Лечение гнойно-воспалительных заболеваний и их осложнений (пострубцовые деформации кожи лица, длительные лимфостазы в области раннее пролеченных областей лица, стойкие контрактуры жевательной группы мышц лица и т.д.) у больных с флегмонами и абсцессами челюстно-лицевой области - очень важная проблема челюстно-лицевой хирургии. Для этого в способе лечения больных с гнойно-некротическими заболеваниями челюстно-лицевой области, включающем хирургическое лечение с введением в гнойно-некротический очаг биоактивного материала, лазерное облучение раны, предложено осуществлять наружный доступ к фокусу гнойно-некротического очага, например, разрезом кожи 4-5 см в закрытой зоне по нижнему полюсу гнойно-некротического очага, и размещать в полости раны салфетку “Колетекс Д” или салфетку “Колетекс М” в форме полого элемента, внутри которого укладывать сквозной двухпросветный дренаж. Комплексная терапия проводится всем больным со второго дня после обязательного хирургического лечения: вскрытие гнойно-некротического очага доступом к фокусу и дренирование раны полиамидными трубками на всей глубине клетчаточного пространства с предварительной обработкой раны растворами антисептика. В ране размещают салфетку “Колетекс М” (при более выраженном гнойном процессе размещают салфетку “КолетексД”) с иммобилизированным на текстильную основу метронидозолом или димексидом (соответственно “Колетекс М”, “Колетекс Д”) в форме полого элемента, закрученного вокруг двухпросветной дренажной трубки для улучшения оттока и пролонгированного лекарственного воздействия в глубоких отделах раны. Края раны формируют подшиванием салфетки между верхним и нижним полюсом, сохраняя линейную плоскость. Лазерную терапию проводят по терапевтической схеме: после операции воздействие ИК-лазером 1-2 минуты по 10 мВт на область раны и по 4-6 минут на глубину раны через дренажные трубки с учетом размеров раны, а также транскутанно лазерное облучение крови на сонные артерии по 2-3 минуты ежедневно до полного очищения раны. Laser therapy is carried out according to the therapeutic scheme: after the operation, the exposure to infrared laser for 1-2 minutes of 10 mW per wound and 4-6 minutes to the depth of the wound through the drainage tubes, taking into account the size of the wound, as well as transcutaneous laser irradiation of blood to the carotid arteries 2-3 minutes daily until the wound is completely cleansed. These actions lead to an increase in fibrinolytic activity, accelerate the processes of lysis of necrotic masses and improve microcirculation in the area of the pathological process.

Больной В, 1991 г.р. поступил в челюстно-лицевую хирургию по экстренным показаниям с диагнозом: Карбункул левой щечной области. Тромбоз угловой вены лица слева. При поступлении общее состояние ребенка тяжелое. Со стороны челюстно-лицевой области отмечались жалобы на боль и припухлость в щечной и подглазничной областях слева, инфицированная рана щечной области с гнойно-некротическим отделяемым, ограничение открывания рта до 1,5 см, затрудненный прием пищи. При осмотре - больной заторможен, выраженная бледность кожных покровов с синюшным оттенком, адинамичен. Отмечается асимметрия средней зоны лица за счет отека и инфильтрации мягких тканей щечной, подглазничной областей с переходом на нижнее веко слева. Глазная щель сужена, зрение не нарушено. Над инфильтратом кожа истончена, гиперемирована, напряжена, в складку не собирается, пальпация вызывает резкую боль, в центре инфильтрата в щечной области имеется инфицированная рана 2,5×4,5 см с грязно-серым налетом и зияющими краями. К медиальному углу глаза прослеживается плотный болезненный тяж, ограничивающий открывание рта. При бимануальной пальпации область инфильтрации распространяется на всю толщу щеки и верхней губы слева, на слизистой оболочке по линии смыкания зубов имеются послеоперационный разрез до 2 см с отечными, покрытыми фибрином краями. По экстренным показаниям после обследования была произведена ревизия раны, некрэктомия в глубоких отделах раны, взяты биоптат и посев из раны на флору и чувствительность к антибиотикам. В полость раны уложена салфетка “ Колетекс Д” в форме полого элемента, внутри которого размещена полиамидная трубка. К краям раны для сохранения линейной плоскости и предотвращения их заворачивания лекарственной основой фиксирована салфетка “Колетекс Д”. Начато облучение каротидных синусов справа и слева ИК-лазером, а также через дренаж и на края раны фотофорез по 3 минуты с 3-х полей, ежедневно. На третьи сутки после начала лечения отмечалось снижение интенсивности боли, улучшился аппетит, ребенок стал активнее. Перевязки производились ежедневно с применением растворов антисептиков (0,5-1% диоксидина, 3% перекиси водорода вместе с фурациллином и т.д.). Верифицирован золотистый стафилококк КОЕ свыше 10 миллионов и начата массивная антибактериальная терапия с учетом чувствительности флоры, а также симптоматическая терапия, включающая общеукрепляющее, иммуностимулирующее, тромболитическое воздействие. На 5 сутки рана щечной области очищается с появлением демаркационной линии по краям и развитием сочных кровоточащих грануляций по дну раны. Удалены дренажи. На область тяжа и раны аппликационно уложены салфетки “Колетекс МЕКС” (с мексидолом), фотофорез ИК-лазером по 2 минуты в течение 7 дней. Полученный результат микрофлоры указывал на значительное снижение титра микробов до 1-2 миллионов, а в биоптатах - картина выраженной обсемененности раны, некроз на всем протяжении ее слоев. Отмечалась значительная положительная динамика на 10 сутки, когда размеры раны уменьшились до 1,5-2 см, эпителизация ее со дна и с краев сочными розовыми грануляциями, тяж по ходу ветви лицевой вены едва пальпируется, открывание рта до 2 см. Учитывая хорошие параметры заживления и рубцевания раны наложение вторичных швов не показано. Ведение раны продолжено до 10 процедур. При последних взятиях мазков на флору - роста ее нет. Лечение закончено и на 12 сутки больной был выписан на амбулаторное наблюдение.Patient B, 1991 was admitted to the maxillofacial surgery for emergency reasons with a diagnosis of Carbuncle of the left buccal region. There is an asymmetry of the middle zone of the face due to swelling and infiltration of the soft tissues of the buccal, infraorbital regions with a transition to the lower eyelid on the left. The palpebral fissure is narrowed, vision is not impaired. Above the infiltrate, the skin is thinned, hyperemic, tense, cannot be folded, palpation causes sharp pain, in the center of the infiltrate in the buccal region there is an infected wound 2.5 × 4.5 cm with a dirty gray coating and gaping edges. A dense painful cord tracing the opening of the mouth is traced to the medial corner of the eye. With bimanual palpation, the area of infiltration extends to the entire thickness of the cheek and upper lip on the left, on the mucous membrane along the closing line of the teeth there is a postoperative incision up to 2 cm with edematous, fibrin-coated edges. According to emergency indications after the examination, the wound was revised, necrectomy in the deep sections of the wound, biopsy and culture from the wound were taken for flora and sensitivity to antibiotics. A Coletex D napkin is laid in the wound cavity in the form of a hollow element, inside which a polyamide tube is placed. To the edges of the wound to preserve the linear plane and prevent them from wrapping the drug base fixed towel "Coletex D". Irradiation of the carotid sinuses on the right and left with an IR laser, as well as through drainage and on the edges of the wound, photophoresis for 3 minutes from 3 fields daily. On the third day after the start of treatment, a decrease in pain intensity was noted, appetite improved, the child became more active. On the 5th day, the wound of the buccal region is cleaned with the appearance of a demarcation line along the edges and the development of juicy bleeding granulations along the bottom of the wound. Drains removed. Coletex MEX wipes (with Mexidol), photophoresis with an IR laser for 2 minutes for 7 days were laid on the area of the cords and wounds. The result of microflora indicated a significant decrease in the titer of microbes to 1-2 million, and in biopsy specimens there was a picture of pronounced contamination of the wound, necrosis throughout its layers. Significant positive dynamics was noted on the 10th day, when the size of the wound decreased to 1.5-2 cm, epithelialization from the bottom and edges with juicy pink granulations, the strand along the branch of the facial vein is barely palpated, opening the mouth up to 2 cm. Given good healing parameters and wound scarring secondary suturing is not shown. Wound management continued up to 10 procedures. With the last smears taken on the flora - its growth is not. The treatment is completed and on the 12th day the patient was discharged for outpatient monitoring.

Больной Б., 1939 г.р. находился в челюстно-лицевой хирургии с диагнозом: одонтогенная флегмона дна полости рта и глубоких пространств шеи справа. В день поступления по экстренным показаниям произведено вскрытие и дренирование флегмоны под эндотрахеальным наркозом. Учитывая тяжесть состояния, больной был доставлен в реанимационное отделение, где продолжалась искусственная вентиляция легких, массивная антибактериальная и детоксикационная терапия. С положительной динамикой был переведен в отделение челюстно-лицевой хирургии на 2 сутки после операции со стабильной гемодинамикой, в сознании, на самостоятельном дыхании. Продолжена консервативная терапия, фотофорез салфетки Колетекс Д (димексида) на раны в подподбородочной области и шеи справа в течение 2-х дней, затем Колетекс М (метронидозола) до 5 дней по описанной выше методике. Отмечалось уменьшение боли в области ран и при глотании, улучшения самочувствия и аппетита, ускорение очищения с помощью аппликаций салфетки “Колетекс МЕКС” и эпителизации раны с формированием гладкого, нежного рубца, не требующего коррекции.Patient B., born in 1939 was in maxillofacial surgery with a diagnosis of odontogenic phlegmon of the bottom of the oral cavity and deep spaces of the neck on the right.
