Abstract
Description
The invention relates to the field of instruments and accessories for surgery and can be used for spatial localization of stones and their fragments during lithotripsy.
State of the art
A device is known for spatial localization of stones and their fragments during lithotripsy (US patent # 7,462,183 B2 Methods for deploying conformed structures in body lumens), consisting of a tubular catheter with an open end, inside of which there is a rigid conductor and a retainer in the form of an elastic tape of constant volume, one the end of which is rigidly fixed to the end of the conductor, and the other is fixed with the possibility of free sliding along the conductor.
The spatial localization of stones and their fragments during lithotripsy with the help of an analogue is achieved by the fact that the end of the catheter is carried out behind the stone and from it, using a conductor, a retainer is brought out into the space behind the stone, which straightens due to its elastic properties and thereby prevents the migration of stones and their fragments.
The disadvantage of an analogue is low efficiency.
Closest to the claimed device in technical essence is the device described in the patent (U.S. Pat. No. 7,674,283 B2 Radially expandable stents) and consisting of a tubular catheter with an open end, inside which is a conductor and retainer with a developed external surface of elastic material. The shape of the retainer is chosen so that outside the catheter due to the elastic properties, the diameter of the retainer exceeds the diameter of the catheter, while its total volume remains unchanged.
Spatial localization of stones and their fragments during lithotripsy using a prototype is achieved by the end of the catheter being carried out behind the stone and out of it using a conductor or an additional piston, the retainer is brought out into the space behind the stone, which changes its shape due to its elastic properties and therefore prevents migration of stones and their fragments.
The disadvantage of the prototype is low efficiency.
The disadvantages are due to the fact that to ensure the required hydrodynamic resistance, the prototype retainer has high elasticity, a large longitudinal size and, as a result, its fragments are traumatic. In addition, the latch located behind the stone is extremely difficult to remove from the organ in case of refusal of lithotripsy, breakage of the conductor, mechanical disruption of integrity, or in another extraordinary situation.
The purpose of the invention
The aim of the invention is to increase efficiency.
The goal is achieved by the fact that in the device for preventing the migration of stone and its fragments during lithotripsy, consisting of a catheter and a conductor, the clamp located inside the catheter has a cylindrical shape and is made of a permeable spongy material with the ability to repeatedly increase its volume by increasing its transverse dimensions when placing in a water-containing medium, for example, a material with programmable characteristics based on collagen, and the inner space of the catheter between one and end of the catheter and the nearest end retainer filled waterproofing material.
The novelty of the claimed features consists in imparting a uniform cylindrical shape to the fixer, making it from a permeable spongy material with the ability to repeatedly increase its volume in the aquatic environment due to its absorption and waterproofing of the end of the catheter closest to the fixative.
A positive effect is achieved due to the fact that, once in a water-containing medium, the fixative swells. In this case, the length of the retainer remains almost unchanged, and its cross section increases many times. As a result, being located between the stone and the kidney, the retainer prevents the stone from migrating towards the kidney. In addition, the acoustic impedance of the swollen fixative becomes almost indistinguishable from the environment, since about 99% of its volume is water. Due to this, the fixator becomes acoustically transparent and the shock waves generated by the lithotripter do not have a mechanical effect on the fixator and do not damage it. In this case, the waterproofing of the end of the catheter closest to the clamp prevents the swelling of the clamp inside the catheter.

(5) Information confirming the possibility of carrying out the invention
1.An example of a specific implementation of the proposed device is presented in figure 1. It consists of a catheter (1) in the form of a thin-walled tube, inside of which there is a thin and flexible conductor (2). At the end of the
(8).Figure 2a shows a diagram of the urethra with a stone (8) located inside the ureter (7).
Фиг.2а, 2б).The purpose of the proposed device is to prevent the migration of
5.For this purpose, according to Fig. 3a, an ureteroscope (9) is inserted into the
8.Once in the water-containing environment of the
2.Since the
