Abstract
Description
Изобретение относится к области медицины, а именно к терапии, кардиологии, функциональной диагностике, и может быть использовано для дифференциальной диагностики изолированной систолической артериальной гипертонии (ИСАГ).The invention relates to medicine, namely to therapy, cardiology, functional diagnostics, and can be used for differential diagnosis of isolated systolic hypertension (ISAG).
- С.30-37).It is known that among patients with older arterial hypertension (AH), ISAH reaches 70% (Moiseev BC ARGUS. Arterial hypertension in people of older age groups / BC Moiseev, Zh.D. Kobalava. - M .: Medical Information Agency, 2002. - C .30-37).
(Mancia G. Methods for assessing blood pressure values in humans // Hypertension. - 1983. - Vol.5, Pt.2. - P.5-13). A known method for the diagnosis of ISAG by the level of intra-arterial blood pressure. The essence of the known method consists in the direct determination of systolic and diastolic blood pressure using an intra-arterial micromanometer (Mancia G. Methods for assessing blood pressure values in humans // Hypertension. - 1983. - Vol.5, Pt.2. - P.5-13) . The diagnosis of ISAG is established with a combination of elevated systolic blood pressure (SBP), which is equal to or greater than 140 or 160 mm RT. Art., and unchanged diastolic blood pressure (DBP), the rate of which is less than 90 mm RT. Art.
The disadvantages of this method include its invasiveness, the risk of complications (infection, bleeding, damage to the median nerve), expensive supplies, and therefore this method is rarely used in domestic clinical practice.
- 2004. - Т.3. - №3, ч.1. - С.105-120).The closest in technical essence to the proposed and adopted by us for the prototype is a method for diagnosing ISAG by measuring office (clinical) blood pressure by the auscultatory method according to N.S. Korotkov. In this case, the subject is in a sitting position and at least two measurements are performed on each arm. The recorded value is taken as the average of the last two measurements. With increased SBP (≥140 mmHg) and a normal level of DBP (<90 mmHg), a diagnosis of ISAH is established, and at the same time that SBP and DBP exceed threshold levels (BP ≥140 / 90 mmHg) establish a diagnosis of systolodiastolic hypertension (SDAG) (The second revision of the recommendations of GFCF for the prevention, diagnosis and treatment of hypertension // Cardiovascular therapy and prevention. - 2004. - T.3. - No. 3, part 1. - S.105-120) .
- С.117-127).The disadvantages of this diagnostic method include its low accuracy due to the high variability of blood pressure and the prevalence of the “white coat” phenomenon in older patients (Moiseev BC, ARGUS. Arterial hypertension in people of older age groups / BC Moiseev, Zh.D. Kobalava. - M.: Medical News Agency, 2002. - S.117-127).
This fact is confirmed by the own data of the authors of the proposed method. Of the 103 patients who were diagnosed with office blood pressure at the initial examination, 73 patients (70.9%) had a history of DBP levels in excess of 90 mm Hg. Art. Subsequently, when measuring office BP three times with an interval of at least 24 hours in 32 patients (31.1%) of these 73 patients, DBP levels equal to or greater than 90 mm Hg were also recorded. Art., which contradicted the established diagnosis of ISAG.
The negative aspects of the known method include the fact that the known method for the diagnosis of ISAG and SDAG leads to the grouping of objects with different SBP, and there is no accounting for the level of pulse pressure (AP) - the main determinant of ISAG.
Задачей заявляемого изобретения является разработка способа диагностики ИСАГ, основанного на результатах суточного мониторирования АД (СМАД).The task of the invention is to develop a method for the diagnosis of ISAG, based on the results of daily monitoring of blood pressure (BPM).
The technical result of the invention is to increase the accuracy of the differential diagnosis of isolated systolic arterial hypertension (ISAG) from systolodiastolic arterial hypertension (SDAG) with increased variability of daily blood pressure.
The technical result is achieved by the fact that a method for diagnosing isolated systolic arterial hypertension is carried out by determining blood pressure, establishing its average values and diagnosing ISAG by systolic blood pressure (SBP) equal to or greater than 140 mm Hg. Art., and diastolic blood pressure (DBP) less than 90 mm RT. Art.
Основное отличие заявляемого способа заключается в том, что диагностику ИСАГ проводят путем анализа данных, полученных при суточном мониторировании артериального давления (СМАД).The main difference of the proposed method is that the diagnosis of ISAG is carried out by analyzing the data obtained during daily monitoring of blood pressure (BPM).
Для анализа используют следующие данные: динамику пульсового артериального давления (ПД); эпизоды нормо- и гипертонии, в которых устанавливают средние значения ПД и САД;For analysis using the following data: the dynamics of the pulse blood pressure (PD); episodes of normo- and hypertension, in which average values of PD and SBP are established;
The difference of the proposed method lies in the fact that they determine the ratio of the average values of PD to the average values of the SBP in episodes of normo and hypertension, and then determine the difference between these values for episodes of normo and hypertension. For this, the ratio of the normotonia episode is subtracted from the ratio of the ratio of the episode of hypertension.
The difference of the proposed method is the determination of the difference in the average values of PD for episodes of normo- and hypertension; for this, the value of the average PD of an episode of normotonia is subtracted from the average PD of an episode of hypertension.
The difference in the method lies in the fact that the prognostic factors F 1 and F 2 are determined by the formulas:
F1=-14,388-187,565×A1+1,749×A2 F 1 = -14.388-187.565 × A 1 + 1.749 × A 2
F2=6,859-167,833×A1+1,099×A2,F 2 = 6.859-167.833 × A 1 + 1,099 × A 2 ,
гдеWhere
А1 - разность отношений среднего ПД к среднему САД при гипертонии и нормотонии Δ(срПД/срСАД);And 1 is the difference between the ratios of the average PD to the average SBP for hypertension and normotonia Δ (srPD / srSAD);
And 2 is the difference in mean PD in hypertension and normotonia (Δcrpd).
If the value of F 1 is equal to or greater than the value of F 2 , the patient is diagnosed with isolated systolic arterial hypertension (ISAG), and with a value of F 1 less than F 2 - systolodiastolic arterial hypertension (SDAG) against the background of increased variability of daily blood pressure.
Comparative analysis with the prototype showed that the proposed method differs from the known above methods and, therefore, meets the patentability criterion of "novelty."
From the analysis of patent and specialized literature, the authors found that the proposed method has features that distinguish it not only from the prototype, but also from other technical solutions in this and related fields of medicine.
The proposed method allows differential diagnosis of ISAG from STAG.
Evaluation of the individual dynamics of PD (absolute (ΔcrPD) and relative Δ (cpD / crSAD) during the ABPM and the subsequent determination of the values of F 1 and F 2 made it possible to diagnose ISAG with an accuracy of 96.9%.
The coefficients of the discriminant function (K 1 j, K 2 j) used in the calculation were established empirically.
F2=-6,859.The authors of the proposed method also set the values of the constants for: F 1 = -14.388 and F 2 = -6.859.
96,9%.Clinical observations of the authors of the proposed method indicate that the use of the proposed technical solution allows the differential diagnosis of isolated systolic hypertension (ISAG) from systolic diastolic hypertension (SDAG) with increased variability of daily blood pressure in a particular patient. Moreover, the accuracy of diagnosis of ISAG is 96.9%.
This allows us to conclude that the technical solution meets the criterion of "inventive step".
A diagnostic method for ISAG constituting the claimed invention is intended for use in medical practice. The implementation of its capabilities is confirmed by the methods and means described in the application. From the foregoing, it follows that the claimed invention meets the condition of patentability "industrial applicability".
The proposed method is as follows.
The subject is undergoing BPA according to the standard method (A. Rogoz, 24-hour blood pressure monitoring (methodological issues) / A.N. Rogoza, V.P. Nikolsky, E.V. Oshchepkova.- M .: Ay and Di, 1996. - 45 s.). Based on the data obtained, the parameters Δ (sRPD / srSAD) and ΔsrPD are calculated according to the formulas:
, ,
гдеWhere
ст.); - the ratio of the average PD to the average SBP for all blood pressure values corresponding to an episode of arterial hypertension (SBP≥140 and / or DBP≥90 mm Hg).
- the ratio of the average PD to the average SBP for all blood pressure values corresponding to the episode of normotonia (blood pressure <140/90 mm Hg).
, где where
- среднее ПД для всех значений АД, соответствующих артериальной гипертензии (САД≥140 и/или ДАД≥90 мм рт. ст.); - average PD for all blood pressure values corresponding to arterial hypertension (SBP≥140 and / or DBP≥90 mm Hg);
- average AP for all blood pressure values corresponding to normotonia (blood pressure <140/90 mm Hg).
The drawing shows an individual graph of ABPM, which shows episodes of blood pressure values with hyper- and normotonia.
Using the obtained calculated values, determine the values of F 1 and F 2 according to the formulas:
F1=-14,388-187,565×A1+1,749×А2 F 1 = -14.388-187.565 × A 1 + 1.749 × A 2
F2=-6,859-167,833×A1+1,099×А2 F 2 = -6.859-167.833 × A 1 + 1,099 × A 2
If the value of F 1 is greater than or equal to F 2 , then the patient is diagnosed with ISAH, and with a value of F 1 less than F 2 - SDAG.
The proposed method for the diagnosis of ISAG is illustrated by examples of specific performance.
Example 1. Patient S., 78 years old. According to the results of the ABPM, when analyzing the individual BP schedule, it was found that out of 78 BP measurements, 26 met the AG criteria (SBP> 140 and / or DBP≥90 mmHg) and 52 met the normotonia criteria (BP <140/90 mmHg) .). We calculate the values of the parameters Δ (srPD / srSAD) and ΔsrPD:
77,0/155,1-58,9/125,7=0,028Δ (srPD / srSAD) = 77.0 / 155.1-58.9 / 125.7 = 0.028
ΔсрПД=77,0-58,9=18,1Δavr = 77.0-58.9 = 18.1
We determine the values of F 1 and F 2 according to the formulas:
F1=-14,388-187,565×0,028+1,749×18,1=12,017F 1 = -14.388-187.565 × 0.028 + 1.749 × 18.1 = 12.017
F2=-6,859-167,833×0,028+1,099×18,1=8,334F 2 = -6.859-167.833 × 0.028 + 1.099 × 18.1 = 8.334
F 1 is greater than F 2 , therefore, patient C. is diagnosed with ISAG.
To prove the established diagnosis, office blood pressure was analyzed according to the outpatient data of this patient. In most measurements, the SBP was above 140 mm Hg. Art., DBP - less than 90 mm RT. Art., the temporary index of the GARDEN by the ABPM was 31%, the temporary index of the DBP - 10%. The average daily PD was 62 mm Hg. Art. Thus, the results of the estimates coincided.
Example 2. Patient C., 72 g. According to the results of the ABPM, when analyzing an individual blood pressure chart, it was found that out of 68 measurements, blood pressure 50 met the AH criteria (SBP≥140 and / or DBP≥90 mmHg) and 18 - criteria for normotonia (HELL <140/90 mm Hg). The values of the parameters Δ (srPD / srSAD) and ΔsrPD were:
54,6/152,8-45,1/125,4=-0,0025Δ (srPD / srSAD) = 54.6 / 152.8-45.1 / 125.4 = -0.0025
The values of F 1 and F 2 according to the formulas were:
F1=-14,388-187,565×(-0,0025)+1,749×9,5=2,696F 1 = -14.388-187.565 × (-0.0025) + 1.749 × 9.5 = 2.666
F2=-6,859-167,833×(-0,0025)+1,099×9,5=4,001F 2 = -6.859-167.833 × (-0.0025) + 1.099 × 9.5 = 4.001
F 1 is less than F 2 , therefore, in patient C., SDAG is established.
To prove the obtained assessment, office BP was analyzed according to the data of the patient's outpatient card C. In most measurements, the SBP and DBP were higher than 140 and 90 mm Hg. Art. respectively. The temporary DBP index for BPP was 68.6%, the temporary DBP index was 61.1%. The average daily PD was 54 mm Hg. Art. Consequently, the results of the estimates coincided.
51,2%.Evaluation of the accuracy of the proposed method for the diagnosis of ISAG was carried out in a group of 260 patients with hypertension aged 60 to 86 years. All patients were diagnosed with ISAG by the proposed method. The accuracy and specificity of the proposed method was compared with the generally accepted method for the diagnosis of ISAG. As a reference criterion for increased PD, the level of office PD> 60 mm Hg was used. Art. The sensitivity of the ISAG diagnostic method for office blood pressure was 62.0%, specificity 47.6%. The diagnosis of ISAG by the proposed criteria was higher, the corresponding criteria were 74.1% and 51.2%.
Thus, the proposed method is simple to implement and allows 96.9% accurate differential diagnosis of ISAH using a modern, accurate and affordable method of daily monitoring of blood pressure for practical medicine, eliminating the effect of the “white coat” effect and increased blood pressure variability. Accurate and timely diagnosis of ISAG and SDAG will make it possible to carry out an individual approach to antihypertensive treatment.
