Abstract
Description
The invention relates to medicine, in particular to neurosurgery and traumatology, and is intended for use during surgery to stop bleeding from intraosseous blood vessels, mainly the cranial vault.
With resectional trepanation of the skull from damaged large blood vessels located in the spongy layer of the bone, as well as with a significant increase in blood pressure, as a rule, significant bleeding occurs, which requires an urgent stop. Existing drugs do not always provide reliable hemostasis during and after surgery, which prompted us to search and develop a more effective method of stopping bleeding from intraosseous blood vessels.
115-119).There is a method of mechanically stopping bleeding by clamping a blood vessel with a hemostatic clamp (YF Kabatov. Medical instruments, apparatus and equipment. - M .: Medicine, 1977, p.115-119).
The disadvantage of this method is the inability to use it during surgery when bleeding from the intraosseous vessels of the skull, since it is impossible to apply a clamp to the intraosseous blood vessels due to the hardness of the bone. It lacks techniques that increase the reliability of hemostasis during bleeding from the intraosseous vessels of the skull with the exception of brain injury and a reduction in complications by more reliable blockage of bleeding channels in the bone.
В.М. 277).It is known to use liquid hemostatic agent for craniotomy to stop bleeding from intraosseous blood vessels by injecting it with a syringe into a bone groove where there is bleeding (Surgery of the central nervous system. / Ed. By Prof. V.M. Ugryumova. - Publishing house “Medicine”, Len.dep., 1969, p.277).
The disadvantage of this method is that when it is used, hemostasis can be achieved as a result of the formation of a blood clot in a bleeding vessel under the influence of a locally administered liquid hemostatic agent. However, from a large blood vessel, as well as with high blood pressure, a formed blood clot may escape from the lumen of the vessel with the resumption of bleeding and the development of complications. In this method, there are no techniques that ensure high reliability of hemostasis during surgery when bleeding from the intraosseous blood vessel of the cranial vault, with the exception of brain injury and the reduction of complications by more reliable mechanical blockage of bleeding channels in the bone.
М. 212).A known method of hemostasis during bleeding from the intraosseous blood vessels of the skull during surgery by rubbing wax into bone tissue (I. M. Erger. Neurosurgery. - M .: Medicine, 1982, p. 212).
The disadvantages of this method include the fact that when bleeding from a large vessel or at high blood pressure, wax can be ejected from the blood vessel and from the bone with the resumption of bleeding and additional loss of precious blood to patients. In addition, this method does not exclude the possibility of damage to the gloves and fingers of the surgeon on the sharp edges of the bone.
The closest in technical essence and the achieved result is the last method of hemostasis using wax, the disadvantages of which are described above and which we take as a prototype.
The technical result consists in increasing the reliability of hemostasis during surgery during bleeding from the intraosseous vessels of the cranial vault, with the exception of brain injury and the reduction of complications by more reliable mechanical blockage of bleeding channels in the bone with a strong sealing element.
The technical result is achieved in that a method for stopping bleeding from a skull bone includes the introduction of a sealing element into a bleeding area of the bone. The sealing element is formed as a cork from an autobone resected during surgery with dimensions not exceeding the thickness of the spongy layer of the bleeding area of the skull bone. At the same time, the sealing element is captured between the jaws of the holding tool, placed against the source of bleeding and the pusher between the jaws is inserted with force until it is completely immersed in the spongy layer of the bone.
In a preferred embodiment of the method for stopping bleeding from the skull bone, the sealing element is formed wedge-shaped from the cortical layer of the bone with a portion of the spongy substance on it and introduced into the bleeding bone with a narrowed end.
In another preferred embodiment of the method, the jaws seize the sealing element along the length of its cortical part, which is introduced transversely to the planes of the bony layers of the skull.
A specific example of the method. During resection trepanation of the cranial vault, when the bone window is formed in it, autobone fragments are harvested for use with hemostasis. In the event of bleeding from a large intraosseous blood vessel or during intensive bleeding as a result of a significant increase in blood pressure, a free piece is selected from the autobone of the skull resected during the operation, and it is formed as a tube with dimensions not exceeding the thickness of the spongy layer of the bleeding area of the skull bone. Moreover, the sealing element is taken wedge-shaped from the cortical layer of the bone with a portion of the spongy substance on it and introduced into the bleeding bone with a narrowed end. At the same time, a sealing element is seized between the jaws of the surgical instrument along the length of its cortical part. The jaws bring the sealing element to the bleeding area of the bone, attach it to the source of bleeding with a narrowed end and its cortical part transverse to the planes of the bone layers of the skull. Then, the pusher between the jaws of the surgical instrument forces the sealing element into the bleeding area until it is completely immersed in the spongy layer of the bone and the bleeding stops.
The significance of the differences of the invented method is as follows. The implementation of the sealing element from the autobone resected during the operation excludes the use of foreign material undesirable for the patient’s body. The sealing element is obtained during trepanation, which does not require an unnecessary expenditure of time on the search for material and guarantees its presence during the operation. The formation of the sealing element as a rigid tube provides the possibility of deep penetration into the bone tissue and reliable retention there for a time sufficient to finally stop bleeding from the intraosseous blood vessel. The manufacture of a sealing element with dimensions not exceeding the thickness of the spongy layer of the bleeding section of the skull bone allows without additional trauma to the cortical and vitreous layer of the skull bone to easily insert a rigid sealing element into the spongy layer of the bleeding section of the skull bone. The formation of the sealing element by a wedge-shaped from the cortical bone layer with a portion of the spongy substance on it facilitates the introduction of its narrowed end and provides better adhesion to the spongy layer of the bleeding portion of the bone, which improves the quality of retention of the autograft and the reliability of the sealing of a bleeding intraosseous blood vessel. The rigidity of the sealing element allows you to easily grab and hold it securely with a surgical instrument, more accurately attach it to the source of bleeding under visual inspection and with effort, without touching it with your hands, push it until it is completely immersed in the spongy layer of the bone without additional tissue injury in the surgical wound and excluding damage to the surgeon's hands. In another preferred embodiment of the method, the gripping of the sealing element by the jaws along the length of its cortical part increases the reliability of its holding by the jaws and does not injure the spongy part of the graft. The introduction of it transversely to the planes of the bony layers of the skull provides a more dense implantation and retention between the bony layers of the skull and greater contact of the spongy part with the spongy layer of the skull bone, which increases the likelihood of clogging of a bleeding intraosseous blood vessel.
Thus, the claimed method in the aggregate of its features improves the result of surgical treatment and reduces the risk of complications by providing during the operation a more reliable stop of bleeding from a blood vessel in the bone tissue of the skull.
The application of the invention is possible in pediatric and field surgery.
