Пациенту была назначена общепринятая медикаментозная терапия, включающая в себя антибиотикотерапию, минералотерапию, антиоксидантную и наружную терапию, а именно: юнидокс солютаб - 50 мг ежедневно на ночь в течение 4 недель; глюконат цинка - по 50 мг ежедневно в течение 4 недель; витамин Е - по 400 ME в сутки вместе с приемом пищи - 4 недели; наружно - днем крем бензоила пероксид, на ночь - крем, содержащий 0,1% адапален - 4 недели. Дополнительно с первого дня лечения пациенту были назначены обкалывания пораженной акне зоны кожи лица и груди озоно-кислородной смесью. Озоно-кислородную смесь получали при помощи аппарата - синтезатора газовых озоно-кислородных смесей низких концентраций физиотерапевтического А-с-ГОКСф-5-02-ОЗОН. Регистрационное удостоверение Министерства здравоохранения РФ №29/06081001/3948-02 от 20.06.02. Концентрация озона 3500 мкг/мл. Вколы производились под каждый морфологический элемент угревой болезни, объемом 0,2 мл на закрытые комедоны и папулы, объемом 0,4 мл на пустулы. Периодичность манипуляций через день в количестве 10 сеансов. Морфологические элементы угревой сыпи в области груди и спины были представлены очагами поствоспалительной гиперпигментации, островоспалительные элементы отсутствовали. Самочувствие пациента хорошее. Общий анализ крови: гемоглобин - 150 г/л, эритроциты - 4,9×1012/л, цветовой показатель - 0,92, тромбоциты - 260×109/л, лейкоциты - 5,9×109/л, нейтрофилы: палочкоядерные - 2%, сегментоядерные - 55%; эозинофилы - 2%, базофилы - 1%, лимфоциты - 31%, моноциты - 9%, СОЭ - 10 мм/ч. Оценка иммунного статуса - нормализация иммуно-регуляторного индекса: CD4+ - 38%, CD8+ - 21%, ИРИ - 1,9 у.е.; нормализация деятельности фагоцитарного звена: в НСТ-тесте коэффициент стимуляции - 1,9 у.е.Treatment results: at the end of the course, the following was noted - pustular and papular elements were absent, there remained a small number of open and closed comedones in the area of the wings of the nose, chin. Пример №2: больной Л., 22 года, студент. Обратился на консультативный прием к дерматовенерологу РостГМУ с жалобами на высыпания на коже лица и вызванный этим косметический дискомфорт. Объективно: патологический процесс носил ограниченный характер, локализуясь на коже лица. Морфологические элементы были представлены множественными папулами, пустулами, конглобатными элементами. Общий анализ крови: гемоглобин - 149 г/л, эритроциты - 4,9×1012/л, цветовой показатель - 0,9, тромбоциты - 260×109/л, лейкоциты - 6,4×109/л, нейтрофилы: палочкоядерные - 4%, сегментоядерные - 53%; эозинофилы - 1%, базофилы - 1%, лимфоциты -30%, моноциты - 11%, СОЭ - 6 мм/ч. Таким образом, положительный клинический результат при применении данного способа лечения угревой болезни был достигнут, при этом отмечалась нормализация показателей иммунного статуса и снижение интенсивности воспаления.Thus, a positive clinical result with the use of this method of treating acne was achieved, while normalization of the immune status indicators and a decrease in the intensity of inflammation were noted.
Под нашим наблюдением находилось 32 пациента, страдающих угревой болезнью средней и тяжелой степени, лечение которых осуществлялось в клинике кожных и венерических болезней РостГМУ согласно предлагаемому способу. Среди них 2 пациента страдали патологией сердечнососудистой системы: 1 пациент имел кардиостимулятор, 1 имел врожденный порок сердца, 4 страдали воспалительными заболеваниями в острой стадии, а именно острой респираторно-вирусной инфекцией и хронической реактивирующей герпесвирусной инфекцией. Проведенное согласно предлагаемому способу лечение было эффективно у 26 пациентов из 32, находящихся под наблюдением, в том числе страдающих описанной выше сопутствующей патологией, что составляет 81%.Under our supervision were 32 patients suffering from moderate to severe acne, the treatment of which was carried out in the clinic of skin and venereal diseases of Rostov State Medical University according to the proposed method. Conducted according to the proposed method, the treatment was effective in 26 of 32 patients under observation, including those suffering from the concomitant pathology described above, which is 81%.
Ретроспективно нами было изучено 50 историй болезни пациентов клиники кожных и венерических болезней РостГМУ, страдающих угревой болезнью средней и тяжелой степени, проходивших курс лечения согласно способу, описанному в прототипе.
