Несмотря на значительные достижения в профилактической стоматологии, проблема лечения осложненного кариеса остается актуальной. Для создания удобного доступа к каналам полость зуба раскрывали, широко создавали правильный контур и правильное осевое направление. Стенки кариозной полости по возможности сохраняли, крышу полости зуба полностью удаляли. Обнажив устья корневых каналов, их воронкообразно расширяли, тем самым удаляли нависающие дентинные карнизы над устьями корневых каналов. После прохождения канала файлом или римером №10 (15) расширяли верхушечное отверстие до 25 размера. Определяли рабочую длину апекслокатором, подтверждали ее рентгенологически. Проводили антисептическую обработку корневого канала. Чресканально заполняли периапикальный очаг деструкции смесью костно-пластического материала «Оргамакс» с пастообразным материалом для пломбирования каналов зубов, в составе которого содержится гидроокись кальция, в соотношении 10:1. В качестве пастообразного материала для пломбирования каналов зубов могут использоваться такие пасты как «Каласепт», «Метафикс» и др. Пластичную массу вносили на кончике зонда в устье корневого канала и с помощью каналонаполнителя и файла №20 выводили за верхушку корня в околоверхушечный очаг деструкции челюстной кости. Эта манипуляция повторялась 2-3 раза. Количество выводимого за верхушку корня костнопластического материала соответствовало размерам очага деструкции кости на рентгенограмме. После введения пластичной массы за верхушку корневой канал расширяли на 2 размера для создания апикального упора. Затем корневой канал подвергали антисептической обработке, высушивали адсорбентом и пломбировали методом латеральной конденсации гуттаперчей с «АН+». Проводили рентген-контроль и восстанавливали коронку зуба методом прямой реставрации гелиокомпозитом.To create convenient access to the channels, the tooth cavity was opened, the correct contour and the correct axial direction were widely created. The plastic mass was introduced at the tip of the probe into the mouth of the root canal and, using the canal filler and file No. 20, they were taken out from the apex of the root to the apical site of the destruction of the jaw bones. This manipulation was repeated 2-3 times. The amount of osteoplastic material displayed at the apex of the root corresponded to the size of the bone destruction site on the radiograph. After the plastic mass was introduced beyond the apex, the root canal was expanded by 2 sizes to create an apical focus. Then the root canal was subjected to antiseptic treatment, dried with an adsorbent and sealed by lateral condensation of gutta-percha with "AN +".
