Коронарный разрез 1 сочетают с разрезом через середину лба, корня носа, продолжая вдоль основания носа справа или слева с окаймлением крыла носа и рассечением верхней губы по средней линии 2. Выполняют разрез по верхнему своду преддверия 3. Скелетируют теменно-височные и лобную области 4, лобный отросток верхней челюсти 5, медиальную часть верхне-глазничного края, со стороны разреза передние стенки обеих верхних челюстей и выделяют подглазничные сосудисто-нервные пучки 6. Производят бифронтальную краниотомию 7. Выделяют субдурально переднюю черепную ямку до planum sphenoidale 8. Осуществляют тракцию лобных долей. Резецируют задне-верхнюю стенку лобного синуса. Производят пропил верхней стенки носа вдоль передней стенки лобного синуса 9, вертикальными пропилами осуществляют остеотомию верхне-глазничных краев с двух сторон 10 и верхне-медиальных стенок орбиты до соединения с лобными отростками 5. Выполняют остеотомию вдоль заднего края лобного отростка, стараясь не повредить носо-слезный канал под контролем зрения со стороны разреза 11. С противоположной стороны с помощью тонкого долота под мануальным контролем выполняют аналогичные манипуляции, поднадкостнично формируя "тоннель" по намеченным линиям 12. Производят остеотомию передней стенки верхней челюсти вдоль края грушевидного отверстия 13 и нижне-глазничных краев, отступая медиально от подглазничного канала на 1,0 мм 14. Вскрывают полость носа вдоль основания и делают остеотомию носовой перегородки на 1/2 15. Тонким долотом остеотомируют боковую стенку носа через распилы передней стенки верхней челюсти и узким долотом производят остеотомию перегородки носа из полости черепа вниз к основанию. После чего кожно-костно-хряще-слизистый лицевой лоскут мобилизуют и отворачивают в сторону 16. Открывается широкий доступ к полости носа и центральной части основания черепа 17. После удаления опухоли и пластического возмещения дефекта передней черепной ямы кожно-костно-хряще-слизистый лоскут укладывают на место, остеотомированные костные фрагменты фиксируют мини-пластинами, производят фиксацию медиального кантуса на прежнем месте. 1. Больной Г., 30 лет, поступил с диагнозом: Хордома верхнего ската. Состояние после субтотального удаления опухоли из переднего трансбазального доступа, курса лучевой терапии (СОД - 40 Гр). Продолженный рост опухоли с разрушением структур основания черепа, распространением в среднюю и переднюю черепные ямки, в обе орбиты, в решетчатый лабиринт, носоглотку.1. Выполнено оперативное вмешательство по предлагаемому способу. Для этого рассекли по средней линии верхнюю губу, далее разрез тканей продолжили вдоль основания носа с окаймлением крыла носа справа, затем к медиальному углу глаза до корня носа и далее по средней линии лба до теменной области. Выполнили коронарный разрез, проходящий через середину теменной области от одной ушной раковины до другой, который соединили с первым разрезом. На всем протяжении раны произвели послойное рассечение тканей до кости. Выполнили разрез слизистой оболочки по верхнему своду преддверия полости рта до кости от 17 до 27 зуба. Поднадкостнично выделили часть передней стенки верхней челюсти с двух сторон с мобилизацией подглазничного сосудисто-нервного пучка. Скелетировали нижнеглазничный край медиальнее подглазничных пучков, лобный отросток верхней челюсти по линии разреза вдоль основания носа, медиальную стенку орбиты с выделением носослезного канала, всю лобную область без верхнеглазничных краев, теменные области с двух сторон. Выполнили бифронтальную краниотомию. Субдурально обнажили переднюю черепную ямку до planum sphenoidale. Резецировали верхне-заднюю стенку лобного синуса. Произвели рассечение дна носа вдоль передней стенки лобной пазухи, далее двумя вертикальными распилами осуществили остеотомию верхнеглазничных краев и верхне-медиальных стенок орбит с двух сторон до соединения с лобными отростками. Под контролем зрения со стороны разреза выполнили остеотомию по задней поверхности лобного отростка, отделяя его от решетчатой кости, не повреждая носослезный канал. Слева остеотомию произвели тонким долотом под мануальным контролем. Далее осуществили остеотомию передней стенки верхней челюсти вдоль краев грушевидного отверстия и нижнеглазничных краев, отступая от медиальной стенки подглазничного канала на 1,0 мм. Тонким долотом выполнили остеотомию боковых стенок носа через распилы передней стенки верхней челюсти. Вскрыли полость носа, вдоль основания остеотомом отделили перегородку носа до середины. Из полости черепа остеотомом рассекли перегородку носа до дна носа. После этого произвели мобилизацию и "откинули" весь кожно-мышечно-костно-слизистый лоскут в противоположную сторону. Таким образом открылся широкий доступ к полости носа и центральной части основания черепа, из которого было выполнено радикальное удаление опухоли "ad blok". После удаления опухоли выполнено пластическое устранение изъяна основания черепа надкостнично-апоневротическим забраловидным лоскутом на поверхностных височных сосудах.Performed surgical intervention on the proposed method. Bifrontal craniotomy was performed. The anterior cranial fossa was subdurally exposed to planum sphenoidale. The upper back wall of the frontal sinus was resected. The nasal fundus was dissected along the front wall of the frontal sinus, then two vertical cuts performed an osteotomy of the upper orbital edges and upper medial walls of the orbits on both sides before joining the frontal processes. Under the control of the vision, an osteotomy was performed on the posterior surface of the frontal process, separating it from the ethmoid bone without damaging the nasolacrimal canal. On the left, an osteotomy was performed with a thin chisel under manual control. Then an osteotomy of the anterior wall of the upper jaw was performed along the edges of the pear-shaped orifice and the lower orbital edges, 1.0 mm from the medial wall of the infraorbital canal. A thin chisel performed an osteotomy of the side walls of the nose through cuts in the anterior wall of the upper jaw. We opened the nasal cavity, along the base of the osteotome, we separated the nasal septum to the middle. From the cranial cavity by the osteotome, the nasal septum was cut to the bottom of the nose. After this, they mobilized and “threw back” the entire skin-muscle-bone-mucous flap in the opposite direction. Thus, wide access was opened to the nasal cavity and the central part of the base of the skull, from which radical removal of the ad blok tumor was performed. After removal of the tumor, plastic repair of the flaw in the base of the skull by periosteal-aponeurotic opral-shaped flap on superficial temporal vessels was performed.
Послеоперационный период протекал гладко. На 4 сутки после операции больной начал садиться. Швы сняты на 9 сутки. Заживление раны первичным натяжением. В отдаленном периоде (срок наблюдения 2 года) состояние остеотомированных для доступа костных фрагментов лица (скуловая кость, кости носа) оставалось стабильным. Послеоперационный период протекал гладко, на 5 сутки больной вертикализирован. Швы сняты на 7-12 сутки. Заживление ран первичным натяжением. В отдаленном периоде (срок наблюдения 6 месяцев) состояние остеотомированных костей лицевого скелета оставалось стабильным.
