Течение туберкулезного спондилита часто сопровождается прорывом казеозных масс в прилегающие мягкие ткани и развитием паравертебрального натечного абсцесса, образованием свищей и вторичным инфицированием патологического очага. Подобные изменения не являются патогномоничными для туберкулезного поражения позвоночника и могут иметь место при спондилитах другого происхождения. Подтверждение диагноза путем обнаружения возбудителя туберкулеза непосредственно в тканях позвоночника затруднено вследствие сложности технического доступа к пораженному отделу, опасности повреждения и инфицирования прилегающих тканей при заборе материала на лабораторное исследование, а также низкой частоты выявления микобактерий туберкулеза и длительности их бактериологического культивирования. Исследование размеров и содержимого натечного абсцесса более доступно и поэтому имеет большое значение в диагностике туберкулезного спондилита и определении тактики лечения больных с этой патологией.The course of tuberculous spondylitis is often accompanied by a breakthrough of caseous masses in adjacent soft tissues and the development of paravertebral sore abscess, the formation of fistulas and secondary infection of the pathological focus. A study of the size and contents of the sore abscess is more accessible and therefore is of great importance in the diagnosis of tuberculous spondylitis and in determining the tactics of treatment for patients with this pathology.
Известен способ пункционной биопсии позвоночника для оценки характера патологического процесса при опухолях, который осуществляется под контролем рентгеновского электронно-оптического преобразователя (В.В.Бодулин с соавт. "Аспирационная биопсия в диагностике опухолевых заболеваний позвоночника"). К недостаткам этого способа относится то, что рентгеновское изображение не позволяет дифференцировать внутренние органы, находящиеся на пути пункционной иглы, а также нарушение нормальных взаимоотношений внутренних органов из-за спаек, которые часто возникают вследствие перифокального воспаления. При больших размерах патологического очага можно установить две и более дренажных трубки, чтобы ускорить удаление содержимого из него. Это важно и для лечения, т.к. при постоянном дренаже из патологического очага снижается интоксикация продуктами распада, уменьшается сдавление внутренних органов. Использование результатов исследования содержимого патологического очага, проведенного в динамике, позволяет выбрать адекватную технологию лечения, через дренажную трубку осуществить либо санацию патологического очага, либо подготовку к радикальному хирургическому лечению. Способ осуществляют следующим образом.The method is as follows.
В перевязочном кабинете или в условиях операционной под контролем УЗИ чрезкожно проводят иглу с пункционной насадкой до патологически измененных тканей и паравертебрального натечного абсцесса. УЗИ контроль позволяет предупредить повреждение внутренних органов и топически точно подвести иглу к патологическому очагу. Далее вводят иглу в патологический очаг и осуществляют забор содержимого очага (натечного абсцесса) для комплексного лабораторного исследования. Затем через иглу в патологический очаг устанавливают проводник, иглу извлекают, а по проводнику вводят бужи для увеличения диаметра входного отверстия и устанавливают в очаг дренажную трубку (ирригатор) необходимого диаметра. Трубку фиксируют к коже на все время наблюдения и проведения необходимых манипуляций. Через дренажную трубку проводят введение различных лекарственных препаратов в зону патологического очага. Пример. Больной Т., 27 лет, и/б №584, с 20 мая 2004 г. находится в отделении внелегочного туберкулеза УНИИФ с диагнозом: "Туберкулезный спондилит поясничного отдела позвоночника, осложненный натечным абсцессом". На УЗИ - паравертебрально расположенное образование с толстыми стенками, заполненное густым жидким содержимым (Фиг.1 - паравертебральный натечный абцесс при УЗИ). Под контролем УЗИ произведена пункция этого образования (Фиг.2 - в полость натечного абцесса под контролем УЗИ введена игла), получен сероватого цвета жидкий гной, который был отправлен на исследование (микроскопия мазка, посев на микрофлору, ПЦР и ИФА диагностика). Затем в иглу введен проводник, по которому осуществлено бужирование хода, а затем установлена полихлорвиниловая дренажная трубка (Фиг.3 - после бужирования в полость натечного абсцесса установлена дренажная трубка). На следующий день произведено контрастное рентгенологическое исследование (Фиг.4 - контрастное рентгенологическое исследование на следующие сутки после дренирования натечного абсцесса). На рентгенограмме - натечный абсцесс, расположенный в паравертебральной области справа. В последующие дни проводилось лечение путем активного дренирования содержимого натечного абсцесса и введения в полость через ирригатор антисептиков. Через сутки выделяемый материал взят на повторные исследования. Обнаружение МБТ методом ПЦР через 2 дня позволило подтвердить туберкулезную этиологию спондилита. После получения результатов комплексного обследования начат курс противотуберкулезной химиотерапии. В последующем у больного отмечено улучшение самочувствия, уменьшение симптомов интоксикации. Результаты рентгенологического исследования в динамике (абсцессография) показали уменьшение полости натечного абсцесса (Фиг.5 - контрастное рентгенологическое исследование через 2 недели после дренирования натечного абсцесса). Результаты лабораторного исследования зарегистрировали положительную динамику в течение патологического процесса.Example. Then, a conductor was inserted into the needle, along which the course was poured, and then a polyvinyl vinyl drainage tube was installed (Fig. 3 - after boughening, a drainage tube was installed in the cavity of the abscess abscess). The next day produced a contrast x-ray study (Figure 4 - contrast x-ray examination the next day after drainage of the sore abscess). On the roentgenogram - a sap abscess located in the paravertebral region on the right. In the following days, treatment was carried out by actively draining the contents of the sore abscess and introducing antiseptics into the cavity through an irrigator. A day later, the allocated material was taken for repeated studies. The detection of MBT by PCR after 2 days confirmed the tuberculous etiology of spondylitis. After obtaining the results of a comprehensive examination, a course of antituberculous chemotherapy was started. Subsequently, the patient noted improvement in well-being, a decrease in symptoms of intoxication. The results of an X-ray examination in dynamics (abscessography) showed a decrease in the cavity of the sap abscess (Figure 5 is a contrast x-ray study 2 weeks after drainage of the sap abscess). The results of a laboratory study recorded a positive trend during the pathological process.
