Предлагаемое изобретение относится к медицине, а именно травматологии и ортопедии. Известен способ электростимуляции остеорепарации при замедленной консолидации переломов (Ткаченко С.С., Руцкий В.В. Электростимуляция остеорепарации. Л. , 1989, стр. 9 - 17) и способ коррекции дегенеративно-дистрофических нарушений опорно-двигательной системы с помощью имплантированных световодов (Берглезов М. А., Вялько В.В., Угнивенко В.И. Методы инвазивной лазеротерапии в травматологии и ортопедии. М., 1995, стр. 6 - 7). Однако сами по себе инвазивные методики, дороговизна аппаратуры и относительная сложность выполнения процедуры не позволяют широко использовать способ в клинической практике. Вышеизложенный способ лечения нарушения репаративной регенерации применен у 12 пациентов. 8 из них страдали замедленной консолидацией переломов и ложными суставами трубчатых костей нижних конечностей, а у 4 больных имелись хронические язвы нижних конечностей на фоне посттравматических трофических нарушений и венозной недостаточности нижних конечностей. В результате проведенного лечения в сочетании с общеизвестными методиками у всех 12 пациентов имелась положительная динамика, проявляющаяся в субъективном уменьшении или исчезновении болей, уменьшении отека и признаков воспаления, активизации репаративно-регенераторных процессов и, в конечном итоге, сращении переломов и заживлении ран в оптимальные сроки. Клинические проявления однозначно подтверждали лабораторные и тепловизионные исследования, выражающиеся в быстрой нормализации анализов крови и термографической динамике зоны повреждения. Больной Т. , 42 лет, и/б N 1257, находился на стационарном лечении в 22 отделении РНИИТО им. P.P. Вредена с 20.02.96 по 22.03.96 по поводу замедленной консолидации косого перелома верхней трети левой большеберцовой кости. Травма 9.07.95, упал с лестницы. Лечился консервативно по месту жительства с диагнозом - открытый косой перелом верхней трети правой большеберцовой кости. Гипсовая иммобилизация в течение 5 месяцев с курсом физиотерапевтических процедур и лечебной физкультуры. При поступлении в клинику института у пациента в верхней трети левой голени имела место пальпаторная болезненность, определялось незначительное булавовидное утолщение и легкая отечность. Кожный покров не изменен, патологической подвижности не отмечалось, ходьба с дополнительной опорой (костыль). На рентгенограмме костей голени в 2-х проекциях фрагментарно прослеживалась линия перелома с выраженной периостальной реакцией без нарушения анатомической оси конечности. Больному проведен курс дозированной местной баро- лазерной терапии в количестве 10 процедур в комплексе с общепринятыми восстановительными методиками (ФТЛ, ЛФК, массаж, бассейн, механотерапия, неспецифические биостимуляторы). Уже после 3 процедур значительно улучшилось самочувствие, уменьшились боли и отечность голени. По окончании курса лечения была возможна ходьба с дополнительной опорой (тростью). На контрольной рентгенограмме - явления индуцированного мозолеобразования по типу интрамедиарной костной мозоли, что подтверждалось данными динамического тепловизионного исследования. Повторно осмотрен 20.05.96 - ходит без дополнительной опоры, движения в суставах в полном объеме, трудоспособен. On a roentgenogram of the lower leg bones in 2 projections, the fracture line with a pronounced periosteal reaction was not fragmented without violating the anatomical axis of the limb. A patient underwent a course of dosed local baro-laser therapy in the amount of 10 procedures in combination with generally accepted restorative techniques (FTL, exercise therapy, massage, swimming pool, mechanotherapy, nonspecific biostimulants). After 3 procedures, the state of health significantly improved, the pain and swelling of the lower leg decreased. At the end of the course of treatment, walking with an additional support (cane) was possible. On the control roentgenogram - the phenomena of induced callus formation according to the type of intramedial bone callus, which was confirmed by the data of a dynamic thermal imaging study. Re-examined 05/20/96 - walks without additional support, movement in the joints in full, able-bodied.
Больная Т., 41 год, и/б N 2095, находилась на стационарном лечении в 22 отделении РНИИТО им. P.P. Вредена с 26. 03. 96 по 17.04.96 по поводу тугого ложного сустава левой большеберцовой кости в нижней трети. Травма 5.02.95, упала на улице. Лечилась в больнице по месту жительства, где 16.02.95 наложен аппарат Илизарова. 16.05.95 аппарат снят ввиду отсутствия сращения большеберцовой кости и наложена гипсовая повязка. При поступлении определялась патологическая подвижность в нижней трети голени, невозможность осевой нагрузки. На рентгенограмме левой голени имелись явления сформированного ложного сустава. 4.04.96 произведена операция - резекция ложного сустава, металлоостеосинтез левой большеберцовой кости компрессирующей пластиной с костной аутопластикой. В послеоперационном периоде больной проведен курс дозированной местной баро-лазерной терапии в количестве 8 процедур с последующим наложением циркулярной гипсовой повязки. На фоне лечения быстро нормализовалась температура, уменьшились боли и послеоперационная отечность, стабилизировалось общее состояние пациентки. In the postoperative period, the patient underwent a course of dosed local baro-laser therapy in the amount of 8 procedures, followed by the application of a circular plaster cast. During treatment, the temperature quickly returned to normal, pain and postoperative swelling decreased, and the patient's general condition stabilized. In the postoperative period, local complications were not observed.
Больная 3., 48 лет, и/б N 1837, находилась на стационарном лечении в 22 отделении РНИИТО им. P.P. Вредена с 18.03.96 по 19.04.96 по поводу закрытого перелома обеих лодыжек правой голени со смещением отломков, варикозной болезни, хронической рецидивирующей язвы нижней трети правой голени. Травма 18.03.96, упала на улице. Страдает варикозным расширением поверхностных вен правой голени в течение 10 лет, периодически отмечала появление язв в нижней трети правой голени. В последние 4 недели имел место рецидив трофической язвы по внутренней поверхности нижней трети правой голени размером 1,5 х 1,0 см. При поступлении в клинику института произведена закрытая ручная репозиция отломков, наложение окончатой гипсовой лонгетной повязки. На фоне тепловизионного контроля проведен курс местной дозированной баро-лазерной терапии в количестве 8 процедур с применением антисептиков и мазевых повязок. Полное заживление язвы отмечалось через 2 недели с момента лечения. На 20 сутки наложена циркулярная гипсовая повязка. Контрольный осмотр через 8 недель выявил четкое формирование костной мозоли обеих костей правой голени при правильном положении отломков, рецидива варикозной язвы не отмечалось. Результаты клинического применения способа лечения нарушений репаративной регенерации опорно-двигательной системы в комплексе с общепринятыми методиками свидетельствуют о выраженных противовоспалительных, активизирующих и оптимизирующих проявлениях на фоне местной дозированной баро-лазерной терапии. Тепловизионный контроль на всех этапах лечения позволяет объективизировать процесс и проводить динамическую коррекцию терапии. Against the background of thermal imaging control, a course of local dosed baro-laser therapy in the amount of 8 procedures was carried out using antiseptics and ointment dressings. Complete ulcer healing was noted 2 weeks after treatment. On day 20, a circular plaster cast was applied. A control examination after 8 weeks revealed a clear formation of bone callus of both bones of the right lower leg with the correct position of the fragments, recurrence of varicose ulcers was not observed. The results of the clinical application of the method of treating disorders of the reparative regeneration of the musculoskeletal system in combination with generally accepted methods indicate pronounced anti-inflammatory, activating and optimizing manifestations against the background of local dosed baro-laser therapy. Thermal imaging control at all stages of treatment allows you to objectify the process and conduct dynamic correction of therapy.
