Abstract
Description
The invention relates to medicine, namely to cosmetology.
М. С. К. М. А. 2001].Lymphotropic method refers to indirect endolymphatic methods of drug administration. It is based on a preliminary injection into the tissue of a high molecular weight drug with a lymphotropic effect and the ability to direct the drug introduced after it through the same needle, including low molecular weight, mainly into the lymphatic system. These substances are called endolymphatic conductors or drug preparations with lymphotropic activity. It has been established that the properties of endolymphatic conductors are possessed by: 1) a drug with hyaluronidase activity of lidase [I. Rusnyak et al., 1957; Mashkovsky M.D., 1984; Baranov A.A., 1998] and the anticoagulant heparin [Mammadov Y.D., 1982; Gubkina M.F., 1996; Sviridkina L.P. et al., 2007]. When a drug enters the lymphatic system, its presence in the blood is maintained in a therapeutic dose for a day, which is accompanied by an increase in the concentration in the lesion compared with standard methods of administration. This allows you to reduce both daily and single dose of the drug [Levin Yu.M. et al., 1982, 1987, 2014; Panchenkov R.T., et al., 1984; Yu.S. Sidorenko, A.K. et al., 1985; Buyanov V.M. et al., 1991; Efimenko N.A. et al., 2001].

In these studies, it was shown that with the lymphotropic administration of drugs, saturation of the lymphatic system with antibiotics, immunomodulators, solcoseryl, non-steroidal anti-inflammatory drugs or glucocorticosteroids, leads to a more rapid regression of the clinical picture of the disease, helps to optimize the regeneration of purulent foci by enhancing lymphatic bactericidal action, stimulating local and general immunity.
However, to date, lymphotropic methods of administering cosmetic preparations to correct skin problems of the face and body have not been used.
A generally accepted method of eliminating ethereal deficiencies and correcting age-related changes in skin and soft tissues in cosmetology is mesotherapy, the introduction of microvolumes of cocktails based on hyaluronic acid, vitamin-mineral components, amino acids, peptide preparations, coenzymes, antioxidants, plant and cell extracts, and classical mono drugs for solving aesthetic problems of soft tissues of the face and body (age-related changes in the skin, cellulite, local fat deposits, hair treatment and others x problems available for correction by mesotherapy) and other pharmacopoeial drugs.
Their introduction is carried out by injection in the classical technique of papules, linear administration technique and nappage technique.
The components of the preparations introduced into the skin act in a small radius. The disadvantage of mesotherapy is pain during administration, local reactions at the points of injection and hematoma.
The problem to be solved in the proposed method was the development of a technique devoid of such aesthetic undesirable phenomena of the mesotherastic session as soreness and hematomas in the facial skin region, however, comparable with mesotherapy in terms of effectiveness, made from several injections, or exceeding its effectiveness.
The technical result consists in enhancing the effects of mesotherapeutic effects due to targeted delivery of cocktail components to the lymphatic system. This result is achieved by providing an increase in the concentration of the components of the mesotherapy cocktail in the lymphatic system, where they are absorbed on lymphocytes, which actively deliver them to all regions of the zone of interest. This is facilitated by a wide representation of the lymphatic vessels in the skin, both of the face and of the body as a whole.
In addition, the developed technique is relatively painless and reduces the risk of developing local reactive reactions (with the exception of cases of drug allergy, which is much less common with lymphotropic injections than with standard methods of administration).
The lymphotropic method of mesotherapy is as follows.
The injection is made into soft tissues into areas of large representation of the lymph nodes of the area of interest (for example, if the area of interest is the face, then the injection is performed under the mastoid process). Injection may be performed, for example, with a 30G needle. An anesthetic is initially injected (for example, 0.1-0.5 ml of a 0.25% solution of novocaine), which provides anesthesia and expansion of regional lymphatic vessels. Then an endolymphatic conductor is inserted through the same needle. The choice of a conductor preparation depends on the composition of the mesotherapy cocktail. If a cocktail of hyaluronic acid is used, a dose of 0.5 ml of heparin (2,500 units) is used. If the cocktail does not contain hyaluronic acid, 16 units of lidase dissolved in 1 ml of 0.25% novocaine are used as a conductor preparation.
With lidase intolerance (a tendency to allergic reactions), heparin should be used.
After administration of the drug-conductor, the needle remains in place for about 5 minutes, after which a mesotherapy cocktail in the volume of 0.1-0.5 ml is very slowly introduced through the syringe or with the help of a dispenser. The rate of administration of 0.1 ml per minute. After the first procedure, if necessary (for example, when injecting on the face), it is repeated from the opposite side.
The duration of the entire procedure does not exceed 15 minutes.
Examples of the method.
The clinical evaluation of the effectiveness of lymphotropic mesotherapy was carried out in 30 patients aged 20 to 60 years, who once a week for 4 weeks were injected lymphotropically into the soft tissues under the mastoid processes of one group of patients with Tiziano: a cocktail for lifting and skin tightening (Tiziano skin lifting &tightening); manufacturer "RENAISSANCE S.R.L.", Italy; certificate of state registration RU 77.01.34.001. E. 022382.22.11 of December 28, 2011. The Tiziano cocktail contains hyaluronic acid, omega STP complex (
Also, as a mesotherapeutic cocktail that does not contain hyaluronic acid, Michelangelo was administered to another group of patients: a cocktail for intensive lipolytic effects, in particular, in the “second chin” area, manufacturer RENAISSANCE S.R.L., Italy. The composition of the Michelangelo cocktail includes phosphatidylcholine, caffeine, carnitine, dioxicholic acid, lipoid acid.
Before the first procedure and a week after the last, fourth, procedures, facial skin condition was assessed using computer diagnostics using the SOFT PLUS apparatus (Callegari, Italy) using the 4 most important parameters: moisture, elasticity, fat content, and melanin level.
Injection technique. Injection was made into soft tissues under the mastoid process with a needle for mesotherapy. Initially, 0.5 ml of a 0.25% solution of novocaine was injected under the mastoid processes, then an endolymphatic conductor was introduced through the same needle: 0.5 ml of heparin (2,500 units), if the mesotherapeutic cocktail contained hyaluronic acid, or 16 units lidase, dissolved in 1 ml of 0.25% novocaine, if hyaluronic acid was absent in the mesotherapy cocktail. 5 minutes after heparin was introduced through the same needle, with a dispenser at a rate of 0.1 ml per minute, the mesotherapy cocktail "Tiziano" or "Michelangelo" was infused in a volume of 0.5 ml. After the procedure, on the one hand, it was repeated on the other hand. After completing both procedures, alcohol compresses were applied to the injection sites for 30 minutes.
1-4:The results of evaluating the effectiveness of the course of lymphotropic mesotherapy are illustrated in figures 1-4:
1. FIG. 1. The frequency of occurrence of changes in the main parameters of the skin of the face (in%) before treatment and after a course of lymphotropic mesotherapy
2. FIG. 2. The frequency of occurrence of positive and negative dynamics of the main parameters of the facial skin (in%) after a course of lymphotropic mesotherapy
3. FIG. 3. Dynamics of indicators of the main parameters of facial skin after lymphotropic mesotherapy
4. FIG. 4. The dynamics of the melanin index in the skin of the face after lymphotropic mesotherapy.
1).Before the first procedure, a decrease in hydration was recorded in 72.7% of patients, a decrease in elasticity in 41.7%, a decrease in fat content in 81.8%, and an increase in melanin level above 3 UE in 36.4%. After completing the course of lymphotropic mesotherapy, the number of patients with reduced hydration decreased to 63.6%, elasticity to 33.3%, fat content to 45.4%. The number of patients with an increased content of melanin in the skin increased to 45.4% (Fig. 1).
2).After a course of lymphotropic mesotherapy, an increase in skin hydration occurred in 54.5% of patients, but in 36.4% it decreased. Skin elasticity improved in 58.3%, while in 16.7% of patients it worsened. Oily skin increased in 72.7% of cases and decreased in only 9.1% of cases. The dynamics of the level of melanin in the skin of the face was not so positive: in 18.2% of patients its content increased and only in 9.1% decreased (Fig. 2).
Средний показатель содержания в коже меланина не изменился (фиг 4): 5,5±1,7 УЕ до лечения и 6,2±1,6 УЕ после лечения (р>0,2).When analyzing the dynamics of the average indicators of the main parameters of the skin, it was found that after a course of lymphotropic mesotherapy, there was a tendency (Fig. 3) to increase skin hydration (from 31.9 ± 6.2 UE to 41.0 ± 7.1 UE, p <0, 2), an improvement in its elasticity (from 38.0 ± 1.7 UE to 41.2 ± 1.4 UE, p <0.05) and an increase in fat content (from 22.8 ± 4.7 to 37.4 ± 4 , 7 UE, p <0.01). The average melanin content in the skin did not change (Fig. 4): 5.5 ± 1.7 UE before treatment and 6.2 ± 1.6 UE after treatment (p> 0.2).
Output. The course of lymphotropic mesotherapy using the developed technique significantly reduces dry skin by increasing fat content and improves its elasticity. The indicator of skin hydration has only a tendency to increase. The therapy does not affect the content of melanin in the skin of the face. It should be noted that all patients noted painless procedures as a result of slow administration of the drug using a dispenser. No inflammatory reactions have been reported at the injection sites.
