Обзор современной литературы, посвященной красному плоскому лишаю, свидетельствует о неуточненной этиологии и отсутствии целостного представления о патогенезе заболевания. Наибольшее внимание исследователи уделяют иммунным нарушениям. Не исключается возможность участия герпес-вирусов в гиперактивации лимфоцитов и формировании аутоиммунных нарушений. В иммунопатогенезе принимают участие как аутоиммунные механизмы, так и аллергические реакции, активаторы иммунной системы (преимущественно T1-хелперы), выявляется недостаточность некоторых звеньев иммунного ответа. Идентификация специфических для красного плоского лишая иммунных механизмов, расшифровка аспектов вирусной теории этиопатогенеза заболевания открывает широкие возможности для разработки новой, эффективной и индивидуальной специфической терапии.A review of modern literature on lichen planus indicates an unspecified etiology and lack of a holistic understanding of the pathogenesis of the disease. Известен способ лечения больных красным плоским лишаем делагилом. Его терапевтический эффект в основном обусловлен подавлением синтеза нуклеиновых кислот. Многолетние наблюдения показали, что наряду с положительным терапевтическим эффектом при применении этого препарата, в значительном проценте случаев наблюдается развитие нежелательных побочных явлений со стороны центральной нервной системы (ЦНС), органов зрения, желудочно-кишечного тракта (ЖКТ), сердечно-сосудистой системы (Справочник Видаль, 1998, М.: АстраФармСервис, 1988 г., Б-186; Машковский М.Д. «Лекарственные средства», М.: Медицина, 1993, с.404-407). Кроме того, применение делагила сопряжено с необходимостью контроля общего анализа крови (определение количества эритроцитов, лейкоцитов, тромбоцитов, уровня общего гемоглобина), общего анализа мочи, функции печени, почек, сердца, наблюдением окулиста. При отмене препарата обычно наступают рецидивы. Для достижения клинического эффекта необходимо проведение 3 курсов терапии делагилом. Курс лечения заключается в назначении препарата внутрь по 250 мг двукратно ежедневно, 5 дней, затем перерыв 2 дня, курсы лечения повторять в течение 1-2 мес (Дерматовенерология. - Под ред. Наиболее близким к предлагаемому способу лечения больных красным плоским лишаем является способ лечения фотохимиотерапией с применением фотосенсибилизаторов внутрь. Фотосенсибилизирующий препарат - метоксален принимают внутрь по 0,6 мг/кг массы тела за 1.5-2 часа до облучения Дж/см2 ультрафиолетового излучения длинноволнового диапазона (УФА). Начальная доза облучения составляет 0,25-1,0 Дж/см2. Заявляемый способ лечения был применен на госпитальном этапе в клинике ФГУ «УрНИИДВиИ» у 12 пациентов с красным плоским лишаем (5 мужчин и 7 женщин, средний возраст составил 37,9 лет, средняя продолжительность заболевания 5,4±1,4 лет. Только двое больных КПЛ не имели сопутствующих заболеваний, у большинства регистрировалась патология желудочно-кишечного тракта, печени, сердечно-сосудистой, нервной систем, сахарный диабет. Предшествующая терапия включала антигистаминные, аминохинолиновые, поливитаминные препараты и наружные глюкокортикостероидные средства. Проведено традиционное параклиническое обследование, дерматологический индекс качества жизни (ДИКЖ) - 30. Симптом Кебнера положительный. На коже правой и левой голени очаги синюшно-фиолетового цвета, плотные с неровной поверхностью, с множественными линейными экскориациями и геморрагическими корочками.
