Abstract
Description
The invention is used in otorhinolaryngology, in particular for the treatment of sinusitis and after radical surgery (RO) on the maxillary sinus (ICP).
Currently, both metal and polymer drainages of various designs are used for the drainage of VChP. Analogs are known (A.S. 1321423 of the USSR, A.S. 1595531). Metal drainage is a gross foreign body and can cause pressure sores, and its sudden displacement during a push or impact leads to perforation of the orbital wall, in addition, it is a long single-cavity tube, often clogged with mucus and blood clots and serves only for forced drainage and forced aeration of the RF .
The objective of the invention is to create a long non-forced drainage and aeration of the ICP in the treatment of sinusitis and after RO on the ICP and the creation of persistent anastomosis in place of the natural opening.
The prototype is the drainage tube described in A.S. 1595531, made of stainless steel grade 30x13 (3x13) or 40x13 (4x13) GOST 5632-72, its length is 4 cm, diameter 2 mm. This is a cylindrical tube with a cutting edge and drainage holes at the distal end, which is worn on the Kulikovsky needle and punctured the bone tissue of the ICP endonasally. Rinsing and administration of medicinal solutions is carried out using a commercially available polyethylene catheter for the subclavian vein with an outer diameter of 2 mm, which is placed in a drainage tube.
The disadvantages of the prototype.
Metal drainage is a gross foreign body and can cause pressure sores, and its sudden displacement during a push or impact leads to perforation of the orbital wall, in addition, being a long single-cavity tube, it often clogs with mucus and blood clots and serves only for forced drainage and forced aeration of the ICP. For the introduction of medicinal solutions, a polyethylene catheter for the subclavian vein with an external diameter of 2 mm is additionally used, which cannot create a sufficiently strong jet for rinsing the ICP. In addition, such drainage cannot be applied after RO on the VChP to create a persistent anastomosis in place of the natural opening.
The purpose of the invention is to improve the design of the drainage tube, improving the quality of treatment.
The goal is achieved in that for the treatment of sinusitis and postoperative management after RO, a drainage tube for forced drainage is used on the VChP, to which an additional tube for involuntary aeration of the maxillary sinus (DTA) is additionally attached. DTA is made of 2 flexible hollow tubes of inert polyethylene. Tube 1 for forced drainage is intended for rinsing and administering medicinal solutions of RFP, its length is 10 cm, diameter 0.5 cm, its fixing part is corrugated 1/3 of the end inserted into the RFP, tube 2 is for forced aeration of the RFP, length its 1 cm, diameter 0.5 cm, fully corrugated. The corrugated part is necessary to give the desired configuration in the nasal cavity and fix the tube in the bone wall.
DTA is used as follows. After the natural opening has been expanded in any way possible, in the treatment of sinusitis or after RO, the DTA is inserted into the DTA hole in the DTA side, where the tubes 1, 2 are fixed by the corrugated part 3 in the bone wall. The part of the tube 1 protruding from the nasal cavity is cut off. Through the tube 1, if necessary, the drug solution is washed or injected, and through the tube 2, aeration of the RFP occurs naturally. Usually, for the treatment of sinusitis, the tubes are left until the patient is cured, and to create a persistent anastomosis, they are left for at least 10 days.
The advantage of the proposed drainage tube for forced drainage and non-forced aeration of the VChP is that it consists of two flexible hollow tubes. One of the tubes 1 makes it possible to rinse the RFP and inject medicinal solutions at any time without resorting to additional devices, and the diameter of the tube allows you to create the necessary pressure of the jet for washing the RFP. The presence of the second tube 2, shorter than the tube 1, allows for natural non-forced aeration of the RF. The presence of the corrugated part 3 in both tubes 1 and 2 ensures reliable fixation in the bone wall. Since the drainage tube is made of inert polyethylene, it is a lightweight construction that, even if displaced from a push or impact, does not lead to perforation of the orbital wall. In addition, the implementation of the fixing part 3 of the drainage tube in the form of corrugations allows you to create a persistent anastomosis in a natural place. Thus, when using the proposed design of the drainage tube, the quality of treatment increases, and the morbidity decreases.
