Abstract
Description
The invention relates to dental materials, and more specifically to materials that stimulate osteogenesis of bone tissue, and can be used in dentistry to restore bone tissue in various pathological processes.
[1, 2]. Known materials that stimulate osteogenesis of bone tissue containing hydroxyapatite powders with different particle diameters connected by fibrinogen preparations and collagen [1, 2].
These materials rather well stimulate osteogenesis, however, their adaptation in bone tissue is complicated, since substances containing fibrinogens are used as binders, which in some cases lead to various kinds of complications.
Closest to the proposed material is the material [3] containing hydroxyapatite, fibronogen, thrombin and calcium chloride. This material, due to this composition, stimulates bone growth in the cavities or cracks filled with it, however, its use is not always effective, since it uses fibrinogen and thrombin as a binder, which increase blood coagulation, as a result of which healing in the postoperative period can to proceed with some complications.
It is known that in inflammatory processes, not only does blood clotting increase significantly, but antithrombin activity also decreases. This can lead to a qualitative change in fibrogen with its loss in the form of fibrin and firbrinoid. All this contributes to intravascular coagulation, clogging of capillaries and precapillaries with impaired microcirculation, leading to the formation of blood clots. Blood clots are a breeding ground for the development of microorganisms, which leads to an inflammatory process and, as a result, to non-inflammatory inflammation. All this complicates the primary adaptation of the material in the bone tissue.
The technical result of the invention is to minimize the spread of the inflammatory process in the postoperative period by reducing blood coagulation and improving regional microcirculation, which is achieved by the fact that the material for filling bone tissue based on hydroxyapatite with tricalcium phosphate with a ratio of 3 - 4: 1 75 - 80%, lincomycin 16 - 17%, trypsin 1 - 0.5%, direct-acting anticoagulants 7 - 7.5%, and it has a pasty consistency.
Comparative analysis with the prototype showed that the proposed material based on hydroxyapatite contains powders or granules of hydroxyapatite with tricalcium phosphate with a ratio of 3 - 4: 1, as well as trypsin and lincomycin, direct-acting anticoagulants. Thus, the proposed solution meets the criterion of "novelty."
The invention consists in the following.
As you know, odontogenic jaw cysts are always foci of chronic infection. With hypergical inflammation, not only does blood coagulability increase significantly, but antithrombin activity also decreases, qualitative changes in fibrinogen occur with its loss in the form of fibrin and fibrinoid. All this contributes to intravascular coagulation, blockage of capillaries and pericapillaries with microcirculation disturbance, leading in some cases to necrotic inflammation. In order to act on the blood fibronolytic system as an activator of fibrinolysis, trypsin is introduced into the material to fill the cavity formed after removal of the cyst containing hydroxylapatite, which also has a proteolytic effect.
A direct-acting anticoagulant is introduced into the material, since it delays the transition of fibrinogen to fibrin, increases the Z-potential and fibrinolysis.
Blood clots that accumulate in a wound can be a good breeding ground for microorganisms. To prevent inflammatory processes on the bones, an antibiotic is introduced into the material - lincomycin, which has an antibacterial effect against gram-positive microorganisms, staphylococci, streptococci and selectively (osterotropically) acting on bone tissue.
The percentage of substances included in the material was selected empirically when tested on animals, and a paste-like consistency was selected to prevent leaching of the material from the wound.
