Abstract
Description
The present invention relates to medicine, namely to ophthalmic surgery, and can be used when performing abdominal operations to protect the posterior corneal epithelium and the simultaneous prevention of hemorrhagic complications.
И.N 10, 1997). Known viscous solution for the protection and treatment of diseases and injuries of the cornea containing cellulose derivatives (alkyl derivatives or carboxylalkyl derivatives, or hydroxylalkyl derivatives with a globule size of 0.2-0.4 microns), buffer salts (sodium chloride, sodium disubstituted, sodium monosubstituted), glycosaminoglycans ascorbic acid and water (RF patent N 2076671, B. I. N 10, 1997).
The disadvantage of the above solution is that it cannot be used in conditions of increased bleeding. The reason for this is the presence in the solution of glycosaminoglycans, including heparin and heparin sulfate, which are strong activators of the plasminogen-plasmin system. Their presence adversely affects the performance of optical reconstructive operations on the anterior segment of the eye, such as iris plastic surgery, dissection of iridocorneal, iridocapsular adhesions - i.e. those operations in which the risk of hemorrhagic and inflammatory complications is increased, where they act as a factor that provokes bleeding.
The task of the invention is to obtain a viscous solution with protective and antihemorrhagic properties.
The technical result obtained by solving this problem is that reduced intraoperative and postoperative complications, resulting in reduced treatment time.
1,0-2,2%.The specified technical result is achieved by the fact that the viscous preparation containing cellulose derivatives, buffer salts and water additionally contains epsilonaminocaproic acid (ε - ACC), and methyl cellulose is used as cellulose derivatives, in the following ratio of ingredients, wt.%:
Cellulose - 1.0-2.2
Sodium chloride - 0.85-0.86
Sodium Phosphate
monosubstituted - 0.0046-0.005
disubstituted - 0.04
Epsilonaminocaproic acid - 0.8-1.0
Water - Else
Among the essential features characterizing the substance, the distinguishing ones are: epsilonaminocaproic acid in an amount of 0.8-1.0%; methyl cellulose, a derivative of cellulose, in an amount of 1.0-2.2%.
Between the totality of essential features and the achieved technical result, there is a causal relationship.
The introduction of 0.8-1% ε -ACA into a viscous solution significantly reduces hemorrhagic intraoperative and postoperative complications, and reduces the patient’s hospital stay. This is due to the biochemical properties of epsilonaminocaproic acid.
ε-ACK is a substance that inhibits fibrinolysis due to competition with plasmin for fibrin. ε - ACC, being an indirect inhibitor of kinins, blocks the mechanisms of aseptic inflammation.
At the same time, it is known that any surgical intervention can be considered as an injury, during which there is contact of eye tissues, rich in microvessels, with a surgical instrument. As a result of this, contact-kallikrein-kinin cascade activation of a number of enzyme-enzymatic reactions occurs. When the basal surface of microvessels comes in contact with a foreign surface (for example, collagen), which usually carries a negative charge, XII coagulation factor (Hageman factor) is activated. In turn, the XII factor is activated under the influence of proteolytic plasma enzymes: plasmin, kallikrein, etc. Thus, when factor XII is activated, coagulation enzymes, fibrinolysis (humoral pathway), and the kallikreinkinin system, i.e. aseptic inflammation mechanisms are triggered.
This cascade of enzymatic-enzymatic reactions with a predominance of the fibrinolytic potential of the blood during manipulations on the iris (iris plastic, separation of iridocorneal, iridocapsular adhesions, etc.) is especially powerfully turned on, since the walls of the microvasculature of the iris are especially rich in proactivators and plasminogen activators. As a result of this, the risk of bleeding increases due to an increase in the fibrinolytic potential of the blood, the failure of the fibrin plug occurs, and tissue edema increases. The concentration of ε - ACA (0.8-1.0%) is due to a sufficient inhibitory effect on the fibrinolytic activity of moisture in the anterior chamber of the eye, which we studied in vitro. At a concentration of less than 0.8%, the inhibitory fibrinolytic activity effect was significantly weakened. At a concentration of more than 1%, the viscosity of the solution increased during prolonged storage, which affects the technical characteristics of the drug.
The use of methyl cellulose as a derivative of cellulose to obtain a viscoelastic preparation is well known information, and the percentage of methyl cellulose is dictated by the necessary and sufficient viscosity of the resulting product.
Thus, the solution we offer is a balanced viscous composition and can be used to introduce into the cavity of the eye during cavity operations in order to protect the posterior corneal epithelium and the simultaneous prevention of hemorrhagic and inflammatory complications.
The proposed solution "Visicap" is obtained by the following technology.
Methyl cellulose is dissolved in bidistilled, deionized water, stirred, centrifuged, then filtered on membranes with a pore diameter of 0.2 μm. Then adjusted to the desired viscosity, removing excess water by heat deposition. After cooling, it is isotonized with sodium chloride, buffered by the addition of sodium phosphate mono- and disubstituted. Epsilonaminocaproic acid is then added, mixed and sterilized.
Conducted experimental studies, preclinical and clinical trials of the claimed drug "Visicap" confirmed the positive effect when used in abdominal operations with a high risk of hemorrhagic complications (plastic iris, separation of iridocorneal, iridocapsular adhesions, etc.). "Visicap" compares favorably with known analogues that do not have a similar effect or are used externally.
