Как известно, [Кузин М.И. Раны и раневая инфекции. - М., 1990. - С.223-297; Хирургические инфекции: Руководство / Под ред. И.А.Ерюхина, Б.Р.Гельфанда, С.А.Шляпникова. - СПб., 2003] для каждой фазы раневого процесса применяется соответствующее лекарственное средство. Так, в первой (экссудативной фазе) используются препараты, действие которых направлено не только на подавление инфекции в ране, но и для усиления оттока раневого экссудата. Во второй фазе раневого процесса применимы лекарства, защищающие и стимулирующие грануляции и способствующие эпителизации. Однако существующие многочисленные известные способы лечения гнойных ран и трофических язв зачастую не дают удовлетворительных результатов. Одной из причин низкого терапевтического эффекта является недооценка имеющегося при этом значительного общего угнетения тканевой регенерации в зоне тканевого дефекта. Поэтому в последнее время внимание исследователей сосредоточено на изучении механизма действия биорегуляторов восстановительных процессов.As you know, [Kuzin M.I. За ближайший аналог принято известное лекарственное средство - ксимедон [патент РФ №2063752 «Средство для лечения старческих, диабетических и лучевых длительно незаживающих ран»]. Ксимедон является производным пиримидинового ряда, обладает низкой токсичностью, редко вызывает побочные явления, улучшает микроциркуляцию в тканях. Ксимедон представляет собой кристаллический порошок белого цвета без запаха, горьковатого вкуса с температурой плавления 139-143°С, растворим в воде, 95%-ном спирте, хлороформе, разведенной соляной кислоте, растворе едкого натрия и изотоническом растворе хлорида натрия. Техническим результатом изобретения является усиление свойств антисептика за счет присутствия свекловичного высокоочищенного пектина, который обеспечивает в более короткие сроки очистку раны или трофической язвы благодаря бактерицидному действию. Кроме того, свекловичный пектин способствует ускорению регенерации раневой поверхности. Предлагаемый состав является абсолютно нетоксичным, что исключает побочное действие. Способ осуществляют следующим образом. Сухой свекловичный жом подвергают промывке-набуханию водой при температуре 30-60°С в течение 20-30 мин. Подготовленное сырье подвергают гидролизу-экстрагированию при температуре 74-75°С в течение 2 часов при соотношении сырья и раствора соляной кислоты с концентрацией 1,3-1,5% соответственно 1:15.The method is as follows. H+ - катионированный пектиновый экстракт обрабатывают анионообменной смолой ЭДЭ-10П в ОН- - форме. Пектин, выделенный из экстрактов, обработанных ионообменными смолами, имеет чистоту 70-72%. Для обеспечения возможности использования в медицине выполняют спиртовую очистку сухого порошка пектина, что позволяет получить пектин с чистотой 75-77% и комплексообразующей способностью 500-600 мг Pb2+/г. Готовый гель пастеризуют для освобождения случайной микрофлоры путем выдерживания в слабокипящей водяной бане в течение 20 мин и рекомендуют в дальнейшем использовать для промывания ран или для обработки ран гелем. Готовый раствор пектина представляет собой мутноватую жидкость серовато-белого цвета, различной степени вязкости (в зависимости от концентрации раствора). При хранении более суток происходит образование аморфного осадка, который легко переходит в раствор при взбалтывании. Клинический пример. Больной Г., 81 год, диагноз: посттромбофофлебитический синдром, ХВН VI ст. по СЕАР, трофическая язва левой голени. Из анамнеза: больной страдает ХВН в течение 20 лет. Трофическая язва у больного появилась около 2 лет назад. Длительное время лечился амбулаторно. Поступил в клинику на стационарное лечение. Для лечения трофической язвы (Фото 1 - вид трофической язвы левой голени до лечения) применили 20% гель пектина (Фото 2 - салфетка на язве, пропитанная 20% гелем пектина). На 2-е сутки от начала лечения отмечено выраженное очищение трофической язвы, заключающееся в очищении язвы с краев и появлении грануляций (Фото 3).Clinical example. Результаты цитологического исследования. До начала местного лечения хронической язвы голени в условиях применения 20% геля пектина мазки-отпечатки, полученные с ее поверхности и окрашенные по Романовскому-Гимзе (Микрофото 1 и 2 - цитологическая картина до лечения), обнаруживали наличие в составе отделяемого многочисленные нейтрофильные гранулоциты, часть из которых имела явные признаки дегенерации, единичные лимфоциты с ядрами, содержащими компактный, грубо-дисперсный хроматин, фибробластоподбные клетки и умеренное количество нитей и сгустков фибрина. Описанная картина соответствует активному язвенному процессу с элементами гнойного воспаления.The results of cytological studies.
