Наиболее близким к предлагаемому изобретению является способ, заключающийся в проведении эпидуральной анальгезии введением 20-25 мл 0,75-1% раствора ропивакаина и общей анестезии пропофолом 5-6 мг/кг/час с миорелаксацией и ИВЛ и послеоперационной эпидуральной анальгезии введением 0,2% раствора ропивакаина через дозатор со скоростью 10-12 мл/час в течение 2-3 суток (Лукьянов Д.С., Шевченко В.П., Рерих В.В. Способ сбалансированной регионарной анальгезии в периоперационном периоде при высокотравматичных операциях на грудном и поясничном отделах позвоночника осуществляют следующим образом: пациентам после внутривенной седации мидазоламом устанавливают эпидуральный катетер на 3-4 сегмента выше предполагаемого уровня хирургического вмешательства в грудном отделе позвоночника от уровня Th4 до Th12 или L1. После проведения тест-дозы (2% раствор лидокаина 2-3 мл) эпидурально вводят болюс 0,75% р-ра ропивакаина от 3 до 10 мл дробно и фентанила 50-100 мкг и начинают инфузию смеси 0,2% раствора ропивакаина с фентанилом (2 мкг/мл) и адреналином (2 мкг/мл) со скоростью 5-10 мл/час. Индукцию в общую анестезию проводят севофлураном 8 об.% и фентанилом (1 мкг/кг). Осуществляют интубацию трахеи, поддерживают анестезию ингаляцией севофлурана 0,6-1,2 об.%, проводят миорелаксацию и ИВЛ. Пробуждение и экстубация пациентов проводятся в операционной. Анестезиологическое обеспечение. Премедикация в палате за 30 мин до операции - реланиум 10 мг, супрастин 1 мл. После внутривенной седации мидазоламом 5 мг катетеризировали эпидуральное пространство на грудном уровне Th6-Th7, катетер проведен краниально на 5 см, введено 10 мл 0,75% раствора ропивакаина дробно по 5 мл в течение 10 мин, 100 мкг фентанила. Начата эпидуральная инфузия 0,2% раствора ропивакаина в смеси с фентанилом 2 мкг/мл и адреналином 2 мкг/мл со скоростью 5-10 мл/час. Индукция в анестезию - болюсная - севофлуран 8 об%, FiO2 - 100%, 8 л/мин, фентанил 100 мкг, интубация трахеи без введения релаксантов, ИВЛ в режиме IPPV, нормовентиляция. Катетеризация правой подключичной вены для проведения инфузионно-трансфузионной терапии и контроля ЦВД. Катетеризация мочевого пузыря для контроля почасового диуреза. Поддержание общей анестезии ингаляцией севофлурана 0,8-1,0 об.%, MAC - 0,5%, FiO2 - 40%, Low-Flow, SpO2 c оценкой глубины наркозного сна (BIS=50-60%). Миорелаксация инфузией тракриума 0,4 мг/кг/час (38 мг/час). Поддержание гемодинамики: АДс - 90-100 мм рт.ст., ЧСС 60-66 уд. в мин, АДср - 60-65 мм рт.ст., СИ - 2,3-2,6. Кровопотеря во время операции составила 350 мл, реинфузия аутоэритроцитов (Cell Saver) - 110 мл. Объем ИнТ - 1000 мл кристаллоиды (раствор Рингера), 500 мл - коллоиды (Волювен), диурез составил 200 мл, длительность операции - 4 часа. В конце операции пациентка проснулась, команды выполняет, спонтанное дыхание восстановилось, адекватное, ЧД - 16 в мин, SpO2 - 98% при дыхании воздухом; больная экстубирована на операционном столе через 6 минут после окончания ушивания раны, переведена в отделение реанимации. Проведена оценка неврологического статуса, движения в конечностях сохранены, болевой синдром отсутствует. Продолжена эпидуральная анальгезия 0,2% раствором ропивакаина с фентанилом 2 мкг/мл и адреналином 2 мкг/мл со скоростью 4-8 мл/час в течение 1 суток в ОРИТ. После операции кровопотеря по дренажу составила 250 мл, реинфузия 100 мл аутоэритроцитов. Утром пациентка была переведена в палату, где продолжена эпидуральная анальгезия через инфузионную помпу в течение 2-х суток. Способ сбалансированной регионарной анальгезии в периоперационном периоде при высокотравматичных операциях на грудном и поясничном отделах позвоночника позволяет надежно прерывать болевую импульсацию на уровне задних рогов спинного мозга. Сочетание регионарной анальгезии с мощным и высокоуправляемым ингаляционным наркозом севофлураном позволяет поддерживать поверхностный уровень седации, что способствует быстрому пробуждению на операционном столе, ранней экстубации пациентов, ранней послеоперационной активизации, что способствует профилактике ранних осложнений. Быстрое управляемое пробуждение больных позволяет оценить неврологический статус в конце операции. Снижение разовой болюсной дозы ропивакаина позволяет избежать нежелательных побочных гемодинамических, а также токсических реакций у пациентов. Применение продленной грудной эпидуральной анальгезии смесью ропивакаина, фентанила и адреналина после операции на позвоночнике способствует более качественному обезболиванию, минимальному числу побочных эффектов, ранней активизации (поворот, подъем и ходьба), профилактике послеоперационного пареза кишечника, уменьшает число случаев периоперационной ишемии миокарда.A method of balanced regional analgesia in the perioperative period during highly traumatic operations on the thoracic and lumbar spine allows reliably interrupting pain impulse at the level of the posterior horns of the spinal cord.
