В лечении заболеваний щитовидной железы за последние годы появился серьезный конкурент обычным хирургическим методам - эндоскопические и видеоассистированные вмешательства. Наиболее весомыми достоинствами методов представляется минимальная травма тканей, косметическое преимущество, отсутствие лигатур при вмешательстве на шее. Определенную роль играет уменьшение сроков нетрудоспособности и использования обезболивающих средств после операции. В зарубежной литературе имеются описания эндоскопических вмешательств на щитовидной и паращитовидных железах с использованием разнообразных доступов: подмышечного, бокового шейного и грудного. Как правило, показанием к проведению эндоскопических и видеоассистированных вмешательств на щитовидной железе служат доброкачественные образования различных размеров.In the treatment of thyroid diseases in recent years, there has been a serious competitor to conventional surgical methods - endoscopic and video-assisted interventions. Видеоассистированные вмешательства на щитовидной железе дополняют возможности классического вмешательства на железе современными приемами и инструментами из эндохирургии. В нашей стране Ю.Т.Цуканов и соавт. в 1998 г. разработали мини-доступ к щитовидной железе и выполнили 39 успешных оперативных вмешательств в объеме от гемитиреоидэктомии до двусторонней лобэктомии. Первые описания видеоассистированного оперативного вмешательства на щитовидной железе с применением ультразвуковых ножниц в нашей стране приходятся на 2001 г., когда В.Н.Егиев и соавт.(Егиев В.Н., Кянджунцев С.Р., Капчак В.М. Видеоассистированные операции на щитовидной железе. Эндоскоп хир., 2001; 6: 44-45) описали новую методику оперативного вмешательства на щитовидной железе при узловых образованиях с применением эндоскопических инструментов и ультразвуковых ножниц и продемонстрировали первый опыт таких вмешательств у 41 пациента. К 2002 г. В.Н.Егиев и соавт. описывают успешные результаты применения видеоассистированного метода с использованием ультразвуковых ножниц на примере 150 оперативных вмешательств на щитовидной железе. Анализ результатов, полученных зарубежными авторами, позволяет предполагать перспективность видеоассистированного метода и применение ультразвуковых ножниц в оперативном лечении заболеваний щитовидной железы. Миниинвазивная видеоассистированная тиреоидэктомия была описана в 1998 г., как сообщают Р.Miccoli и соавт. Авторы проводят анализ 336 операций на щитовидной железе, выполненных видеоассистированным методом в клиниках Италии, Бельгии и Германии (из отчета на XXXIX Мировом конгрессе хирургов в Брюсселе в августе 2001 г.). Операция выполнялась безгазовым способом. Центральный разрез длиной 15-20 мм располагался на 15 мм выше вырезки грудины. Операция производилась под видеоконтролем, с использованием 30° эндоскопа 5 мм и ретракторов. В ходе операции при мобилизации железы уделялось внимание визуализации верхнего гортанного и возвратного нервов и паращитовидных желез. Только после уверенности в том, что гортанные нервы и паращитовидные железы не повреждены, выполнялась мобилизация боковой доли щитовидной железы и ее удаление. Перешеек мобилизовался и отделялся от трахеи как при традиционной операции. После повторной ревизии возвратного гортанного нерва операция заканчивалась. Дренажи не использовались. Кожный разрез сопоставлялся стрипами. Из осложнений отмечены: парез возвратного гортанного нерва у 8 (2,1%) больных (7 - преходящий и 1 (0,3%) - стойкий), гипопаратиреоз у 11 (9,8%) больных на 112 выполненных тиреоидэктомий, в том числе у 9 - преходящий и у 2 (1,8%) - постоянный. В 3 наблюдениях было кровотечение, в 1 из них потребовалась конверсия. Всего отмечено 15 случаев конверсии (4,5%), причинами которой стали трудности при мобилизации железы и неуверенность в визуализации возвратного гортанного нерва. При сравнительном анализе послеоперационных осложнений отмечено меньшее количество парезов возвратного гортанного нерва (2,1%). При традиционных вмешательствах парез возвратного гортанного нерва авторы отмечают в 2,7-3,0% наблюдений. При этом не исключается, что отбор пациентов для видеоассистированной операции обусловил наличие более легких операционных ситуаций, а значит, и меньшее количество послеоперационных осложнений.The operation was performed in a gas-free manner. A comparative analysis of postoperative complications showed a smaller number of paresis of the recurrent laryngeal nerve (2.1%). With traditional interventions, paresis of the recurrent laryngeal nerve were noted in 2.7-3.0% of cases. At the same time, it is possible that the selection of patients for video-assisted surgery led to the presence of lighter surgical situations, and hence a smaller number of postoperative complications.

С учетом опыта использования видеоассистированных вмешательств на щитовидной железе в нашей клинике нами достоверно установлена возможность проведения видеоассистированного вмешательства при высокодифференцированном раке щитовидной железы, отдаленные результаты которой не только не уступают, но по ряду параметров превосходят результаты традиционных вмешательств. Помимо того, что нами разработана сама тактика видеоассистированного вмешательства, мы предлагаем надежный способ дооперационного определения показаний к проведению такого вида вмешательства. Из уровня техники известен способ дооперационного определения тактики оперативного вмешательства при подозрении на рак щитовидной железы путем осуществления тонкоигольной аспирационной биопсии (ТАБ) под контролем ультразвука с последующим гистологическим исследованием биоптата и формированием вывода о дальнейшей тактике оперативного вмешательства (А.Е.Коваленко, Современные подходы к диагностике и лечению злокачественных опухолей щитовидной железы, Здоровье Украины, 2006, №4). Указанный способ позволяет на дооперационном этапе принять решение о необходимости хирургического вмешательства и его объеме. В известном способе при гистологической верификации диагноза рака щитовидной железы, в том числе высокодифференцированного, автор считает показанным и необходимым проведение экстрафасциальной тиреоидэктомии. Техническим результатом предложенного нами способа является преодоление недостатков способа-прототипа. Так, предлагаемая нами совокупность приемов способа позволяет с высокой точностью и достоверностью определять наличие или отсутствие у пациента злокачественного заболевания щитовидной железы благодаря тому, что диагностическому исследованию (биопсии с дальнейшей гистологической верификацией) подвергается не только выявленное в результате ультразвукового исследования узловое образование размером более 10 мм, но и другие участки ткани щитовидной железы (узловые образования меньшего размера, неизмененная ткань щитовидной железы), а полученные результаты способствуют выбору такой тактики оперативного вмешательства, которая, с одной стороны, обеспечивает максимально необходимую радикальность вмешательства при выявленном виде рака ЩЖ, а с другой стороны, является наименее травматичной.The technical result of our proposed method is to overcome the disadvantages of the prototype method. Для достижения вышеизложенного технического результата нами впервые предложено в целях выбора тактики оперативного вмешательства на ЩЖ необходимой радикальности и наименьшей травматичности осуществление мультифокальной тонкоигольной аспирационной биопсии (мультифокальной тонкоигольной биопсии - МТАБ) ЩЖ из 7 точек (3 точки в правой доле ЩЖ, три в левой, 1 в перешейке; с последующим гистологическим исследованием биоптата), которая позволяет определить клинические ситуации, в которых возможно проведение такого вида операции, которая обеспечит малую травматичность, низкую частоту осложнений, косметический эффект, сокращение продолжительности стационарного лечения, сроков нетрудоспособности наряду с необходимым и достаточным соблюдением онкологических принципов. Перед решением вопроса о тактике оперативного вмешательства на щитовидной железе при подозрении на наличие рака щитовидной железы выполняют мультифокальную тонкоигольную аспирационную биопсию (МТАБ) ткани щитовидной железы (ЩЖ) под контролем ультразвука. При этом в правой и левой доле осуществляют биопсию трех участков ткани щитовидной железы, а в перешейке - одного. Полученный материал отправляют на гистологическое исследование. В том случае, если по результатам гистологического исследования биоптата диагностирован высокодифференцированный рак щитовидной железы с локализацией в одной из долей и/или перешейке, осуществляют видеоассистированное удаление доли и перешейка ЩЖ путем осуществления линейного разреза кожи длиной до 2,0-3,0 см в нижней трети шеи на уровне щитовидной железы по медиальному краю грудино-ключично-сосцевидной мышцы, рассечения кожи, подкожной жировой клетчатки, подкожной мышцы шеи, выделения медиального края кивательной мышцы, отсепаровки грудино-подъязычной и грудино-щитовидной мышц эндоскопическим диссектором с последующим их отведением и приподнятием вверх с помощью эндоскопического крючка со световодом либо ретрактора с каналом для 4 мм телескопа и каналом для отсасывания крови, после обнажения доли щитовидной железы производят механическую тракцию тканей вверх и под видеоассистированным контролем с помощью эндоскопического диссектора выделяют верхнюю щитовидную артерию, клипируют ее клипатором средне-большими клипсами, пересекают эндоскопическими ножницами, после чего также под видеоассистированным контролем с помощью 4 мм телескопа под десятикратным увеличением, передаваемым на экран монитора, с помощью эндоскопического диссектора выделяют и прослеживают на протяжении возвратный гортанный нерв, затем с помощью эндоскопического диссектора выделяют нижнюю щитовидную артерию, клипируют ее клипатором средне-большими клипсами и пересекают эндоскопическими ножницами, после чего производят удаление доли и перешейка щитовидной железы.Before deciding on the tactics of surgical intervention on the thyroid gland, in case of suspected thyroid cancer, a multifocal fine-needle aspiration biopsy (MTAB) of thyroid gland tissue (thyroid gland) is performed under ultrasound control. Наши многочисленные наблюдения показали, что при использовании МБАТ практически исключается возможность «необнаружения» злокачественных новообразований тканей ЩЖ, в связи с чем, при выявлении по результатам МБАТ высокодифференцированного рака ЩЖ в одной доле и/или перешейке, можно быть уверенным в том, что в ткани ЩЖ присутствуют какие-либо другие изменения, не позволяющие провести видеоассистированное вмешательство. В то же время само видеоассистированное вмешательство по предложенной методике является необходимым и достаточным при обнаружении у пациента высокодифференцированного рака ЩЖ. Пример 1: Пациент И., 54 лет, поступил в МНИОИ им. Герцена с подозрением на рак щитовидной железы для обследования и последующего лечения. В левой доле щитовидной железы узловое гипоэхогенное образование размером 14 мм. Под контролем ультразвука проведена тонкоигольная аспирационная биопсия (ТАБ) выявленного узлового образования. По результатам гистологического исследования биоптата поставлен диагноз высокодифференцированного рака ЩЖ. Произведена тиреоидэктомия. При наблюдении пациента в ближайшем и отдаленном послеоперационном периоде данных за прогрессирование, рецидив заболевания не получено. Однако в послеоперационном периоде были отмечены: односторонний парез гортани вследствие повреждения возвратного нерва, паратиреоидная недостаточность, неудовлетворенность пациента косметическим результатом операции.Example 1: Patient I., 54 years old, was admitted to the Moscow Research Institute of Medicine. Проведено видеоассистированное вмешательство по известной ранее методике. Интраоперационно: проведение лигатурного гемостаза из мини-доступа сопровождалось некоторыми сложностями, потребовавшими увеличения времени вмешательства по сравнению со стандартной тиреоидэктомией. Кроме того, при проведении контрольного обследования через 3 месяца после операции были обнаружены метастатические изменения лимфатических узлов шеи слева. Проведена повторная операция - удаление оставшейся доли ЩЖ и лимфаденэктомия. При гистологическом исследовании удаленного материала (левая доля, лимфатические узлы) обнаружены изменения, типичные для папиллярного рака щитовидной железы. Таким образом, первоначальное исследование (ТАБ узла правой доли) и выбранная в соответствии с его результатами тактика лечения не позволили поставить правильный диагноз и выбрать необходимый вид лечения.Example 2: Patient K., 42 years old, was sent to the Moscow Research Institute of Medicine. Herzen suspected tumor formation in the right lobe of the thyroid gland for further examination and treatment. Under the control of ultrasound, a TAB of nodular formation of the right lobe of the thyroid gland was performed, and according to the results of a histological examination of the biopsy, a highly differentiated thyroid cancer was revealed. A video-assisted intervention was performed according to a previously known technique. Intraoperative: ligature hemostasis from mini-access was accompanied by some difficulties that required an increase in the time of intervention compared to standard thyroidectomy. In addition, during a follow-up examination 3 months after the operation, metastatic changes in the lymph nodes of the neck on the left were detected. A repeated operation was performed - removal of the remaining thyroid lobe and lymphadenectomy. Histological examination of the removed material (left lobe, lymph nodes) revealed changes typical of papillary thyroid cancer. Thus, the initial study (TAB of the node of the right lobe) and the treatment tactics selected in accordance with its results did not allow to make the correct diagnosis and choose the necessary type of treatment.
Пример 3: Пациентка Г., 49 лет, при обследовании в МНИОИ им. Герцена по поводу рака правой молочной железы при ультразвуковом исследовании выявлены многочисленные гипоэхогенные образования в обеих долях ЩЖ. Для исключения синхронного рака произведена МТАБ ЩЖ: 3 участка в правой доле, 3 участка в левой доле, 1 участок в перешейке. По результатам гистологического исследования: левая доля - очаговая гиперплазия, правая доля - высокодифференцированный рак. Пациентке проведено видеоассистированное вмешательство по предложенной нами методике. Операция прошла успешно, ранний восстановительный период - без осложнений. При контрольных осмотрах через 3, 6, 12, 18 и 24 месяца признаков прогрессирования заболевания не выявлено.Example 3: Patient G., 49 years old, when examined at the Moscow Scientific Research Institute for Surgery. During follow-up examinations after 3, 6, 12, 18 and 24 months there were no signs of disease progression.
