При этом получение такого признака отличалось бы технической простотой, отсутствием использования каких-либо специальных приборов и химических реактивов.The task of the invention is to remedy these disadvantages. Нами были изучены особенности морфологической картины фаций сыворотки крови у послеоперационных больных с тяжелой сочетанной патологией, которым во время операции или после нее переливали эритроцитную массу при массивной кровопотере. Дальнейшее наблюдение за этими больными и исходом критического состояния дали нам основания трактовать выявленные морфологические особенности фации сыворотки крови в качестве оценочных.We have studied the features of the morphological picture of the serum facies in postoperative patients with severe concomitant pathology who received red blood cells during or after the operation during massive blood loss. Для этого в способе оценки состояния больного после переливания эритроцитной массы при критических состояниях, включающем исследование сыворотки крови больного, предложено взятие крови у больного проводить после переливания ему эритроцитной массы. При этом для исследования готовят пробу сыворотки крови больного методом клиновидной дегидратации и при выявлении в краевой зоне фации структуры типа «языка» оценивают дальнейшую динамику процесса восстановления гомеостаза как благоприятную, а при их отсутствии - как неблагоприятную, вплоть до летального исхода.For this, in the method for assessing the patient’s condition after transfusion of the erythrocyte mass in critical conditions, including a study of the patient’s blood serum, it is proposed to take blood from the patient after transfusion of the red blood cell mass. Объяснение проявления описанного выше прогностического феномена состоит в том, что в организм тяжелого больного вливается донорская эритроцитная масса, при этом небольшая часть эритроцитов обязательно гемолизирована. Попадание в кровь чужеродного белка (антигена) в организм человека, способного противостоять действию раздражающего фактора, вызывает у него стимуляцию иммунной системы с формированием антител. Активная реакция иммунной системы на чужеродный агент характеризует достаточно высокую способность организма к восстановлению гомеостаза и, как следствие, выходу организма из критического состояния. Отсутствие иммунной реакции в ответ на вливаемую эритроцитную массу свидетельствует о полном угнетении иммунологической активности организма и, возможно, у них жизненно важных систем, и, как следствие, гибель больного при полном исчерпании резервных возможностей и неспособности организма выйти из критического состояния, т.е. о неблагоприятном прогнозе.An explanation of the manifestation of the prognostic phenomenon described above is that a donor erythrocyte mass is poured into the body of a severe patient, while a small part of the red blood cells is necessarily hemolized. The absence of an immune response in response to the injected red blood cell mass indicates a complete suppression of the immunological activity of the body and, possibly, their vital systems, and, as a result, the death of the patient with the complete exhaustion of reserve capabilities and the inability of the body to emerge from a critical state, i.e. about an unfavorable prognosis.

Больной Р., 65 лет. Страдает комплексной соматической патологией (системный атеросклероз, гипертоническая болезнь, нефролитиаз). Оперирован по поводу резекции желудка. Во время операции в связи с массивной кровопотерей ему была перелита эритроцитная масса.Patient R., 65 years old. Осуществили забор крови из локтевой вены, получили сыворотку путем отстаивания в пробирке (возможно центрифугирование), нанесли каплю сыворотки крови в количестве 0,01-0,02 мл на поверхность предметного стекла, высушили при температуре 24-25°С и через 18 часов (по времени проба выдерживается от 18 до 24 часов) оценили картину фации при микроскопии. Больной П., 67 лет. Страдает комплексной соматической патологией (системный атеросклероз, ишемическая болезнь сердца, аденома предстательной железы). Проведена холецистэктомия. Во время операции в связи с массивной кровопотерей ему была перелита эритроцитная масса.Patient P., 67 years old. Сразу после операции была взята кровь и картина оценена по указанному способу.
