Затем после перерыва 2 дня осуществляют облучение укрупненными фракциями по РОД 3,6 Гр в течение трех дней, при этом перед каждым сеансом под язык пациента помещают медицинское изделие - Матрицу гидрогелевую высокоструктурированную на основе альгината натрия, в которую предварительно введены метронидазол в количестве 150 мг и черника в количестве 20-35 мг на 4-5 часов дважды с перерывом в 1-2 часа. Перед последующими двумя сеансами облучения при РОД=2,4 Гр под язык помещают такую же матрицу гидрогелевую на 4-5 часов однократно, далее, после 2-дневного перерыва, последующие 5 сеансов лучевой терапии проводят при РОД=2,4 Гр до СОД 30 Гр, при этом под язык предварительно помещают высокоструктурированный диск «Колегель-ДНК-Ч» на 4-5 часов.This is achieved by the fact that in the treatment of locally advanced cancer of the oropharyngeal zone, including radiation therapy in a dynamic dose fractionation mode in combination with application of a material containing sodium alginate with metronidazole to the tumor, it is proposed that radiation therapy be started with a single focal dose of ROD 2, 4 gr. Применение медицинского изделия - Матрицы гидрогелевой высокоструктурированной на основе альгината натрия производства ООО «Колетекс» (ТЗ №024-13 от 22.03.2013), в которую предварительно введены метронидазол и черника, наряду с изменением схемы облучения позволяет решить поставленную задачу. Используемое медицинское изделие обладает повышенной степенью клейкости и адгезии к слизистой поверхности, а также размерами, обеспечивающими расположение в подъязычной области в течение длительного времени, высокой адгезивной и биодеградивной стойкостью к слюне, обеспечивает местное проникновение препарата в опухоль. Кроме того, местное применение данного медицинского изделия обеспечивает отсутствие общетоксических свойств метронидазола на организм и приводит к купированию воспалительного процесса в ротовой полости, очищению изъязвленных форм опухоли и сопутствующих травм слизистой оболочки (например, прикусывание и травматизация слизистой щеки зубами во время приема пищи), уничтожение патогенной микрофлоры, стабилизации гемопоэза и иммунитета пациента.The use of a medical device - Highly structured hydrogel matrices based on sodium alginate manufactured by Koleteks LLC (TK No. 024-13 of 03/22/2013), into which metronidazole and blueberries were previously added, along with a change in the irradiation scheme, allows us to solve the problem. Диспептических явлений в виде тошноты и рвоты не было. Диспептических явлений и усиления местных лучевых реакций также отмечено не было. Регрессия опухоли составила до 90%. На консилиуме с участием хирурга, радиолога и химиотерапевта было принято решение о продолжении лучевой терапии.
