Abstract
Description
The invention relates to medicine, namely to otorhinolaryngology, and can be used to treat adenoiditis in children.
The nasopharyngeal tonsil is part of the lymphatic system of the body and provides its inherent protective function. Against the background of a decrease in the local and general reactivity of the body with adenoiditis, microorganisms begin to multiply and disseminate in the tissues of the nasopharyngeal tonsil, which leads to the development of diseases such as otitis, sinusitis, etc.
For the treatment of adenoiditis and sinusitis, the Proetz method [1] is known which requires the patient to actively participate in the manipulation / lie on the couch, hang his head down and, when the antibiotic solution is instilled into the nose, inhale one or the other nostril, clamping them alternately /. Young children have a negative attitude to this procedure, so it is difficult to carry out it. In addition, the antibiotic solution only for a short time comes into contact with the surface of the nasopharyngeal tonsil and does not have a sufficient therapeutic effect.
Surgical treatment of adenotomy in young children is undesirable because of the important role of the nasopharyngeal tonsil in the fight against respiratory tract infection.
Closest to the proposed method is a method of antibiotic therapy, when antibiotics are administered intramuscularly or given orally in tablets [1] Moreover, antibiotics enter the lymphatic tissue already bound to proteins, that is, weakened. An increase in dose is fraught with the development of dysbiosis, depression of immunity, increased toxic effects. Treatment with this method has the general effect of an antibiotic on the child’s body, has a temporary effect, does not prevent recurrence of otitis media and exacerbations of adenoiditis, and often leads to surgical interventions. To effectively affect the pathogen with adenoiditis, it is necessary to create a high concentration of the antibiotic in the lymphatic tissue of the nasopharyngeal tonsil.
The essence of the invention lies in the treatment of adenoiditis in children by means of ultrasound on the region of the upper cervical lateral lymph node of the first order for the nasopharyngeal tonsil with preliminary application of 3-5% ointment containing an antibiotic to this region.
The method is as follows.
The sensitivity of the nasopharynx microflora to the effect of antibiotics is determined in the traditional way. Then prepare an ointment containing 3-5% antibiotic, which is harmful to the microflora of the nasopharynx. The use of an ointment containing less than 3% of the antibiotic, when administered once with ultrasound, is not enough to create a high concentration of the drug in the focus of inflammation. An increase in the concentration of the antibiotic over 5% leads to the manifestation of a toxic effect on the body.
The ointment is applied to the skin 2 cm below the apex of the mastoid process, located behind the external auditory meatus, on both sides. This area corresponds to the location of the upper cervical lateral lymph node, which is the first order lymph node for the nasopharyngeal tonsil [2] Then, an ultrasound exposure is performed with an exposure of 5 minutes alternately on both sides per session for 10-15 days depending on the age of the child and the severity of the clinical manifestations according to the generally accepted method [3]
Treatment by the proposed method was carried out in 17 children with adenoiditis at the age of 3-4 years. All patients received a positive clinical effect due to the maximum concentration of the antibiotic in the focus of inflammation: nasal breathing improved, mucopurulent discharge from the nose stopped. With repeated bacteriological studies, pathological microflora was not sown. No side effects and relapses were noted.
Example 1. Child O. 3 years old, complained of a periodically occurring runny nose of a mucopurulent nature, cough, difficult nasal breathing during sleep. During the last year, he suffered three respiratory viral infections complicated by bronchitis. Examination of the ENT organs revealed edema and congestive hyperemia of the mucous membrane of the nasal concha, mucopurulent discharge in the lumen of the common nasal passages. The mucous membrane of the posterior pharyngeal wall is hyperemic, thickened, from under the soft palate a strip of viscous mucopurulent discharge. In the study of peripheral blood ESR 12 mm in 1 h. In a bacteriological study of microflora from the nasopharynx, hemolytic streptococcus sensitive to ampicillin was seeded. Prepared 3% ampicillin ointment and conducted a course of phonophoresis according to the proposed method for 10 days. The child's condition improved: the runny nose stopped, nasal breathing became free. I stopped worrying about the cough, which was caused by the drainage of the discharge along the back of the throat. The mucous membrane of the nose and posterior pharyngeal wall turned pink, and swelling of the nasal concha decreased. ESR 7 mm per 1 h, with repeated examination of the smear from the nasopharynx on the microflora, sterile culture. No disease recurrence was observed within 3 months.
Example 2. Child K., 4 years old, suffered from recurrent acute otitis media (3 episodes over 4 months). He complained of difficulty in nasal breathing and a runny nose periodically. For 4 months, antibiotics were administered intramuscularly several times. In this case, as in many others, the antibacterial therapy carried out intramuscularly had a temporary effect, did not prevent recurrence of otitis media and exacerbations of chronic adenoiditis. Pathogenic microflora continued to be sown from the nasopharynx. With anterior rhinoscopy, mucopurulent discharge in the nasal cavity, edema of the anterior ends of the nasal concha. With posterior rhinoscopy, the nasopharyngeal tonsil is hyperemic, covered with viscous mucus, covers the upper third of the lumen of the choan. With stethoscopy, the eardrum is dull, somewhat retracted. Diagnosed with adenoiditis, bilateral acute recurrent otitis media. During bacteriological examination of the discharge from the nasopharynx, Staphylococcus aureus, sensitive to the effects of tetracycline, was isolated. We performed the treatment of the proposed method with 5% tetracycline ointment for 15 sessions. The condition of the child improved, nasal discharge stopped, the nasal mucosa acquired a normal color, and swelling of the nasal concha was reduced. When taking a smear from the nasopharynx again, staphylococcus was not sown. There were no relapses of acute otitis media within 3 months.
Thus, in the treatment of the proposed method, the action of the drug is localized in the focus of inflammation through the use of ultrasound, which has the property of stimulating lymph circulation, enhancing the antibiotic lymphotropy [4]
The method is technically simple, eliminates injuries to the child’s psyche, injections, an undesirable general effect of antibiotics on the body, which is important in the treatment of young children.
