Известны способы лечения тяжелых сгибательно-разгибательных контрактур кисти после ожога путем свободной кожной пластики расщепленными толстыми трансплантатами (см. например: Бирюков О.М. "Принципы восстановительно-реконструктивных операций при ожогах кисти", ж. "Вестник хирургии им. Грекова", 1970, N 7, с. 80-84; Андрусон М.В. "Свободная кожная пластика при оперативном устранении рубцовых деформаций кисти", ж. "Ортопедия, травматология и протезирование", 1971. N 4, с. 1-6). Способ осуществляется следующим образом: иссекают рубцы, выкраивают кожный лоскут с подкожной клетчаткой на питающей ножке, создают искусственную синдактилию, закрывают кисть и пальцы кожным лоскутом, отсекают питающую ножку после приживления лоскута и поэтапно разделяют сшитые между собой пальцы. Однако известный способ имеет ряд существенных недостатков. Лоскут, пересаженный на тыльную поверхность кисти и пальцев с брюшной стенки, денервирован, по своей структуре и свойствам отличается от кожных покровов тыльной поверхности кисти, возвышается над поверхностью окружающей кожи, в связи с чем требуются повторные оперативные вмешательства с целью удаления избыточной жировой клетчатки. Кроме того, для разделения пальцев после создания искусственной синдактилии также требуются дополнительные оперативные вмешательства, причем, разделение двух смежных пальцев нецелесообразно, т.к. это приводит к нарушению циркуляции. Таким образом, известный способ выполняется, как минимум, за 4-5 этапов, что увеличивает сроки лечения, а косметические и функциональные результаты не всегда удовлетворяют больных. Задачей предлагаемого изобретения является восстановление полноценного кожного покрова тыльной поверхности кисти и пальцев за счет неизмененных местных тканей. The task of the invention is to restore the full skin of the back surface of the hand and fingers due to unchanged local tissues.
Поставленная задача решается за счет того, что в способе, включающем иссечение рубцового массива тыла кисти и пальцев и закрытие раневого дефекта, растягивают неизмененные кожные покровы тыльной поверхности кисти и формируют из растянутой кожи отдельные лоскуты для каждого пальца. Способ осуществляют следующим образом: растягивают ткани в области неизмененного участка кожи тыльной поверхности кисти с помощью экспандера. Иссекают рубцы на тыльной поверхности пястно-фаланговых и проксимальных межфаланговых суставов, основных и средних фаланг II-V пальцев. Удаляют экспандер, мобилизуют растянутый лоскут до дистального края раневого дефекта, рассекают его на 4 части соответственно раневым дефектам тыльной поверхности II-V пальцев и закрывают этими лоскутами раневые дефекты отдельно каждого пальца. N 176335, у которого в результате ожога кипятком IIIБ степени правой кисти на площади 0,5% поверхности тела развилась рубцовая сгибательно-разгибательная контрактура кисти с повреждением глубоких анатомических структур (костей, суставов, сухожилий). Гипертрофические рубцы располагались на тыльной поверхности пястно-фаланговых и проксимальных межфаланговых суставов, основных и средних фаланг II-V пальцев и подвергались периодическому изъязвлению. На тыльной поверхности кисти кожные покровы были не изменены. 1/X-97 года под кожно-жировой слой по фасции тыльной поверхности правой кисти был внедрен экспандер. Наполнение его началось через 1 неделю и продолжалось в течение 1 месяца, общий объем введенной жидкости составил 250 мл (наполнение экспандера производилось каждый день по 7-12 мл). 11/XI-97 г. выполнена операция по устранению контрактуры правой кисти. Иссечены грубые рубцы на тыльной поверхности в области пястно-фаланговых и проксимальных межфаланговых суставов, основных и средних фаланг II-V пальцев. Рубцы грубые, спаяны с подлежащими глубокими структурами (кости, суставы, сухожилия). Удален экспандер. Лоскут был мобилизован по лучевому и локтевому краю, растянут. После этого дистальная часть лоскута рассечена на 4 части, соответственно раневым дефектам тыльной поверхности II-V пальцев. Лоскуты подшиты узловыми швами, наступило полное приживление. Проводилась активная и пассивная разработка движений в суставах пальцев кисти. Достигнут полный объем движений в пястно-фаланговых суставах. Clinical example: B. 3., 24 years old, and. b. The flaps are hemmed with interrupted sutures, complete engraftment has begun. Active and passive development of movements in the joints of the fingers of the hand was carried out. The full range of movements in the metacarpophalangeal joints has been achieved.
Таким образом, предлагаемый способ обеспечивает надежное покрытие суставов и сухожилий на тыльной поверхности кисти и пальцев. Растянутый с помощью экспандера кожный покров по соседству с дефектом имеет явные преимущества в функциональном и косметическом плане: по цвету, толщине, структуре он соответствует кожному покрову неповрежденной кисти, никогда не подвергается пигментным изменениям, нередко свойственным трансплантируемым лоскутам, и что особенно важно, данный лоскут сохраняет иннервацию. Важным преимуществом предлагаемого способа является то, что он позволяет восстановить полноценный кожный покров кисти одномоментно на всех пальцах, без предварительного создания искусственной синдактилии. Это сокращает количество оперативных вмешательств, и следовательно, длительность пребывания больного в стационаре. Кроме того, исключается использование донорских участков для выкраивания лоскутов на ножке. Способ дает возможность получить надежные функциональные и косметические результаты. This reduces the number of surgical interventions, and therefore, the length of the patient’s stay in the hospital. In addition, the use of donor sites for cutting flaps on the leg is excluded. The method makes it possible to obtain reliable functional and cosmetic results.
