Известны медикаментозные способы лечения обожженных с острой ингаляционной травмой, основанные на приеме внутрь организма лекарственных препаратов (см., например, патент РФ № 2188633, МПК А61К 1/00, 2002). Как показали наши клинические исследования на основе лечения 35 больных с острой ингаляционной травмой, именно воздействие на указанные зоны биполярными несимметричными электрическими импульсами с амплитудой положительной части 25 В и амплитудой отрицательной части в диапазоне от 0 до 185 В при частоте импульсов 7-12 Гц обеспечивает максимальное сокращение сроков лечения. При других параметрах электрических импульсов положительный лечебный эффект был меньше. Время воздействия в сеансе на каждую зону 1,4-1,6 минут также является оптимальным для максимального сокращения сроков лечения, так как при меньшей экспозиции лечебный эффект был незначительным, а при большей - имело место только бесполезное и некомфортное воздействие на кожный покров больного без положительного лечебного эффекта. Чрескожную электронейростимуляцию проводили с использованием аппарата ЧЭНС-2М, который воздействует биполярным несимметричным электрическим импульсом. Воздействие осуществляли кожные зоны Захарьина-Геда, соответствующие трахее, бронхам, легким, биполярными несимметричными электрическими импульсами с амплитудой положительной части 25 В и амплитудой отрицательной части в диапазоне от 0 до 185 В при частоте импульсов 7-12 Гц, ток потребления аппаратом 4,5 мА, причем время воздействия в сеансе на каждую зону кожного покрова Захарьина-Геда, соответствующую трахее, бронхам, легким, составляет 1,4-1,6 минут. Общее суммарное время воздействия не превышало 5 минут. Курс лечения не превышал 10 сеансов. Нами наблюдались 35 больных, которым с первого дня поступления в стационар кроме медикаментозного лечения проводили чрескожную электронейростимуляцию зон Захарьина-Геда (трахея - бронхи -легкие). Все больные имели либо изолированную острую ингаляционную травму (ИТ), либо острую ингаляционную травму, которая сочеталась с поражением кожного покрова в размере до 10%. Этиологический фактор возникновения ожога - пламя, вдыхание дыма, продуктов горения. До лечения у всех больных отмечался учащенный пульс, поверхностное частое дыхание. Выявлено уменьшение ЖЕЛ до 1,2±0,2 л при увеличении минутного объема дыхания до 7,28±0,15 л/мин. Выявлено уменьшение МВЛ до 3,5±0,3 л/мин без существенного изменения ФЖЕЛ.Before treatment, all patients had a rapid pulse, superficial frequent breathing.
