The invention relates to medicine, in particular to medical equipment, and can be used in the treatment of patients with extensive burns, severe injuries, wounds, and in other cases requiring a long stay of patients in bed in a stationary state.
Known systems and devices for treating patients with burns:
И. Вести АМН СССР, 1979, №3, с.56-57).- Cabs with a laminar air flow for the maintenance of victims in abacterial conditions. However, this did not solve the problem of treating extensive circulatory burns and lesions with predominant localization on the back surface of the body (Kudin M.I. Vesti, Academy of Medical Sciences of the USSR, 1979, No. 3, pp. 56-57).
16.08.91.- The bed of the company “Clinitron” (“Traumatology and Orthopedics” A guide for doctors in three volumes. Edited by Yu.G. Shaposhnikov. M: Medicine, 1997. Volume 1, p.336). The bed is made in the form of a bathtub filled with glass microspheres and covered with a filter screen. The flow of warm air supplied from below causes the process of fluidization of the microspheres, and the patient's body experiences a pressure proportional to its immersion in water. The disadvantage of this method is the ingress of infected fluids into the bed (sweat, blood and other body fluids), which form conglomerates with microspheres, on which the bacterial flora actively develops. Passing warm air, capturing microbial bodies, washes the patient’s wounds and thereby increases the concentration of microbes on the wound. Conglomerates also disrupt the fluidization process of microspheres. All this requires frequent changes in the entire volume of microspheres; bactericidal additives (substances of calcium, magnesia, bismuth or a carbon sorbent) must be introduced into the composition of the microspheres. Patent RU 2026048 C1 dated 08.16.91.
As a prototype of the invention adopted the system of hospital care in need of prolonged immobilization. Patent RU 2288688 C2 dated January 28, 2005. The system includes a bathtub, a lifting mechanism, a soft, waterproof, detachable in the horizontal plane, shell. Inside the shell is lined with layers of knitted cotton bulk fabric yarn. The patient is placed in the casing, both halves of the casing are hermetically closed, leaving the patient's face open, and in the ankle zone, an exhaust ventilation system is connected to the casing. Then, the patient in the shell, using a lifting mechanism, is lowered into a bath with a solution of sodium chloride 2-4% to a depth of 10-15 cm. By switching on the exhaust ventilation, create air movement between the waterproof shell and the patient's body and thereby remove the moisture absorbed by the cotton tissue from the surface of the patient.
The disadvantage of this method is that the patient, for conducting medical procedures and sending natural necessities, must be completely removed from the saline solution and the membrane. This procedure is very painful and time-consuming and requires frequent repetition, to prevent adhesion of the membrane to the wounds, leading to granulation injury, worsening, it is necessary to ensure the integrity of the membrane from saline.
The present invention will allow for more effective and high-quality treatment of burn patients, makes the treatment process less painful and time-consuming, aimed at eliminating granulation injuries, secondary necrosis and wound deepening as a result of adhesion of a layer of knitted fabric of the sheath, screen, sheets and gauze dressings.
8.To achieve the claimed technical result, the proposed device (Fig. 1) for hospitalization of a patient during prolonged immobilization includes: a bathtub filled with a liquid inert to the human body with a density of 1.2-1.6 g / cm 3 (glycerin) 1; a mechanism for lowering and raising the patient from the bath, made in the form of a broadband mesh wound on rollers fixed to the edge of the bath 2; soft helmet-pillow for fixing the patient’s head in the bath (to prevent the head from lowering below the level of the solution in the bath) 3; a device for supplying medicine and oxygen to the bath and to the patient’s body 4; a container with a clean solution connected to the bath 5; capacity for contaminated solution with infected fluids (sweat, blood and other excreta of the patient’s vital activity) 6; pump 7; heater 8.
A patient is placed on a broadband grid stretched over the bathtub. A helmet-pillow is put on the patient’s head, covering the occipital part of the head, ears and preventing the lowering and turning of the head below the level of the solution. Using the rollers, the mesh, together with the patient, is lowered into a bath solution heated to a predetermined temperature. The bath in the head has a narrowing and a protrusion, to prevent the head from lowering below the liquid level. Due to the high density of the bath solution, the patient is partially immersed in the bath, the net is immersed a little deeper by 2-3 cm (to ensure free access of the bath solution, saturated with oxygen and drugs, to the patient's body and insurance against overturning). According to the doctor’s prescription, they include a device for supplying purified air enriched with oxygen and necessary medicines, which rise with the flow of air bubbles to the affected areas of the patient’s body. Periodically or if necessary, the pump is turned on, which removes the contaminated solution into the disposal tank and pumps fresh. This complex allows you to deliver medicine and oxygen to the wounds without lifting, turning over or injuring the patient. Significantly facilitates the care of the patient when sending natural necessities. Placing the patient in a bath with a liquid of increased density will eliminate granulation injuries, secondary necrosis, and wound deepening as a result of adhesion of the tissue of the membrane, screen, sheet and exclude gauze dressings.
