Abstract
Description
The invention relates to medicine, namely to ophthalmology, and can be used for cavity eye operations (extraction [phacoemulsification] of cataract with IOL implantation, vitrectomy and other vitreoretinal operations), for intracameral administration, in order to achieve and maintain mydriasis, and also treatment and diagnosis of eye diseases.
There is a method of intraoperative achievement of mydriasis during cataract extraction with IOL implantation (Zubareva LN Technology: Surgery of traumatic cataract with IOL implantation in children. Moscow, 1993, p. 17). The method consists in the fact that during the operation, an aqueous solution of 1% mesatone in an amount of 0.25-0.5 ml is introduced into the anterior chamber of the eye.
The disadvantage of this method is that at certain stages of the operation, when there is a significant passage of the irrigation solution through the eye chamber (for example, hydrodissection, aspiration and irrigation during phacoemulsification and leaching of the lens masses), the mydriatic is washed out of the eye, and pupil constriction may occur. Often this requires repeated administration of mesatone.
The objective of the invention is to achieve effective and prolonged mydriasis throughout the surgical and diagnostic procedures.
The technical result is to reduce the number of complications during surgery, the creation of persistent mydriasis in the treatment and diagnosis of eye diseases.
The technical result is achieved by the fact that in the method of medical expansion of the pupil in ophthalmology, including the use of mydriatics, according to the invention, mydriatics are used in the form of a 0.001 - 2% solution in a viscoprotector in an amount of 0.1-1.0 ml. At the same time, during abdominal eye operations, a 0.001 - 1% solution of mesatone or 0.001 - 0.01% solution of adrenaline in the viscoprotector is injected into the anterior chamber of the eye, and in the treatment or diagnosis of eye diseases 0.01 - 2% solution of mesatone, 0.01 - 2 % solution of atropine or 0.01 - 0.1% solution of adrenaline in the viscoprotector is instilled into the conjunctival cavity of the eye.
Viscoprotectors are viscous solutions of various substances: methyl cellulose derivatives (Occucoat [Eye World, v. 2, N 3, p.62, 1997], Visiton 1 [RF patent N 971047863), sodium hyaluronate (Provisc, Healon), chondroitin sulfate sodium (Viscoat). The main purpose of using viscoprotectors is to protect the corneal endothelium from injury during surgery.
The advantage of this method is that the viscoprotector is a viscous solution and therefore the release of mydriatic dissolved in it into the environment occurs gradually, over a fairly long time, because the used volume of 0.1-1.0 ml is located in the anterior chamber of the eye (protecting the corneal endothelium from surgical trauma and enveloping the iris) throughout the operation or in the conjunctival sac throughout the procedure. This leads to the fact that the mydriatic is much more slowly washed out of the tissues of the eye or conjunctival cavity, and therefore, with a smaller amount of mydriatic, an effective and prolonged mydriasis is achieved.
The concentration of mesatone from 0.001 to 1% and adrenaline from 0.001 to 0.01% during abdominal eye operations is due to the fact that a lower concentration does not cause pupil expansion, and a higher one damages the corneal endothelium. The concentration of mesatone is 0.01 - 2%, atropine 0.01 - 2% and adrenaline 0.01 - 0.1% in the treatment and diagnosis of eye diseases due to the fact that a lower concentration does not have the necessary effect, and a higher one has a toxic effect on nerve fibers and corneal epithelium.
The method is as follows, when performing abdominal surgery (cataract phacoemulsification with IOL implantation, vitrectomy, and other vitreoretinal surgeries), during the operation, a 0.001 - 1% solution of mesatone or 0.001 - 0.01% adrenaline solution in the viscoprotector in the amount of 0.1-1.0 ml. The operation continues further using standard technology.
One of the embodiments of the invention is that in which, during treatment and diagnosis of eye diseases, a 0.01 - 2% solution of mesatone, 0.01 - 2% solution of atropine or 0.01 - 0.1% solution of adrenaline are instilled into the conjunctival cavity Viscoprotector, the procedure continues further using standard technology.
EXAMPLE: Patient A., 53 years old, with a diagnosis of the right eye: immature complicated cataract, open-angle operated initial compensated glaucoma. During the operation, cataract phacoemulsification with IOL implantation, the pupil was dilated using the proposed method. At the same time, a 0.1% solution of mesatone in a viscoprotector - Visiton 1, in an amount of 0.5 ml was injected into the anterior chamber of the eye. During the operation and throughout its duration, mydriasis adequate to the surgical requirements was achieved. As a result, the operation was quick and without complications. The postoperative period was uneventful. The loss of endothelial cells did not exceed 5-6% in the follow-up period up to 3 months after surgery.
EXAMPLE: Patient B., 40 years old, with a diagnosis of left eye acute iridocyclitis. In order to treat the disease, the pupil was dilated with the proposed method. At the same time, a 0.1% solution of atropine in a viscoprotector in the amount of 0.1 ml was instilled into the conjunctival cavity of the patient. After 20 minutes after instillation, the pupil expanded to 8 mm, subsequently throughout the duration of treatment, mydriasis was effectively supported by a single daily instillation throughout the duration of treatment. This allowed, along with the standard treatment regimen, to quickly stop the acute phenomena of iridocyclitis and to cure it.
