Изобретение относится к области медицины, в частности пульмонологии, и может быть использовано для диагностики типа мотивационной сферы к образовательному процессу у больных хронической обструктивной болезнью легких (ХОБЛ).The invention relates to medicine, in particular pulmonology, and can be used to diagnose the type of motivation for the educational process in patients with chronic obstructive pulmonary disease (COPD).
Прототипом избран способ диагностики типа мотивационной сферы к обучению с помощью тест-опросника МУН [1]. Способ заключается в том, что тип мотивационной сферы оценивается по сумме набранных баллов, полученных в ходе тестирования.The prototype selected a method for diagnosing the type of motivational sphere for learning using the EOR test questionnaire [1]. Цель изобретения заключается в повышении эффективности способа диагностики типа мотивационной сферы к обучению у больных ХОБЛ в общеклинической практике путем интегральной оценки объема форсированного выдоха за 1 секунду (ОФВ1) в %, общих и специфических параметров качества жизни (КЖ), соответственно психического здоровья (ПЗ); социальной активности (СА); воздействие в баллах.The purpose of the invention is to increase the efficiency of the method for diagnosing the type of motivational sphere for training in patients with COPD in general clinical practice by integrating the forced expiratory volume per 1 second (FEV 1 ) in%, general and specific parameters of quality of life (QOL), and mental health (PP) ); social activity (SA); impact in points.

Исходные данные составили: ОФВ1 69% от должного значения; показатель СА 60 баллов, показатель ПЗ 49 баллов, показатель шкалы Воздействие 48 балла.The initial data amounted to: FEV 1 69% of the due value; CA indicator 60 points, PZ indicator 49 points, scale indicator Impact 48 points.

Больной прошел курс обучения по программе ХОБЛ. При анализе динамики показателей в конце исследования, нами зафиксировано достоверное улучшение параметров среди большинства шкал общего КЖ, всех значений специфического КЖ, а также отмечено снижение уровня общей тревоги и депрессии (р2<0,01). Так, показатель шкалы Воздействие улучшился на 28%, что соответствовало 34,6±2,6 баллам (р1<0,01, по сравнению с исходным значением), значения шкал СА и ПЗ были лучше соответственно на 16% (71,2±2,9; р2<0,01) и 20% (61,4±2,9; р2<0,01) по сравнению с исходными данными, что свидетельствует о значимом положительном влиянии образовательного вмешательства на КЖ больного.The patient completed a training course in the COPD program. Больной прошел курс обучения по программе ХОБЛ. В конце исследования отмечено незначительное улучшение показателей общего КЖ только по шкалам СА и ПЗ, которые были выше исходных значений на 9% (р2<0,05) и 7% (р2>0,05) соответственно. Анализируя показатели специфического КЖ, нами выявлено минимальное достоверное улучшение параметров только по шкале Воздействие на 12% (р2<0,05). Общий уровень тревожности и субдепрессивной симптоматики сохранялся на прежнем уровне (р2>0,05), что свидетельствует о менее значимом влиянии образовательного вмешательства на КЖ больного.The patient completed a training course in the COPD program. Учет типа мотивационной сферы к обучению у больных ХОБЛ позволяет проводить дифференцированный отбор пациентов на образовательные программы, что может повысить их эффективность.Taking into account the type of motivational sphere for training in patients with COPD allows differentiated selection of patients for educational programs, which can increase their effectiveness.
Предлагаемый способ прошел клиническую апробацию на базе пульмонологического отделения ДНЦ ФПД СО РАМН. С использованием способа проведена диагностика мотивации к обучению у 48 больных хронической обструктивной болезнью легких.The proposed method has been clinically tested on the basis of the pulmonological department of the DSC FPD SB RAMS. Using the method, the diagnosis of motivation for learning in 48 patients with chronic obstructive pulmonary disease was diagnosed.
Результаты проверки заявленного способа показали его эффективность, правильный прогноз определялся в 89,3% случаев.The verification results of the claimed method showed its effectiveness, the correct prognosis was determined in 89.3% of cases.
Таким образом, предлагаемый способ может быть эффективно использован в качестве скрининга для диагностики мотивации к обучению у больных хронической обструктивной болезнью легких в общеклинической практике.Thus, the proposed method can be effectively used as a screening for the diagnosis of motivation to learn in patients with chronic obstructive pulmonary disease in general clinical practice.
