Abstract
Description
The invention relates to medicine, in particular clinical oncology, and can be used to treat solid malignant tumors of various locations. The known method (Kogan E.A., Nevolskikh A.A., Zharkova N.N., Loshchenov V. B. Morphological and pathogenesis of damage to malignant tumors during photodynamic therapy. Arch.pat., N 6, p. 73-76 , 1993), which consists in the fact that a patient with a malignant tumor is injected intravenously with a photosensitizer (PS) based on hematoporphyrin derivatives, which has a tropism for a malignant tumor and increases its sensitivity to laser radiation. After reaching the maximum difference in the accumulation of PS between the tumor and the surrounding unaffected tissue (for known FSSD this time is 24-72 hours), photodynamic therapy is carried out (irradiating the pathological area with a laser, the radiation of which is absorbed by the drug, which causes PDT).
The main disadvantage of this method is that the mechanism of action of PS based on hematoporphyrin derivatives (hereinafter referred to as FSSD) is based on the destruction of tumor vessels and cells located near them. This is due to the fact that FSSD accumulates mainly in the vascular wall of the tumor and in nearby cells. Therefore, if in the tumor there are areas depleted in blood vessels, then they will not be destroyed as a result of laser exposure. This leads to the fact that the undamaged parts of the tumor (which are usually more malignant, since they are capable of anaerobic respiration and can use the products of necrosis of dead cells as a result of PDT) begin to grow rapidly and a new formation may be more resistant not only to PDT, but also to other treatments.
Известен также способ ФДТ (Loh C.S., Bedwell J., MacRoben A.J., Krasner N., Phillips D., Bown S.G. Photodynamic therapy of the normal rat stomach: a comparative study between disulphonated aluminum phtalocyanine and 5-aminolaevulinic acid. A PDT method is also known (Loh CS, Bedwell J., MacRoben AJ, Krasner N., Phillips D., Bown SG Photodynamic therapy of the normal rat stomach: a comparative study between disulphonated aluminum phtalocyanine and 5-aminolaevulinic acid. - J. Cancer 1992, v.66, p.452-462), in which a photosensitizer is introduced into the patient’s body, which does not linger in the walls of blood vessels and mainly accumulates in tumor cells remote from blood vessels (photosensitizer of the cellular action of FSKD), in particular disulfurized aluminum phthalocyanine. Then the pathological section is irradiated with therapeutic laser radiation, exciting the photosensitizer. With this method of PDT, massive tumor necrosis occurs, and the possibility of oxygenation of tissues, and hence PDT, is maintained throughout the entire therapeutic process. However, with this method and a non-optimal dose of laser irradiation, cells of the deep layers of the tumor, located near unbroken vessels and not receiving sufficient radiation energy, can break away from tissue tumors (since the thermal effect during laser irradiation causes an increase in cell mobility) and migrate through the blood stream. This can lead to generalization of the tumor process.
Thus, none of the known methods of PDT provides complete tumor necrosis, especially for tumors in the late stages of development.
The aim of the invention is to ensure the greatest efficiency of PDT due to the most complete destruction of the tumor.
The goal is achieved by the method of photodynamic therapy of malignant tumors, including the introduction of photosensitizers into the patient’s body and irradiation of the pathological area with optical radiation, characterized in that PDT is carried out by the combined action of vascular cell photosensitizers, while the vascular action photosensitizer is introduced and the corresponding therapeutic radiation is applied to it not earlier than the introduction of a photosensitizer of cellular action and therapeutic irradiation of the pathological area with radiation corresponding to this photosensitizer.
The proposed method for the treatment of malignant tumors is as follows. The FSKD photosensitizer is introduced, which mainly accumulates in cell structures distant from blood vessels. Then, immediately after administration, the dynamics of its accumulation in various parts of the tumor tissue and its contrast with healthy tissue are analyzed.
After reaching the therapeutic concentration of FSKD, laser irradiation is performed with the dose necessary to destroy the main volume of the tumor. Then, immediately after irradiation, FSSD is administered in a concentration sufficient to destroy the main great vessels supplying the tumor with subsequent laser exposure. The dose and method of exposure are determined for each specific tumor individually.
4 series of experiments were carried out on white mice (males) with Erlich transplantable carcinoma. In the first series, disulfurized aluminum phthalocyanine (tip-FSKD) was administered at a dose of 4 mg / kg body weight for PDT. Upon reaching the therapeutic concentration of the drug in the tumor, PDT was performed using radiation from a krypton laser with wavelengths of 647 and 675 nm for 15 minutes at a dose of 500 J / cm 2 , after 48 after which tumor necrosis was detected by morphological studies.
In the second series, Fotofrin-ll (FSSD) was used in the same concentration and dose of laser radiation.
In the third series, the method of sequential administration of FS was used. First, half-dose FDCS was administered, then irradiation was performed, then half-dose FSSD was administered, and repeated irradiation was performed. The full dose of laser irradiation corresponded to the doses in the two previous cases.
During all procedures, parallel monitoring of the dynamics of the accumulation of FSSD in various parts of the tumor was performed.
The fourth series is the control group.

Morphological analysis results:

The intact tumor tissue is located mainly along the intact vessels. In the lumen of blood vessels and in the foci of necrosis, an accumulation of polynuclear lymphocytes.
Group 2 - invasive carcinoma, massive necrosis, occupying 2/3 of the tumor volume, with karyerexis, extending to great depths down to the bone tissue. A distinct vascular reaction in the form of necrosis of the vascular wall. In places of invasion of muscle tissue, carcinoma cells are preserved. In the foci of necrosis, silhouettes of necrotic vessels and the accumulation of polynuclear lymphocytes.
3rd group - invasive carcinoma, total necrosis of the tumor and adjacent muscle tissue with pronounced karyerexis. On the periphery - pronounced macrophage-eosinophilic - lymphocytic reaction. In the depths, individual dystrophically altered carcinoma cells were found.
4th group - invasive carcinoma with minor foci of necrosis in the center of the lobular layers of cancer cells, sprouting into the surrounding muscles. Vascular and cellular reaction to necrosis is absent.
Thus, the results of morphological analysis showed that the method of PDT, implemented in the third group, gives a significantly more complete therapeutic effect than in the first and second.
