В известных способах лечения генерализованного туберкулеза с целью поддержания достаточных концентраций действующего начала в очагах поражения практикуется длительное назначение массивных доз туберкулостатиков, что зачастую чревато выраженными токсикоаллергическими реакциями. В нашем же случае использован водорастворимый пролонгированный препарат, который в силу лизосомотропности использованной матрицы накапливается в лизосомах макрофагов (в том числе и в инфицированных микобактериями туберкулеза макрофагах гранулем), то есть на внутриклеточном уровне, в непосредственной близости от находящегося там возбудителя заболевания. В этом случае есть все основания говорить не только о куммуляции, но и о частичной адресации лекарственного препарата, т.е. об избирательной доставке действующего начала непосредственно в определенную популяцию клеток (макрофаги), являющихся местом локализации инфекта. В связи с повышенной кислотностью лизосомального содержимого происходит гидролиз пролонгированного препарата с высвобождением изониазида в активной форме и созданием вследствие этого высоких концентраций изониазида на внутриклеточном уровне, т.е. в микроокружении микобактерий туберкулеза. Этим и объясняется увеличение терапевтической эффективности. Снижение же токсичности обусловлено замедленным высвобождением продуктов метаболизма изониазида из внутриклеточного депо и, главным образом, снижением курсовой дозы туберкулостатика. This explains the increase in therapeutic efficacy. The decrease in toxicity is due to the slow release of isoniazid metabolism products from the intracellular depot and, mainly, to a decrease in the course dose of tuberculostatic.

Синтез пролонгированного препарата осуществлен с использованием принципиальной схемы получения подобных соединений, описанной в исследованиях К.П. Хомякова и соавторов, в два этапа:

В результате этого существенно упрощается процедура синтеза пролонгированного препарата, а также увеличивается его стойкость к гидролизу в физиологических условиях, что особенно важно для решения поставленных нами задач.We offer technological modifications:

Это является фактором, повышающим резистентность организма к микобактериальной инфекции и позволяющим достичь лечебного эффекта при меньшей дозе противотуберкулезных препаратов.
