Abstract
Description
The device relates to medical equipment and is intended to affect the segmental apparatus of the spinal cord in the treatment of pain syndromes.
Known multi-electrode catheter block for stimulation of the spinal cord, containing located at the same distance electrodes connected by terminals to the receiver [1]
Such a catheter block does not have a channel for introducing drugs into the epidural space, which does not allow its wide use.
A device is known that contains a catheter in the form of two coaxial tubes with a rounded working end, contact elements and conductors [2] A tubular electrode is placed in the channel of the inner tube, one end of the conductor is inserted into its inner hole, and flexible holes exit through the holes in the side wall of the outer tube stabilizing elements that facilitate the insertion of the end of the electrode into the endolumbar space of the spine.
However, the known device, due to the lack of a cavity in the inner tube, does not allow the administration of drugs into the epidural or subdural space of the spinal cord simultaneously with electrical stimulation, which narrows its functionality.
The objective of the invention is the expansion of the functionality of the electrode for therapeutic effects on the segmental apparatus of the spinal cord.
The problem is solved in that in the epidural neuroelectrode containing the catheter, in the form of two coaxial tubes with a rounded working end, contact elements and conductors, coaxial tubes are installed with the formation of a cavity between them, conductors connected to the current source and contact elements are placed in the cavity, made in the form of rings placed on the outer surface of the catheter and on the rounded end, while the non-working end of the inner tube is made with the possibility of connection with the container for the drug atm, and the working end being open.
The presence of a cavity in the inner tube, the working end of which is open, and the non-working one is connected to the dispenser capacity, allows drugs to be administered against the background of an electropulse effect on segments of the spinal cord. Contact elements made in the form of rings and placed on the outer surface of the catheter and the end significantly improve contact with the nerve structures, and an increase in their total surface prevents damage to the nerve tissue by electric pulse current. Since the conductors are located between the coaxial tubes and are isolated from each other, one pair of them can be connected to the generator for bipolar effects on the nerve structures of the spinal cord, and the second pair with recording equipment for removing spinal potentials. All this greatly expands the functionality of the neuroelectrode.

The epidural neuroelectrode contains a hollow
The epidural neuroelectrode uses as follows.
The level of the affected segments of the spinal cord relative to the spinal column is determined. An epidural space of the spinal cord is punctured with a Tougley needle, at this level the working end of the neuroelectrode is inserted through the lumen of the needle into the epidural space. After checking the adequacy of the location of the working end of the electrode, the needle is removed, and the electrode is fixed to the fascia. The conductors are connected to the device for electrical stimulation, and the inner tube catheter to the capacity of the dispenser. Medicines (painkillers, antispastic, stimulating and others) are administered around the clock into the epidural space of the spinal cord against the background of electrical stimulation. The type of drug, the parameters of the pulsed current, as well as the duration of treatment are determined individually, it depends on the nature and severity of the disease. If necessary, the device for electrical stimulation and a dispenser with a medicinal substance are implanted into the patient’s body, for which the second end of the neuroelectrode is carried out along the subcutaneous tunnel to the place chosen for implantation.
The medical and clinical effectiveness of the proposed neuroelectrode consists in the possibility of multi-contact electrical effects on the posterior columns of the spinal cord in combination with a direct effect on its neuronal structures of drugs.
