Abstract
Description
The invention relates to medicine, namely to the technology of dosage forms with bacterial preparations.
The introduction of antibiotics and sulfonamides into the clinical (gynecological) practice has led to a number of undesirable consequences, the main of which is the dissemination of antibiotic-resistant strains of bacteria and changes in the normal human microflora (dysbacterioses). In this regard, the creation of drugs with probiotics is very relevant. Considering that lactobacilli are the most physiological for use in the field of gynecology, it seems advisable to develop dosage forms for vaginal use with the inclusion of strains of lactobacilli isolated from the vagina of healthy women and having high antagonistic activity. The most convenient dosage form for this route of administration is suppositories.
Known suppositories containing as an active ingredient lactobacilli, effective for the treatment of vaginal dysbiosis. So, from the invention (application GB 2261372, A 61 K 39/02, 9/02, 1990), suppository compositions are known containing lactobacilli in an amount of at least 10 9 bacteria per suppository, using a gelatin base.
Another source (US Pat. No. 5,466,463, LAFOR., 424/433, 11/14/95) describes among many of the examples vaginal suppositories with pre-microencapsulated lactobacilli in an amount of at least 10 6 viable bacteria per suppository. The suppository is made on a gelatinous basis with the inclusion of various active and auxiliary substances, as well as eucalyptus oil, which performs an antiseptic function. In the analogs described above, the safety of bacteria during long-term storage is not ensured, and the given technological process is not perfect, since it does not ensure uniform distribution of bacteria in the suppository mass, which affects the quality of the drug.
The closest in technical essence and in the achieved result is the composition of the suppository described in the article (E.I. Molokhova, V.N. Tarasevich, V.A. Neschislyaev "Development of the composition of suppositories with lactobacterin", J. "Pharmacy", 1994 ., N 3, p. 28-32). Known suppositories for the treatment of vaginal dysbiosis contain lyophilized lactobacilli and a hydrophobic fat base, including an emulsifier in an amount of 4-6% by weight of the base.
The technical result of the invention is the creation of suppositories for the treatment of vaginal dysbacterioses with increased exposure efficiency and shelf life.
The technical result is ensured by the fact that the composition of the suppositories includes lactobacilli of the strain Lactobacillus fermentum VKMP B-7573 in the amount of (0.8-4) • 10 8 bacteria per suppository, while the hydrophobic fat base with emulsifier additionally contains 4-6 soybean oil % by weight of the base.
Highly hydrogenated vegetable oil, mainly palm kernel, cottonseed, sunflower, peanut or Vitepsol composition can be used as a hydrophobic fat base.
As the emulsifier, the sodium salt of sulfonates of higher fatty alcohols of sulfuric acid and / or monoglycerodistillate and / or triglycerol distearate can be used.
The basis of the invention are the following premises.
It is known that lactobacilli of the strain Lactobacillus fermentum VKMP B-7573 are characterized by increased adhesive activity on the endothelial cells of the vagina, therefore, their use is especially effective in the treatment of vaginal dysbiosis. Microbiological analysis shows that the nature of the suppository base significantly affects the survival of lactobacilli and, ultimately, the effectiveness of the drug. It is preferable to use hydrophobic fatty bases as providing a higher viability of lactobacilli in relation to other types of bases. Highly hydrogenated vegetable oils, such as palm kernel, cottonseed, sunflower, peanut, etc., as well as the composition "Vitepsol" (see, for example, "Technology of dosage forms", s / p L., can be used as hydrophobic fatty bases). A. Ivanova, Volume 2, M., "Medicine", 1991, p. 521). The selection of suppository components providing enhanced bioavailability and shelf life is an object of the present invention.
It is known that soybean oil in large quantities contains lecithin, which has a high affinity for the membrane of bacterial cells. Lecithin is a part of membranes and performs structural and metabolic functions. We found that when preparing a suspension of lyophilized lactobacilli in soybean oil, lecithin forms particles in the form of micelles around bacterial cells, thus forming an additional protective layer. As a result of this phenomenon, from a technological point of view, the suspension is thinner and more stable, and as a result, the high viability of lactobacilli is ensured both during use and during storage.
The novelty of the causal relationship "distinctive features - the technical result" allows us to conclude that this invention is protected.
Implementation examples.
Example 1
In the preparation of suppositories using lactobacilli strain Lactobacillus fermentun VKMP B-7573 in the amount of 3 • 10 8 bacteria per suppository. As the fat base, palm kernel oil of the highest degrees of hydrogenation was used, which, together with the emulsifier sodium salt of sulfo esters of higher fatty alcohols of sulfuric acid (5% by weight of the fat base) (known as "Emulsifier N 1") is melted in a water bath. Separately lyophilized lactobacilli are suspended in soybean oil (5% by weight of the fat base). To the resulting suspension of lactobacilli in oil with constant stirring, a molten base with an emulsifier is added. Stirring is continued until a homogeneous mass is obtained. The resulting mass is poured into the mold for suppositories of the right size and kept in the cold until they solidify.
Example 2
In the preparation of suppositories, the process is carried out similarly to that specified in example 1, except that the composition "Vitepsol" was used as the fat base for suppositories, and monoglycerodistillate was used as an emulsifier.
The production of suppositories is technologically advanced and can be implemented according to the description on equipment for producing suppositories with the latter being molded in metal molds and then packed into packaging material (foil, cellophane, etc.) or directly molded into packaging forms (using, for example, packing tape "Rotoplast").
