Известны способы лечения наружных брюшных грыж с применением дополнительных материалов (Коваленко И.Б., 1997; Левчик Е.Ю., 1997; Галимов О.В., 1999; Корнилаев П.Г., 1999; Kranich H., 1990). Однако применение различных пластических материалов (ауто-, алло-, ксенотрансплантатов, эксплантатов) не решило всех проблем герниологии. Одной из ведущих причин плохих исходов герниопластики являются атрофически-дегенеративные и рубцовые изменения тканей передней брюшной стенки в зоне вмешательства, снижение их прочностных свойств и регенераторных возможностей, поэтому одним из перспективных направлений в лечении грыж является воздействие на замедленную регенерацию тканей пациента.Known methods of treating external abdominal hernias using additional materials (Kovalenko IB, 1997; Levchik E.Yu., 1997; Galimov OV, 1999; Kornilayev P.G., 1999; Kranich H., 1990). Последний заключается в выкраивании кожного лоскута в области операционного доступа, иссечении с него подкожной клетчатки, нанесении перфорационных отверстий; затем производят обычное грыжесечение с фиксацией приготовленного лоскута над апоневрозом дермальной поверхностью к апоневрозу, обкалывание последнего раствором стимулятора регенерации "Аллопланта". Эмбриональные фибробласты заготавливались в отделении иммунотерапии и клеточной трансплантации курской областной клинической больницы, получали их из эмбрионов самок крыс серии "Вистар" на сроке беременности 7-9 недель, обрабатывая их 0,25% раствором трипсина с последующим двукратным отмыванием в растворе Хенкса. Культивирование проводили в культуральных флаконах на полной среде RPM 1640 с добавлением 10% эмбриональной телячьей сыворотки до формирования монослоя фибробластов. 72 крысам серии "Вистар" были проведены срединные лапаротомии в мезогипогастральной области, мышечно-апоневротический слой ушивали "край в край" или дубликатурой по Сапежко (контрольные группы А и Б), имплантация эмбриональных фибробластов производилась тот час (группы В и Г) или по микроирригатору (группы Д и Е) 1 млн. клеток в 5 мл физиологического р-ра. Мышечно-апоневротический слой ушивался "край в край" в группах В и Д, дубликатурой по Сапежко - Г и Е. Выведение животных из эксперимента осуществляли в четыре приема на 14-28-38-52-е сутки после операции (по 6 особей в контрольных группах А и Б, 12 - экспериментальных). Забор материала проводили путем иссечения зоны вмешательства вместе с окружающими тканями, затем изготовлялись гистологические препараты. Срезы окрашивались гематоксилином и эозином. После чего проводилось их микроскопическое и морфометрическое исследование. Учитывались следующие признаки: клеточная плотность экссудативной реакции (в клетках на кв. мм), клеточный состав экссудата в процентах, активность воспалительной реакции по процентному содержанию сегментоядерных нейтрофилов в экссудате, абсолютное количество лимфоцитов в инфильтрате, пролиферативная активность фибробластов по процентному соотношению фибробластов и фиброцитов.Embryonic fibroblasts were harvested in the immunotherapy and cell transplantation department of the Kursk Regional Clinical Hospital, received them from embryos of female Wistar rats at a gestational age of 7–9 weeks, treating them with 0.25% trypsin solution followed by washing twice in Hanks solution. The animals were removed from the experiment in four doses on the 14-28-38-52nd day after the operation (6 animals each control groups A and B, 12 - experimental). Material sampling was performed by excision of the intervention zone together with surrounding tissues, then histological preparations were made. Sections were stained with hematoxylin and eosin. Then their microscopic and morphometric research was carried out. The following signs were taken into account: the cellular density of the exudative reaction (in cells per square mm), the cellular composition of the exudate in percent, the activity of the inflammatory reaction by the percentage of segmented neutrophils in the exudate, the absolute number of lymphocytes in the infiltrate, the proliferative activity of fibroblasts by the percentage of fibroblasts and fibroblasts.
