обследования исключены все причины бесплодия, кроме эндокринного, = 171 человек;2. Women aged 18-35 years who have children who could not conceive for more than one year with regular sexual intercourse with a healthy partner and who, after a comprehensive examination, excluded all causes of infertility, except endocrine, = 171 people;
3. Women aged 18-35 years who do not have children who, after a thorough examination, were assigned to the endocrine infertility group and underwent IVF treatment, = 46 people.
According to the results of the survey, reference values of the level of kispeptin for the diagnosis of endocrine infertility were established, which are indicated in the table:
As a result of the study, it was concluded that at the concentration of kispeptin in serum there are the following values:
- 2,5-12 нг/мл - нормальное содержание;- 2.5-12 ng / ml - normal content;
- 1,7-2,5 нг/мл - умеренное снижение;- 1.7-2.5 ng / ml - moderate decrease;
1.7 ng / ml - significant decrease.
Depending on the indicators of kispeptin in blood serum, it can be concluded about the etiology of infertility in women. The advantage of the method are:
- определение первичного звена причины патологии эндокринного бесплодия;- determination of the primary link of the pathology of endocrine infertility;
- возможность диагностики в амбулаторных условиях;- the possibility of diagnosis in an outpatient setting;
- отсутствие привязки исследования к дням менструального цикла;- the lack of binding research to the days of the menstrual cycle;
- невозможность получения ложноположительных или ложноотрицательных результатов на фоне влияния внешних и внутренних факторов на репродуктивную сферу из-за генетической детерминированности секреции кисспептина в организме женщины;- the impossibility of obtaining false-positive or false-negative results against the background of the influence of external and internal factors on the reproductive sphere due to the genetic determinism of secretion of kisspeptin in the body of a woman;
Результат возможно получить пациентке и врачу в день исследования.- simplicity of the method: no additional reagents, special facilities for blood sampling and enzyme-linked immunosorbent assay (ELISA) are required. The result can be obtained by the patient and the doctor on the day of the study.
This method allows you to quickly and reliably determine the cause of infertility and conduct differential diagnosis between a large number of causes of endocrine infertility.
