Abstract
Description
The invention relates to medicine, namely to outpatient surgery, and for the prevention of the formation of coarse proliferation of the nail plate and the treatment of nail panaritium.
И. Панариций., Мед, 1975 г., с.60).There is a method of treating paronychia, in which linear incisions are made in the area of the inflamed nail cushion of the affected finger, wound dressing (M.I. Lytkin. Panaricius., Honey, 1975, p. 60).
А. 1986 г., с.82, с.137).There is a method of treating paronychia, in which paired linear sections are made in the area of the transition angles of the inflamed nail roller, necrectomy and wound dressing (V.A. Popov et al. Panaritsy. - L .: Med. 1986, p. 82, p. 137).
И. 241).There is a method of treating nail panaritium, which consists in making an incision in the area of the inflammatory lesion of the nail roll, necrectomy, administering drugs (antibiotics) to the base of the affected finger and wound dressing (V.I. Struchkov and other antibiotics in surgery, 1973 , p. 241).
The disadvantages of the analogs described above are that linear sections with these methods of treatment of paronychia are irrational, since the edges of linear wounds after surgery quickly close, poorly drain or regenerate before stopping the inflammatory process in the lesion, which leads to complications in the form of repeated purulent inflammation of the underlying tissues of the nail phalanx .
Прототипом изобретения является способ лечения гнойного панариция, заключающийся в том, что иссечение гнойно-некротических тканей ногтевого валика производят клиновидным разрезом, в основание пораженного пальца вводят антибиотики (пенициллин или стрептомицин в зависимости от чувствительности больного к препарату, в дозе 1000000 500000 единиц соответственно в 7-10 миллилитрах 0,25-0,5% раствора новокаина), после чего осуществляют дренирование операционной раны с помощью вакуум-отсоса, соединенного с надетым на палец герметизирующим наконечником, в качестве которого используют корпус от одноразовой системы для внутривенного введения жидкостей (Патент RU 2062607, 1996.).The prototype of the invention is a method of treating purulent panaritium, namely, the purulent necrotic tissues of the nail roll are excised by a wedge-shaped incision, antibiotics are introduced into the base of the affected finger (penicillin or streptomycin depending on the patient's sensitivity to the drug, at a dose of 1,000,000 to 500,000 units, respectively 7 -10 milliliters of a 0.25-0.5% solution of novocaine), after which the surgical wound is drained using a vacuum suction connected to a sealing tip worn on the finger , which use a housing from a disposable system for intravenous administration of fluids (Patent RU 2062607, 1996.).
A significant disadvantage of this method is that the authors performed bilateral wedge-shaped incisions, this leads to the deformation of the nail plate and its surface and its rapid wedging in the bed of the nail angle due to its rapid growth, repeated inflammatory process, repeated excision of the plate.
In addition, the use of vacuum suction is irrational in this case of treatment due to the fact that it is forced drainage, which leads to a violation of the natural, spontaneous discharge of exudate, as well as vacuum compression of the surrounding tissues, blood vessels, which entails a violation of trophic tissue and its long regeneration. The use of the fingertip also violates trophism, squeezes the surrounding tissue, and its tightness adversely affects the enrichment and contact of the wound with oxygen due to the tight closure of the entire surface of the finger with the fingertip, which results in oxygen deficiency of the wound, trauma to the wound surface due to forced vacuum suction of exudate on the wound, which leads to a duration of wound healing, insufficient regenerative and anti-inflammatory effect.
The technical result is the prevention of the formation of coarse growth of the nail plate, the prevention of rapid growth of the plate and complications in the inflammatory focus, the rapid cleansing of surgical wounds and relief of the inflammatory process in the affected finger.
The proposed method is as follows. The surgical field is treated with an alcohol solution, conduction anesthesia according to Oberst-Lukashevich. The extent of the defeat of the nail phalanx is determined. After that, a zigzag section is made from the lesion side along the entire height of the nail plate in the region of the lateral roller to the unaffected sprout zone of the nail plate. This ensures a slow, unprogrammed growth of the nail plate, prevents its gross proliferation, wedging in the nail bed and a full discharge of exudate. After opening the abscess, sanitation is carried out without drainage. After that, gauze dressings are impregnated with an antibacterial solution of Dioxidin. Dressings are carried out 2 times a day until the wound is cleaned.
The object of our study was 63 people. 43 patients of the main group were treated using the proposed method.
20 patients made up the control group, which was treated with the prototype method.
When comparing clinical results in the main and control groups, the following results were noted.
Based on the results of the application of the proposed method in the main group, high regenerating properties, the least trauma to the finger tissue due to a zigzag overlay of the cut on the nail plate and slow growth due to the absence of compression and the natural exudate flow, providing exposure to the wound of the drug and the wound in contact with the environment, were revealed, enriched with oxygen. Also, undeformed growth of the nail plate and non-wedging of it in the nail bed of the lateral roller of the finger was noted, which prevented a relapse of the inflammation.
Studies have shown that the application of the proposed method prevents the formation of a rough growth of the nail plate and its rapid growth, wedging in the nail bed, provides a complete discharge of exudate, quick cleansing of the wound and stopping the inflammatory process in the affected finger, and also prevents complications in the inflammatory focus.
The difference in healing in the main and control groups was 6 days, which indicates the appropriateness of the application of the proposed method with the aim of rapid rehabilitation and recovery
