Abstract
Description
The invention relates to medicine, namely to otorhinolaryngology, and can be used to treat adenoiditis, both chronic and acute.
Currently, in the treatment of this pathology, a surgical method of treatment, conservative treatment with symptomatic agents or by irrigation washing of the nasopharynx are used, photodynamic therapy and combined treatment methods are also used.
A known method of treating acute and chronic adenoiditis (RF Patent No. 2219971, IPC A61N 5/067, publ. 2003), which consists in the fact that the nasal passages are washed with 0.9% saline solution. Additionally, the projection area of the tonsils in the submandibular region is affected by the Ultraton-AMP-2 apparatus for 3 minutes left and right for 5-7 days. In the presence of inflammatory processes in the paranasal sinuses, the area of their projection is affected by the phototherapy apparatus. For this, a Geska-2 source of infrared radiation is used. Affect for 5 minutes on both sides for 5-7 days.
The disadvantage of this method is: low efficiency due to the fact that 0.9% saline does not have a bactericidal property, it is impossible to directly contact the mucous membrane of the nasopharynx and nasopharyngeal tonsil, which reduces the effectiveness of the effect.
Known is a combined method of treating chronic adenoiditis in children of primary school age (RF Patent No. 2307686, IPC A61N 5/067, publ. 2007), including irrigation of the pharyngeal tonsil with a solution of miramistin diluted with distilled water in a ratio of 1: 2. At the same time, the washing liquid is sucked off. They are additionally exposed to defocused coherent monochromatic red light of a certain length and power. In this case, the light guide is introduced into each half of the nose to the nasopharynx for 5 minutes daily, once a day, for 5-7 days.
The disadvantage of this method is: trauma due to discomfort while simultaneously introducing into the nasal cavity an endoscope, an electric suction pump, and then a fiber to the child’s nasopharynx for a long time - 5 minutes. Age restrictions on the use of this method of treatment, the impossibility of carrying out this method in the presence of abnormalities in the development of intranasal structures, for example, curvature of the nasal septum. It has been established that miramistin, being a cationic detergent, increases the permeability of the cell wall not only of microorganisms, but also of the mucous membrane of the nasopharynx, thereby leading to cytolysis, hypersensitivity, and inhibition of mucociliary clearance. The presence of a side effect in the form of a burning sensation.
1997).The closest is a method of treating adenoiditis in children by applying to the region of the upper cervical lateral lymph node of the first order for the nasopharyngeal tonsil, located behind the external auditory meatus, 3-5% of the ointment containing the antibiotic, and exposure to this area with ultrasound alternately on both sides (Patent RF №2087166, IPC A61N 7/00, publ. 1997).
The disadvantage of this method is that there is no direct contact of the physiotherapeutic and antibacterial factors on the mucous membrane of the nasopharynx and nasopharyngeal tonsil, which reduces the effectiveness of the treatment and increases the duration of treatment.
The objective of the invention is to eliminate these drawbacks, increase the effectiveness of treatment of adenoiditis and its tolerance in children, reduce the number of required medications, increase the period between relapses of adenoiditis due to more effective rehabilitation and activation of factors of local humoral immunity.
To this end, in a method of treating adenoiditis in children, including drug therapy, ultrasound exposure to the site of inflammation, it was alternately proposed on both sides that the transnasal effect on the pharyngeal tonsil with an octenisept solution diluted 1:10 with 0.9% sodium chloride solution as a drug therapy. This solution is supplied alternately through the right and left halves of the nose for 3 seconds 30-40 times for 6-10 minutes, while ultrasonic treatment is performed transnasally with the Sonoka 180 apparatus with a frequency of 20-40 KHz, for a course of 6-7 procedures daily or in one day.
This sequence of actions allows to increase the effectiveness of the treatment of adenoiditis due to the acceleration of sanitation of the nasopharynx due to a decrease in the local inflammatory reaction of the mucous membrane of the nasopharynx and pharyngeal tonsil, a decrease in the degree of bacterial colonization of the mucous membrane of the nasopharynx and pharyngeal tonsil by the pathogenic flora and improvement of nonspecific resistance of the pharyngeal tonsil.
The implementation of the method shown in specific clinical examples.
1.Example 1
Patient D., 5 years old, turned to the Children's Diagnostic Center in Domodedovo with complaints of a lack of nasal breathing, night snoring, mucopurulent discharge from the nose.
After examination by the otolaryngologist, the diagnosis was made: adenoiditis, adenoids of the III century. With anterior rhinoscopy, the nasal mucosa is hyperemic, a large amount of mucopurulent discharge of the nasal cavity. When conducting endoscopy of the nasopharynx, the diagnosis is confirmed. A bacteriological examination of the nasopharynx revealed Haemophilus influenzae 10 in 8 tbsp. CFU / ml.
The treatment of the proposed method.
Before starting the procedure, the patient sat down, tilting his head forward, but not tilting it to the side, in front of him held a tray for draining the medicinal substance used for irrigation into it. The pharyngeal tonsil was treated with a solution of octenisept in a 1:10 dilution of 0.9% sodium chloride solution, the solution being supplied to the right and left halves of the nose alternately for 3 seconds for 8 minutes 30 times, ultrasonography was performed transnasally with a Sonoka 180 device manufactured by the company Soring GmbH (Germany) with a frequency of 25 KHz.
During the operation of this apparatus, the solution enters through a system of tubes and is fed transnasally to the nasopharynx through the irrigating channel of the zonotrode, intermittently. The energy of ultrasonic vibrations is transferred to the fluid, which is supplied through the central channel in the waveguide, as a result of which cavitation processes occur in the fluid.
These procedures were performed daily for 6 days.
After the course of therapy, the child’s condition improved, nasal breathing recovered, night snoring passed, nasal discharge disappeared, the mucous membrane of the nasal cavity turned pink, discharge in the nasal passages disappeared.
During the control endoscopy of the nasopharynx: the mucous membrane of the nasopharynx is pink, the nasopharyngeal tonsil is hypertrophied to 1-11 degrees, pink, there is no pathological discharge. With repeated swabs from the nasopharynx, pathological flora was not detected.
