Способ относится к области медицины и может быть использован для местного лечения больных сахарным диабетом с гнойно-некротическими осложнениями на стопе и высокой резистентностью микроорганизмов к антибиотикам. The method relates to medicine and can be used for local treatment of patients with diabetes mellitus with purulent-necrotic complications on the foot and high resistance of microorganisms to antibiotics.
Известен способ повышения эффективности местного лечения больных СД с гнойно-некротическисми процессами на стопе при полирезистентной микрофлоре с использованием для местного применения озонированных антисептиков или дистиллированной воды. (см. М.Л. Пупышев. Применение локальной озонотерапии в лечении больных с деструктивными формами диабетической стопы. Тезисы докладов; "Актуальные вопросы гнойных осложнений и заболеваний в хирургической практике"; 7-8 октября 1999 года; стр. 215-216). Недостатком известного способа является необходимость специального оборудования, высокая токсичность озона, отсутствие эффекта удержания в ране кислой среды, быстрое окисление контактирующих с озоном лекарственных препаратов. Недостатком известного способа, взятого в качестве прототипа, является необходимость поддержания высокой концентрации препарата в очаге поражения, окисление контактирующих с гипохлоритом лекарств, необходимость специального оборудования для производства гипохлорита натрия, быстрая инактивация активного кислорода, отсутствие эффекта удержания в ране кислой среды.There is a method of increasing the effectiveness of local treatment of patients with diabetes mellitus with purulent-necrotic processes on the foot with multiresistant microflora using applications with gauze swabs moistened with 0.1% sodium hypochlorite solution. Сущность заявляемого изобретения заключается в том, что согласно способа лечения деструктивных форм диабетической стопы, заключающегося в применении препарата, снижающего резистентность микробов к антибиотикам, в качестве препарата используют 1% раствор соляной кислоты, наносимой в виде влажной повязки на предварительно покрытую антибиотиком раневую поверхность, перевязки повторяют ежедневно до появления свежих грануляций. Простота, а также экономическая привлекательность способа позволяет широко использовать его в практике, следовательно, улучшить качество жизни больных осложненными формами сахарного диабета. Способ осуществляется следующим образом. После хирургической обработки гнойно-некротического очага, на подсушенную раневую поверхность наносится в виде порошка один из антибиотиков широкого спектра действия. Поверх антибиотика на рану накладывают влажно-марлевую повязку, смоченную 1% раствором соляной кислоты. Перевязки выполняются ежедневно до появления свежих грануляции и полного очищения раны от гноя. Больной М. , 51 год. Диагноз: Флегмона стопы. Синдром диабетической стопы, нейропатическая форма. Сахарный диабет 2 типа, тяжелое течение. Хроническая венозная недостаточность нижних конечностей II степени. Стопа отечна, гиперемирована, обширная гнойная рана (7•4 см). Показатели крови: мочевина (9,8 ммоль/л); лейкоциты (5,6•109/л); СОЭ (50 мм/ч). На фоне дезинтоксикационной, антибактериальной терапии, после хирургической обработки раны и вскрытия затеков, выполнялись ежедневные перевязки с амикацином и соляной кислотой. Результаты посевов из раны: 1й день - высеялись Proteus, Str. hemoliticus. Отсутствие чувствительности к гентамицину, эритромицину, карбеницилину, ципрофлоксацину, офлоксацину, доксициклину, фурагину, полимиксину, цефобиду. Умеренная чувствительность к амикацину. На 5-й - высевается Proteus, чувствительный ко всем вышеперечисленным антибиотикам за исключением фурагина и эритромицина.Patient M., 51 years old. Diagnosis: Phlegmon of the foot. Больная Б. , 36 лет, II группа инвалидности. Диагноз: Обширная инфицированная язва пяточной области правой нижней конечности. Синдром диабетической стопы, нейропатическая форма. Сахарный диабет II тип, тяжелое, лабильное течение. Хроническая венозная недостаточность нижних конечностей 2 степени. Правая нижняя конечность отечна, гиперемирована до средней трети голени, паховый лимфаденит. В области голеностопного сустава выраженный отек, ближе к медиальному краю стопы инфицированная язва 3,5•4 см. Дно язвы покрыто грязно-серыми некротическими тканями. Показатели крови: мочевина (8,4 ммоль/л); лейкоциты (6,6•109/л); СОЭ (45 мм/ч). На фоне дезинтоксикационной, антибактериальной и физиотерапии, после хирургической обработки раны и вскрытия затеков, выполнялись ежедневные перевязка с цефобидом и соляной кислотой. Результаты посевов из раны: 1-й день - высеялись Pseudomonas aeruginosa, Staph. aureus. Отсутствие чувствительности к гентамицину, доксициклину, карбеницилину, офлоксацину, ципрофлоксацин, фурагину, полимиксину, клафорану. Умеренная чувствительность к цефобиду и амикацину. На 5-й - высевается Pseudomonas aeruginosa, чувствительная к цефобиду, офлоксацину, ципрофлоксацину, амикацину. Умеренная чувствительность к полимиксину и карбеницилину.Patient B., 36 years old, group II disability. Diagnosis: Extensive infected ulcer of the calcaneal region of the right lower limb.
