Метод полимеразной цепной реакции с обратной транскрипцией (ОТ-ПЦР) является наиболее распространенным методом, используемым в лабораторной практике для дифференциальной диагностики инфекционных заболеваний. В основе метода лежат две реакции - обратная транскрипция и полимеразная цепная реакция. В реакции обратной транскрипции на матрице вирусной РНК строится комплиментарная ДНК (кДНК). В следующей за реакцией обратной транскрипции ПЦР происходит многократное избирательное удвоение целевого участка кДНК (амплификация). Амплифицируемый участок кДНК является маркерным, так как строго ограничен последовательностями ДНК-затравок (праймеров), без которых невозможно протекание ПЦР. Для детекции накопления продуктов амплификации в режиме реального времени помимо пары праймеров необходим также флуоресцентно-меченый ДНК-зонд. Сложность выбора праймеров и зонда обусловлена требованием их строгой видоспецифичности. В случае если выбранные олигонуклеотиды не обладают видовой специфичностью к целевому объекту либо не обладают достаточной гомологией к целевой нуклеотидной последовательности, возможны ложноотрицательные результаты исследования. В случае если выбранные праймеры и зонды демонстрируют сродство к нецелевым последовательностям ДНК, то возможны ложноположительные результаты исследования. Правильный выбор сочетания пары праймеров и флуоресцентно-меченого ДНК-зонда позволяет осуществить строго специфическую амплификацию целевого фрагмента кДНК и провести детекцию накопления продуктов ПЦР в режиме реального времени.The reverse transcription polymerase chain reaction (RT-PCR) is the most common method used in laboratory practice for the differential diagnosis of infectious diseases. In order to detect the accumulation of amplification products in real time, in addition to a pair of primers, a fluorescently-labeled DNA probe is also required. The complexity of the choice of primers and probe is due to the requirement of their strict species specificity. If the selected oligonucleotides do not have species specificity for the target object or do not have sufficient homology to the target nucleotide sequence, false negative results of the study are possible. If the selected primers and probes demonstrate an affinity for non-target DNA sequences, then false-positive test results are possible. The correct choice of a combination of a pair of primers and a fluorescently labeled DNA probe allows for strictly specific amplification of the target cDNA fragment and real-time detection of PCR products accumulation.

На основе теоретического изучения структуры генома респираторно-синцитиального вируса человека на консервативную область гена вирусного гликопротеина слияния - fusion protein (F-ген) были рассчитаны и синтезированы праймеры и флуоресцентно-меченые ДНК-зонды для проведения полимеразной цепной реакции (таблица 1). Положительные контрольные образцы были получены методом TOPO-T/A клонирования. После чего компетентные клетки Е. coli линии ТОР 10 (Invitrogen, США) были трансформированы полученными плазмидами pCR2.1 (Invitrogen, США), несущими вирусспецифическую вставку синтезированных фрагментов ДНК. В ходе проделанной работы была сконструирована рекомбинантная плазмида: pRSV. Наличие специфической к вирусному геному ДНК-вставки подтверждали секвенированием.Positive control samples were obtained by TOPO-T / A cloning. Измерение флуоресценции проводили на приборах "Rotor Gene 6000" (Corbet Research, Австралия) и iQ5 (BioRad, США). Результаты оценивали по наличию флюоресценции до 30 цикла (фиг. 1). На фиг. 1 представлены результаты анализа клинических образцов методом ПЦР в реальном времени с использованием патентуемых олигонуклеотидов, из графиках которой видно, что: в образцах №8, 14 обнаружена РНК респираторно-синцитиального вируса человека; К+ - положительный контрольный образец (pRSV).Real-time PCR was performed according to the amplification program shown in Table 5. Предлагаемые к патентованию олигонуклеотиды были авторами апробированы в эпидемиологическом исследовании по изучению видового разнообразия агентов, вызывавших ОРВИ у жителей г. Новосибирска и Новосибирской области в октябре-апреле 2011-2012 гг. Авторами было исследовано 164 образца назофарингеальных смывов от пациентов с симптомами острой респираторной вирусной инфекции. Этиологический агент заболевания был авторами установлен в 43% случаев (69 образцов). Респираторно-синцитиальный вирус в исследуемых образцах был выявлен в 4% случаев (7 образцов) (фиг. 1). В структуре сочетанных инфекций респираторно-синцитиальный вирус был выявлен авторами в 17,4% случаев (4 образца из 11).
