Abstract
Description
The invention relates to medicine, namely to transplantology, anesthesiology and resuscitation, and can be used to prevent reperfusion injuries of internal donor organs during their transplantation in the perioperative period - pre-, intra- and postoperative.
Under conditions of ischemia, enzymatic systems of biological oxidation are damaged, the purpose of which is the production of energy, its substrate is adenosine triphosphate (ATP). For this, an electron passes from oxygen through a circle of amino acids (Krebs, Warburg cycles), as a result of which the final product is formed - ATP molecules. During ischemia, ATP is converted to adenosine monophosphate (AMP), followed by the formation of adenosine, inosine, hypoxanthine. The main production of tissue-damaging oxygen radicals occurs during reperfusion, when xanthine oxidase in the presence of oxygen converts hypoxanthine to urate and oxygen radicals.
In terms of sensitivity to ischemic damage to enzyme systems, organs are distributed as follows: brain, liver, kidneys, myocardium, muscles, endocrine glands, skin.
Oxygen is necessary for normal metabolism and energy production, but the whole process consists of three components - metabolite transport, enzyme systems, oxygen. The absence of at least one of the components leads to disruption of this process.
When an inadequately large amount of oxygen arrives in tissue with enzymatic amino acid biological oxidation systems damaged by ischemia, tissue peroxidation occurs, in which cell membranes are damaged, numerous enzymes are inactivated and polysaccharides are depolymerized.
Thus, peroxidation is a consequence of not only absolute, but also relative hyperoxia. Obviously, the restoration of oxygen in the tissues must begin with low oxygen concentrations, as well as the use of antioxidants, antihypoxants and energy donors.
Various methods of protection are proposed for the prevention of reperfusion injuries of the donor organ, mainly medication, both in the donor’s body, as part of preserving solutions, and at the reperfusion stage and further in the recipient’s body.
A known method of restoring the viability of damaged donor organs [RU patent for the invention No. 2423931]. According to the proposed method, after organ removal, its extracorporeal perfusion with a solution is carried out. Which structure includes not less than 25% of Custodiol preservative solution, not less than 20% perfluoran, not less than 1.5 million ME streptokinase, not less than 50,000 ME heparin, not less than 300 micrograms of prednisolone for at least 2 hours, the contour of hardware perfusion with the help of a leukocyte filter, activated leukocytes are removed, and the temperature of the perfusate is reduced to 5-12 ° C.
However, this method, used when collecting organs from asystolic donors, allows us to solve only the issue of increasing the transplant storage time, determining the recipient and its antigenic typing, but it does not solve the main problem - optimizing the inclusion of the transplant into the bloodstream, which is extremely necessary for patients during transplantation from living related donors.
- 31 с. Prevention methods are also known, including the introduction of a number of medications, the pathogenetic mechanisms of action of which (antioxidant, disaggregant) should help to weaken reperfusion injuries of donor organs. Among these drugs, of particular interest is the domestic drug "Perftoran", which has not only antioxidant, but also oxygen-transporting properties, as well as drugs from foreign companies - fraksiparin - an analogue of heparin, and ticlide - an analogue of the disintegrant of trental [Balkarov A.G. Drug anti-ischemic protection of the renal transplant in the intraoperative and early postoperative period: Author. dis. Ph.D. - M.: Research Institute of Transplantol. and arts. bodies of the Ministry of Health of the Russian Federation, 2000. - 31 p. Rus.].
However, these methods do not take into account the prevention of reperfusion injuries of donor organs that are still functioning in the body of the donor at the stage of the preoperative period.
The closest analogue to the claimed invention is a method for the prevention of reperfusion injuries of a donor organ, in particular a kidney [Inf. materials are attached, website: http://perftoran.ru/index.php?option=content&task=view&id=128]. It is characterized by the introduction of therapeutic doses of the drug "Perftoran" into the body of the donor in preparation for withdrawal and into the body of the recipient.
However, the proposed method does not fully compensate for ischemic reperfusion injury, since it insignificantly affects the enzyme systems and metabolite transport in tissues, and also reduces the viability of donor organs due to developing hyperoxia.
The task of the invention is to improve blood flow and function of the donor organ in the perioperative period due to the prevention of its expected reperfusion injuries.
The essence of the claimed invention lies in the fact that in a method for the prevention of reperfusion injuries of donor organs, comprising administering the drug “Perftoran” to the body of the donor before removing the donor organ and to the recipient during and after the donor organ is included in the bloodstream, the dose of the drug “Perftoran” is 2-3 ml / kg body weight; additionally, along with the introduction of the Perftoran preparation, Neoton is infused in a dose of 50-100 mg / kg of body weight, in the postoperative period, the patient receives the Wessel Doué F dose in the dose for 4-6 days 500-600 mg per day.
There is also a method for the prevention of reperfusion injuries of donor organs with the above characteristics, in which the introduction of the drug "Wessel Due F" is carried out by intravenous administration on the first day at a dose of 600 mg and oral administration at a dose of 500 mg for 3-5 days.
The technical result of the claimed invention.
The introduction of the drug "Neoton" during periods of organ harvesting from the donor and the inclusion of the donor organ in the bloodstream of the recipient improves tissue metabolism due to the fact that this drug is similar in its chemical structure to macroergic endogenous phosphocreatine. Performing this manipulation helps to slow down the process of destruction of the sarcolemma of ischemic cells and provides intracellular energy transport, which, in turn, improves microcirculation of tissues and organs and prevents the development of both ischemia and apoptosis of the cell. The combination of the use of Perftoran and Neoton preparations during certain periods of a critical condition allows us to maximize the viability of the cells of the donor organ and its functional capabilities, which helps to reduce the rehabilitation time for donors and recipients in the postoperative period.
The introduction of the drug "Wessel Duet F" in the postoperative period contributes to the early restoration of the functions of the endothelium and vascular-platelet hemostasis.
The implementation of the introduction of drugs in the doses indicated in this application avoids the violation of the homeostatic potential of the body of both the donor and the recipient.
Thus, the claimed method improves blood flow and the function of the transplanted donor organ in the perioperative period and allows you to: prevent reperfusion damage to the graft by normalizing hemodynamic parameters, avoid thrombotic complications and subsequent necrosis of the graft, and also stabilize the functions of organs and systems of the recipient against the background of the use of drugs, providing normal energy.
A method for the prevention of reperfusion injuries of a donor organ is as follows. When a donor organ is taken from a living related donor until it is turned off from the bloodstream, Perftoran is administered at a dose of 2-3 ml / kg body weight for 10-15 minutes. Then, Neoton is infused to 50-100 mg / kg body weight for 10-20 minutes. After that, the donor organ is turned off from the bloodstream of the donor, removed from the body of the donor, washed by perfusion of the cuododiol and placed in a refrigerated state in the thermal container, in which it is transferred to the operating room where the recipient is located.
The recipient undergoes surgical access and begins the process of including the donor organ in the bloodstream of the donor. At the same time, the Perftoran drug in a dose of 2-3 ml / kg of body weight and the Neoton drug are infused in a dose of 50-100 mg / kg of body weight in the recipient’s body during and after the donor organ is included in the bloodstream . In the postoperative period, the drug “Wessel Due F” is administered in a dose of 500-600 mg per day to the recipient's body for 4-6 days.
In particular, the administration of the Wessel Douay F preparation is carried out by intravenous administration on the first day at a dose of 600 mg and oral administration at a dose of 500 mg for 3-5 days.

6 days after surgery, the patient restored the functions of the transplanted kidney - concentration and filtration, accompanied by a normal amount of filtered urine per day with stabilization of its specific gravity. There was a normalization of the water-electrolyte and nitrogen balance. These indicators testified to the complete elimination of reperfusion disorders of the kidney.
