Abstract
Description
The invention relates to medicine, and more particularly to ophthalmology, and can be used as a therapeutic agent for edema of the cornea, keratitis and keratoconjunctivitis, burns, sore throats, ulcers; in ophthalmic surgery for keratotomy, keratomileusis, for corneal transplants to protect the transplant, as well as for insertion into the anterior chamber of the eye in order to prevent the loss of endothelial cells.
A known solution and method for its preparation containing sulfated glycosaminoglycans (C GAG), which are obtained from the corneas of farm animals, which include the composition of keratan sulfate and chondroitin sulfate in a ratio of 2: 1 (RF patent N 2056851).
However, in some cases, the chondroitin sulfate included in the solution competitively inhibits the action of keratan sulfate, and often causes neovascularization of the graft during corneal transplants. With an increase in the concentration of chondroitin sulfate in the composite, in some cases there is inhibition of the action of the solution in the treatment of certain diseases (for example, burns, ulcers).
The objective of the invention is the creation of a dosage form with pronounced therapeutic properties for diseases and injuries of the cornea.
д.).The technical result is achieved in that the solution comprising keratan sulfate and physiological saline, according to the invention, it further includes cytochrome-C in the following ratios of components, wt.%:
Keratan sulfate - 0.5-2
Cytochrome C - 0.001-0.006
Saline - Rest
Keratan sulfate is the main acidic glycosaminoglycan of the cornea of the eye of humans and animals; according to its chemical structure it enters into the category of complex polysaccharides with a molecular weight of 20,000-50,000 Daltons. Keratan sulfate has the following biological properties - it determines the transparency of the cornea, regulates its water-salt metabolism, takes an active role in corneal repair, by directly affecting collagen fibrillogenesis. Keratan sulfate actively affects the normalization of the cornea metabolism as a whole, reduces the permeability of the blood-ophthalmic barrier, which is especially important during corneal transplantation. By regulating water-salt metabolism, it prevents corneal edema after surgical interventions, which is especially valuable for the preservation of endothelial cells during various abdominal operations (cataract removal, IOL implantation, etc.).
Keratan sulfate is included in the solution in a concentration of from 0.5 to 2%, since a decrease in the concentration below 0.5% does not lead to a therapeutic effect, the decongestant effect is reduced. An increase in concentration over 2% can cause side effects in the form of an allergy.
Cytochrome-C is a respiratory enzyme and is secreted from the heart muscle of cattle and pigs (FS 42-25 34-88). This enzyme has a pronounced effect on the binding of free radicals, which accumulate in large quantities during diseases and injuries of the cornea, especially during thermal burns and other extensive injuries. This enzyme is introduced into the solution in a concentration of from 0.001 to 0.006%. At a concentration below 0.001%, free radical binding does not occur. With an increase in concentration above 0.006%, allergic phenomena (conjunctival edema) can be observed.
Thus, the proposed solution is a balanced composition and can be used both in the form of instillations and for introduction into the cavity of the eye.
The proposed solution is prepared as follows.
Obtaining keratan sulfate.
Диализат лиофилизируют и делают навески с нужной концентрацией, а именно от 0,5 до 2 мас.%.To 1 kg of the cornea of farm animals washed with 0.02 M phosphate buffer, add 50-55 ml of 1.25 g of papain. The mixture was incubated at 60-65 ° C for 4 hours, after which it was boiled for 10-15 minutes, the precipitate was separated by centrifugation, and the clear hydrolyzate was dealized against a mixture of sodium chloride and ethanol in a 7: 3 ratio. 0.0125 g of β-hyaluronidase is added per 1 liter of obtained dialysate and the solution is continued to incubate at 60-65 ° C for 4 hours with constant stirring. The hydrolyzate is again boiled, the precipitate is separated by centrifugation. The clear hydrolyzate is finally dialyzed against a mixture of sodium monochloride and ethanol and finally against distilled water. The dialysate is lyophilized and weighed with the desired concentration, namely from 0.5 to 2 wt.%.
