Abstract
Description
The invention relates to neurosurgery, in particular to selective catheterization of the spinal canal and intervertebral openings and local endolumbar pharmacoperfusion in the treatment of diseases of the nervous system of vertebrogenic etiology, and can also be used for intravascular manipulations for dosed administration of the drug directly into the lesion.
Diseases of the nervous system of vertebrogenic etiology, in particular sciatica, make up to 80% in the general structure of diseases of the peripheral nervous system, are associated with a long loss of working capacity, and often lead to disability. Therefore, it is obvious that the search and improvement of treatment methods for this pathology are quite relevant.
With the development of new medical technologies, a large number of various methods of treating these diseases have been developed, and more recently, minimally invasive ones, which improves the quality of life of patients. The abundance of various methods indicates that one of them is not sufficiently effective and universal and has its positive and negative sides.
59 - 5). There is a method of treating vertebrogenic pain syndromes, which consists in carrying out a course of epidural blockade by repeated punctures of the lower sacral orifice and the introduction of large volumes of drugs through a needle and syringe (VK Romanov. Treatment of radiculitis with epidural drug administration. L .: Medicine, 1971 g., pp. 59 - 5).
(David J.Allison, H.Peterson, A. Hemmigway Interventinal Radiology The NICER Year Book 1994, p. 236), There is also known a method of multiple precision puncture injection of medicinal mixtures through a needle and syringe directly into the root irritation area under the control of a computer tomograph (David J. Allison, H. Peterson, A. Hemmigway Interventinal Radiology The NICER Year Book 1994, p. 236), which is used only with foraminar lesions.
The closest to the achieved goal is the method of catheterization according to the patent of the Russian Federation 2074010, MKI 6 A 61 M 25/08, publ. 02/27/97, Bull. N 6, which allows for the manipulation of several wires on the proximal portion of the inner conductor to provide a 360 ° deviation of the controlled distal portion of the catheter around the axis of the instrument to advance the catheter through the curved sections of the vessels and into the branches.
The objective of the proposed technical solution is to create a method for selective catheterization with the location of the lesion in the spinal canal and intervertebral openings, which avoids damage to the dural sac during puncture and catheterization, avoids accidental administration of drugs under the dura mater and general toxic effects of drugs administered epidurally .
The problem is solved by introducing a rigid external conductor, advancing along it to the lesion area of the epidural catheter until a predefined deviation is accepted by the catheter in the lesion zone. Moreover, the inner conductor with an ultra-soft distal part is inserted into the catheter in advance, which, prior to the insertion, is set to the deviation from the axis of the conductor corresponding to the location of the lesion zone and returned to its original position. After that, the catheter with the guidewire is moved through the external conductor and the spinal canal of the affected vertebral foramen. Then, by rotating the proximal part of the inner conductor, its distal part is moved so that the catheter can penetrate the interconductor hole to a predetermined depth.
The introduction of an external rigid conductor through the lower sacral opening and the use of the walls of the spinal canal as a continuation of the external conductor during the advancement of the soft epidural catheter with the internal conductor inserted with an ultra-soft distal part makes it possible to guide the catheter through the lower sacral opening to the level of any affected segment of the spine.
The fact that before the extra soft distal part of the inner conductor is inserted into the soft catheter, any lateral and axial deviation from 0 to 360 o , corresponding to the previously identified location of the lesion zone, is set and returned to its original position, allows the catheter to be precisely guided to the lesion located in the intervertebral opening improve the quality of life of the patient.
The set of features is new and allows, firstly, to reduce tissue injury, which is caused by only one puncture of the spinal canal, to reduce the volume and dose of drugs, thereby avoiding the general toxic effect of drugs, eliminating damage to the dural sac and roots, and in many cases avoiding open surgical interventions, reduce treatment time, improve the quality of life of the patient.
The essence of the method is illustrated by drawings, where in FIG. 1 shows a diagram of selective catheterization of the spinal canal; FIG. 2 is a diagram of catheterization of the intervertebral foramen.
3. In FIG. 1 and 2 - external conductor 1,
The inventive method is as follows. When the lesion area is located in the spinal canal with a rigid external conductor 1, the lower sacral opening is punctured to the spinal canal, the walls of which perform the function of continuing the stiffness of the external conductor when it moves along the soft lesion of the
