Abstract
Description
The invention relates to medicine, namely to obstetrics.
М. 1991, с.63-64). A known method for diagnosing the condition of the fetus during pregnancy and childbirth, consisting in electropletismographic recording of changes in uterine blood supply during labor (F.A.Syrovatko, N.V. Pilipenko. Study of the dynamics of uterine blood supply in late miscarriages and premature births with high-frequency plethysmography. Abstracts dokl. 3rd Congress of Obstetricians and Gynecologists of the RSFSR. M. 1991, p.63-64).
However, in the known method, the condition of the fetus is judged only by the level of total blood supply to the uterus, which is associated with the saturation of the body with hormones, in particular estrogen, as well as the state of vascular tone. The hormonal level is very stable and changes slowly, gradually. Therefore, in the event of an acute catastrophe with the fetus (fetal asphyxia due to tight cord entanglement around the fetal neck, placental abruption, uterine rupture, etc.), the general level of uterine blood supply remains unchanged for a long time and its performance does not allow to determine the degree of circulatory disturbance in the mother-fetus system.
Н. С. М. Акушерство и гинекология, 1976, N 5, с.5-9). There is also a method of assessing the state of the circulatory system of the fetus, which consists in the simultaneous registration of electrocardiograms of the mother and the fetus (V. N. Demidov, V. S. Tsvigun, E. M. Demidova. Obstetrics and gynecology, 1976, N 5, p. 5-9 )
However, this method does not allow to identify the initial signs of impaired functioning of the circulatory system of the fetus and determine the degree of asphyxiation.
As a prototype, a method for assessing the state of the circulatory system of the fetus was adopted, which consists in simultaneously recording the electrocardiograms of the mother and the fetus (ed. St. N 624602, class MKI A 61 B 5/02, 1978). In order to identify the degree of fetal hypoxia, the magnitude of the mismatch in the rhythm of the electrocardiograms of the mother and the fetus is determined.
However, the known method has several significant disadvantages.
No isolation (recognition) of all fetal cardiocomplexes was carried out, especially in the case of their coincidence in time with the appearance of maternal cardiocomplexes. In these cases, the true value of the cardiac signal was not used, but its average value, which significantly reduced the diagnostic accuracy.
The condition of the fetus was judged by the magnitude of the mismatch of the rhythms of the electrocardiograms of the mother and the fetus without specifying specific indicators in various obstetric situations.
Thus, the known method does not allow with sufficient accuracy to diagnose the condition of the fetus during pregnancy and childbirth, which is especially important in threatening conditions.
The objective of the invention is to provide a more accurate method for diagnosing the condition of the fetus during pregnancy and childbirth.
The essence of the invention lies in the fact that in the method for diagnosing the condition of the fetus during pregnancy and childbirth, including the simultaneous recording of the electrical activity of the heart of the mother and fetus, the first and second derivatives of the joint electrocardiogram are measured, the time corresponding to the maximum correlation of the cardiac signals of the mother and the fetus is recorded, and in the absence of correlations diagnose a threatening condition of the fetus.
As a result of using the proposed method, the following technical result is achieved.
It is possible to isolate all the cardiac signals of the fetus, even in cases of their coincidence in time with the cardiac signals of the mother.
The method allows for mathematical analysis of the true cardiac signals of the fetus and to determine the correlative relationships of the mother and the fetus.
All this makes it possible to identify with a high degree of accuracy even insignificant impairments of the fetal vital functions, timely determine the degree of impairment of the fetal condition, diagnose the threatening condition of the fetus and conduct appropriate treatment measures, which helps to reduce perinatal mortality and the birth of full-fledged children.
Improving the accuracy of diagnosis of the state of the fetus is achieved due to the fact that as a diagnostic criterion use the correlation value of the cardiac signals of the mother and the fetus, which is determined by measuring the first and second derivatives of the combined electrical activity of the heart of the mother and the fetus.
The method is as follows.
A pregnant woman or a woman in labor on the front wall of the abdominal cavity in the area of the projection of the uterus impose sensors to register the joint electrical activity of the heart of the mother and fetus. The sensors are connected to appropriate devices and then the electrical activity of the heart of the mother and the fetus is recorded. Then measure the first and second derivative of the joint electrocardiogram of the mother and the fetus, fix the time corresponding to the maximum correlation of cardiac signals of the mother and the fetus. In the absence of correlation, a threatening condition of the fetus is diagnosed.
Example. A mother of 24 years old, was admitted to maternity hospital N 72 in the first stage of labor with a normal course of delivery, with whole amniotic fluid and opening of the cervix by 5 6 cm. With almost full opening of the cervix, amniotic fluid flowed and progressive advancement of the fetal head along birth canal. At the moment of lowering the fetal head into a narrow part of the small pelvis, there was a lack of correlation of the electrocardiograms of the mother and the fetus. The threatening condition of the fetus was diagnosed, in connection with which the operation was performed: the obstetric forceps were placed on the fetal head, which was in a narrow part of the pelvic cavity, and an urgent delivery was performed. A girl was removed in a state of hypoxia, weight 3450 g, height 51 cm. The cause of hypoxia was a three-fold tight tight entanglement of the umbilical cord around the neck. As the fetal head advances through the birth canal due to the tension of the umbilical cord, there is a disturbance in blood circulation in the placenta-fetus system and cerebral circulation in the fetus, which caused fetal hypoxia.
