Известен способ лечения поллиноза, заключающийся в назначении причинно-значимого аллергена в постепенно возрастающих дозах в виде водно-солевых экстрактов перорально, который прост в применении, легко переносится, но вследствие частичного разрушения экстракта аллергена в желудочно-кишечном тракте обладает недостаточной эффективностью. Наиболее близким по эффективности к предлагаемому методу является метод парентерального введения водно-солевых экстрактов аллергенов, однако он имеет ряд существенных недостатков (1, 2). Метод травматичен, требует назначения большого числа инъекций, отличается длительностью и часто дает побочные эффекты в виде местных и общих аллергических реакций. Препараты изготовлялись в гомеопатической аптеке N53 г. Томска (лицензия N1 от 05.01.95 г.) в форме гранул (маленьких шариков из молочного сахара), которые пропитывают разведенным аллергеном таким образом, что каждая гранула соответствует капле жидкого аллергена в адекватном разведении, что обеспечивает точность дозирования препарата. Весь курс лечения занимает в среднем 2,5 месяца. Контрольные осмотры детей проводятся 1 раз в 14 дней, кроме того, всеми больными заполняются специальные листки наблюдения, в которых фиксируются дата введения аллергенов, их разведение и доза, наличие местной или общей реакции, изменения в состоянии ребенка, прием лекарственных препаратов. Темп наращивания дозы в предложенном способе лечения поллиноза у детей более медленный, чем в ранее использовавшихся, что обеспечивает адекватную защитную перестройку иммунной системы и предотвращает аллергические реакции во время лечения, что подтверждается поэтапным обследованием больных детей. Хорошие и отличные результаты получены у 73,4% детей после первого курса лечения, после двух курсов эффективность возросла до 80,0%. Практически не отмечались аллергические реакции на вводимый препарат. Лишь у одного ребенка на разведении аллергена 1:1000 появились жалобы на зуд кожи, в результате чего терапия была приостановлена на 2 дня. После исчезновения жалоб лечение продолжено с увеличением интервалов между приемами препарата и в дальнейшем протекало гладко. Больной С., 6 лет. Обратился к аллергологу в мае 1994 года с жалобами на зуд, покраснение и отечность глаз, заложенность носа, частое чихание, приступообразный кашель, одышку. Аллергологом выставлен диагноз: Поллиноз, риноконъюнктивит, пыльцевая бронхиальная астма. В течение всего периода обострения ребенок получал антигистаминные препараты, бронхолитики и сосудосуживающие препараты, но симптомы заболевания полностью не купировались. При аллергологическом обследовании методом скарификационных проб, проведенном в зимнее время, была выявлена сенсибилизация к пыльце березы. С февраля 1995 года ребенку была начата специфическая иммунотерапия аллергеном пыльцы березы, приготовленным в виде гранул сублингвальным способом. Лечение протекало гладко и было закончено к середине апреля. При осмотре ребенка в период цветения березы состояние оценено как удовлетворительное, отмечено небольшое затруднение носового дыхания, легкая гиперемия конъюнктивы. Кашля, одышки не было. В течение периода цветения березы ребенок получил короткий курс антигистаминных препаратов, на фоне которых симптомы заболевания исчезли.Example

During the entire period of exacerbation, the child received antihistamines, bronchodilators and vasoconstrictor drugs, but the symptoms of the disease did not completely stop. An allergological examination using the method of scarification tests conducted in winter revealed a sensitization to birch pollen. Since February 1995, the child was given specific immunotherapy with a birch pollen allergen prepared in the form of granules in a sublingual way. The treatment went smoothly and was completed by mid-April. When examining a child during the flowering period of birch, the condition was assessed as satisfactory, slight difficulty in nasal breathing, mild conjunctival hyperemia was noted. Cough, shortness of breath was not. During the flowering period of birch, the child received a short course of antihistamines, against which the symptoms of the disease disappeared.

На фоне проводимого лечения, кроме уменьшения интенсивности клинических симптомов заболевания, были отмечены положительные сдвиги в системе местного иммунитета - уменьшилась эозинофилия слизистой носа с 13% до 1%, содержание иммуноглобулинов A, G и SIgA в назальном секрете повысилось и приблизилось к нормальным значениям. Метод не требует госпитализации пациентов, дает высокий клинический эффект и обладает стимулирующим действием на местный иммунитет, что позволяет рекомендовать разработанную схему лечения к применению в детской аллергологической практике для лечения поллиноза.
