Широко известны способы диагностики пиелонефрита, основанные на лабораторных методах исследования, главным из которых является обнаружение белка в моче, определение которого основано на его коагуляции химическими реактивами (например, азотной, сульфосалициловой кислотами), физическими методами (нагревание мочи и др.)
В качестве прототипа принят клинический анализ мочи, включающий определение таких показателей, по которым можно судить о наличии пиелонефрита: цвет мочи, прозрачность, относительная плотность, наличие белка, присутствие в осадке мочи эритроцитов, лейкоцитов, цилиндров, клеток почечного эпителия. However, using the prototype cannot establish the calculous form of pyelonephritis.
Цель изобретения повышение точности способа, а также сохранение атравматичности и упрощение способа при выявлении калькулезной формы пиелонефтрита. Предложенный способ позволяет выявлять калькулезную форму пиелонефрита, не прибегая к данным рентгенологического и ультразвукового иcследований, и подтвержден в 100% случаев. Сущность изобретения заключается в том, что при выявлении калькулезной формы пиелонефрита каплю мочи высушивают в течение не менее 40 мин на стеклянной подложке и при наличии полной непрозрачной кристаллизации капли в сочетании с положительной реакцией на наличие белка в моче диагностируют калькулезную форму пиелонефрита. Оптимальными признаками предложенного способа является высушивание капли мочи на стеклянной подложке и характер кристаллизации ее поверхности. У больного берут произвольно выделенную порцию мочи в емкость, дают возможность ей отстояться в течение 10-15 мин. Наносят каплю мочи (0,01-0,02 мл) на поверхность предметного стекла при помощи пипетки-дозатора и оставляют при комнатной температуре до полного высушивания (не менее 40 мин). За это время отливают исследуемую профильтрованную мочу в 2 пробирки по 3 мл. В опытную пробирку прибавляют 6-8 капель 20%-ного раствора сульфосалициловой кислоты. На темном фоне сравнивают контрольную пробирку с опытной. Помутнение в опытной пробирке указывает на наличие белка, пробу считают положительной. В заключение сопоставляют картину кристаллизации образца мочи и качественной реакции на белок в пробирке. Способ диагностики пиелонефрита позволяет: избегать трудоемкого обследования больных и, в частности, устранить факторы, связанные с рентгеновским облучением больного и персонала; уточнить диагноз при рентгенонегативных камнях почек; в виду широкой доступности может быть использован как скрининговый метод во всех случаях обнаружения белка в моче.
