Предлагаемое изобретение относится к медицине, в частности к пластической хирургии, и может быть использовано при лечении ожоговых ран. The present invention relates to medicine, in particular to plastic surgery, and can be used in the treatment of burn wounds.
Известен способ оперативного лечения ожоговых ран, заключающийся в их свободной кожной пластике по методу "почтовых марок" (см. Арьев Т.Я. Ожоги и отморожения - Издательство "Медицина", Л., 1971, - с. 136-137). В качестве прототипа нами выбран способ лечения ожоговых ран, заключающийся во взятии аутотрансплантата с донорского места и закрытии им пораженного участка кожи (см. Соколов Л.Н. Ранняя свободная кожная пластика при глубоких ожогах // Автореферат диссер. на соиск. уч. степени кандидата медицинских наук, Горький, 1964, с. 10). Задачей предлагаемого изобретения является предупреждение инфицирования донорских ран и сокращение сроков восстановления утраченного кожного покрова за счет улучшения условий для приживления аутотрансплантатов. Поставленная задача решается за счет того, что в способе, предусматривающем взятие аутотрансплантата с донорского места и закрытие им пораженного участка кожи, предварительно на донорское место наносят эмульсию 30% раствора димексида и 10% масляного раствора токоферола ацетата в соотношении 5:1. Способ оперативного лечения ожоговых ран осуществляют следующим образом: за 15 минут до начала операции свободной кожной аутопластики на заранее выбранное донорское место наносят эмульсию, состоящую из 30% раствора димексида и 10% масляного раствора токоферола ацетата в соотношении 5:1. Непосредственно перед взятием аутодермотрансплантатов донорские участки обрабатывают спиртом и стерильным растительным маслом для лучшего скольжения дерматома. Взятые кожные лоскуты толщиной 0,3-0,5 мм распластывают на гранулирующие или иссеченные ожоговые раны. После этого накладывают сухую асептическую повязку. Первую перевязку ран после выполненной аутодермопластики делают на 3-4 день после операции. Повязку, наложенную на донорские раны, оставляют до полного самостоятельного восстановления кожного покрова. Больная В. , 6 лет, И/б N 167776, поступила в Нижегородский НИИ травматологии и ортопедии по поводу ожога кипятком IIIА-IIIБ ст. живота, промежности, нижних конечностей на площади 30% поверхности тела. Для восстановления утраченного кожного покрова выполнена свободная кожная пластика на гранулирующие раны с предварительной обработкой донорских мест эмульсией 30% раствора димексида и 10% масляного раствора токоферола ацетата (витамина E) по вышеописанной методике. Первая перевязка выполнена через 3 дня после операции. Донорские раны зажили под повязкой в течение 8 дней без осложнений. Больная выписана на амбулаторное лечение с полным восстановлением кожного покрова. Рубцовые деформации и контрактуры отсутствовали, движения в суставах были в полном объеме.Clinical example

Раствор димексида служит хорошим проводником для масляного раствора токоферола ацетата, насыщая им кожные покровы донорского участка. Использование димексида предотвращает инфицирование трансплантатов и донорских ран. Способ сокращает сроки восстановления утраченного кожного покрова на 2-3 дня, что особенно важно при обширных поражениях и дефиците донорских ресурсов кожи.
